Evaluation of simultaneous bilateral femoral distraction osteogenesis with antegrade intramedullary lengthening nails in achondroplasia with rhizomelic short stature: a retrospective study of 15 patients with a minimum follow-up of 2 years.

BACKGROUND AND PURPOSE: Bilateral femoral distraction osteogenesis in patients with achondroplasia is insufficiently reported. We aimed to perform the first study that exclusively analyzed simultaneous bilateral femoral distraction osteogenesis with motorized intramedullary lengthening nails via an antegrade approach in patients with achondroplasia focused on reliability, accuracy, precision, and the evolving complications. PATIENTS AND METHODS: In this retrospective singlecenter study we analyzed patients with achondroplasia who underwent simultaneous bilateral femoral lengthening with anteg…

A Hand-Palm Technique for Localization of the Greater Trochanter Tip During Proximal Femoral Nailing: A Prospective Pilot Validation Study.

INTRODUCTION: An accurate localization of the tip of the greater trochanter (GT) is required for proper skin incision placement and correct entry point creation during proximal femoral nailing (PFN) for intertrochanteric femur fractures. Direct palpation of the GT tip may be unreliable in obese or edematous patients, potentially resulting in extended incisions and improper entry points. This study describes the "hand-palm technique," a surface landmarking method for localization of the GT tip. MATERIALS AND METHODS: The technique was applied in two variations based on the surgeon's glove size

Clinical and Functional Outcomes of Closed Intramedullary Interlocking Nailing in Extra-articular Distal Tibial Fractures: A Prospective Observational Study.

INTRODUCTION: Extra-articular distal tibial fractures are difficult to manage because of poor soft-tissue coverage, limited vascularity, and the tendency for malalignment and delayed union. Closed intramedullary interlocking nailing has become an increasingly preferred treatment option because it preserves fracture hematoma and periosteal blood supply. AIMS: The aim of the study was to evaluate the clinical and functional outcomes of closed intramedullary interlocking nailing in extra-articular distal tibial fractures. MATERIALS AND METHODS: This prospective observational, single-center stud

Intramedullary nailing compared with primary total hip arthroplasty for pertrochanteric fractures in autonomous elderly patients : a prospective randomized study.

AIMS: The optimal surgical strategy for pertrochanteric fractures in elderly patients remains debated. While intramedullary nailing is the standard of care, primary total hip arthroplasty (THA) has been proposed to enhance early recovery. This study compared functional outcomes, complications, and survival between both techniques in autonomous elderly patients. METHODS: This monocentric randomized controlled trial was conducted between January 2015 and October 2017. Patients aged ≥ 70 years with pertrochanteric fractures and a pre-fracture Parker score ≥ 5 (indicating outdoor walking ability)

Technical considerations and early results of magnetic compressive intramedullary nailing for tibial and femoral shaft non-unions: a case series.

BACKGROUND: Non-union of femoral and tibial shaft fractures remains a challenging complication following surgical fixation. Magnetic intramedullary nails (mIMNs) allow controlled compression and distraction, offering a potential alternative to traditional exchange nailing or external fixation. However, limited clinical data exist regarding their efficacy and complications in treating long bone non-unions. This case series presents early outcomes and technical considerations of compressive magnetic intramedullary nailing for femoral and tibial shaft non-unions. CASE DESCRIPTION: We conducted a…

The Effect of Transphyseal Rigid Tibial Nailing on Proximal Tibial Morphology in Pediatric Patients With at Least 1 Year of Growth Remaining.

INTRODUCTION: Tibial shaft fractures are common in the pediatric population. As patients approach skeletal maturity, rigid intramedullary nailing of these fractures is a potentially advantageous option, allowing rigid fixation and earlier mobilization and weight-bearing. However, potential injury to the open proximal tibial physis in patients with remaining growth remains a concern. This study sought to determine the rate of growth disturbance following rigid, transphyseal tibial nailing of tibial shaft fractures in patients with open physes. METHODS: This was a retrospective study of skeleta

Fibular nailing is associated with fewer complications in high-risk ankle fractures: A propensity score-matched case-control study.

PURPOSE: Ankle fractures are common, with higher complication rates in elderly, comorbid patients and those with complex injury patterns. Minimally invasive techniques such as intramedullary fibular fixation have gained interest for their potential to reduce morbidity. This study compared outcomes of fibular nailing versus plating in high-risk ankle fractures and aimed to identify the patient profile most likely to benefit from intramedullary fixation. METHODS: A retrospective, propensity score-matched case-control study was conducted at a level I Trauma Center. Patients with low-energy rotat

Positional difference in deep femoral artery during intramedullary nailing for proximal femoral fractures: a within-subject comparative study.

BACKGROUND: We aimed to evaluate the positional changes in the deep femoral artery (DFA) during intramedullary nail surgery. We hypothesized that the femur and DFA may be closer due to the medial compression of the thigh on a traction table during the surgery. METHODS: This within-subject comparative study included 20 patients with femoral trochanteric fracture (five males, 15 females; mean age: 83 ± 10.2) who underwent intramedullary nailing. Procedures were performed by five orthopedic surgeons. We performed computed tomography (CT) scans on the healthy thigh in the neutral and surgical lim

Anatomical safety and clinical feasibility of percutaneous supraclavicular humeral nailing via the Neviaser portal: a combined cadaveric and clinical study.

BACKGROUND: Antegrade intramedullary nailing via a deltoid-splitting approach is standard for humeral shaft fractures but may compromise the rotator cuff tendon. The Neviaser portal provides a superomedial entry through the supraspinatus muscle belly; however, its safety for nailing, particularly the risk of suprascapular nerve injury, remains uncertain. We aimed to quantify the safety margin between the Neviaser nailing trajectory and the suprascapular nerve in cadavers and to assess the clinical feasibility of the technique. METHODS: Eight fresh-frozen cadaveric shoulders underwent standard

Diaphyseal forearm fractures without fluoroscopy: Titanium elastic nailing system (TENS): A paediatric case series from Nepal.

In resource-limited settings such as rural Nepal, the lack of intra-operative fluoroscopy often forces clinicians to choose between suboptimal conservative management or referral to distant centres. Our case series presents the feasibility of the titanium elastic nailing system for displaced childhood both-bone forearm fractures using tactile feedback and portable radiography instead of a C-arm. While the rate of open reduction was higher than in a fluoroscopy-guided series (2/9 for radius, 4/9 for ulna), all cases achieved stable intramedullary placement and clinical union. This approach offe

Comparative Study of Functional and Radiological Outcomes of Antegrade Versus Retrograde Titanium Elastic Nailing for Pediatric Ulnar Diaphyseal Fractures.

Background Pediatric forearm fractures account for approximately 40% of childhood skeletal injuries, with ulnar fractures being particularly prevalent. Titanium Elastic Nailing (TEN) offers an effective minimally invasive option for these injuries; however, the optimal nail entry point - antegrade (olecranon) versus retrograde (distal ulna) - remains debated. This study compared functional and radiological outcomes, complication profiles, and implant-related morbidity between the two approaches. Methods A retrospective comparative study was conducted from May 2023 to May 2025 at the R.L. Jalap

Care Controversies: Proximal Tibia Fracture in a 29-Year-Old Woman: Locked Lateral Plating Versus Intramedullary Nailing.

A 29-year-old woman with no medical history presented after being a pedestrian struck by a motor vehicle. She sustained an APC2 pelvic ring injury, a closed olecranon fracture, and a closed proximal tibia fracture ( Fig. 1 ). Clinical evaluation is notable for significant edema and early serous fracture blister formation overlying the proximal tibia fracture. The patient is neurovascularly intact. Imaging of the tibia demonstrates an extra-articular proximal tibia fracture. The patient underwent application of anterior pelvic external fixation and knee spanning external fixator within 24 hours

Intramedullary Threaded Nail Versus Dorsal Plate and Screw Fixation of Comminuted Metacarpal Shaft Fractures: A Cadaveric Biomechanical Study.

PURPOSE: Indications are expanding for the use of intramedullary threaded nail (IMTN) fixation in metacarpal fractures. We hypothesized that IMTN fixation would have superior bending stiffness compared to dorsal plate and screw fixation and provide sufficient stability to withstand cyclic loading comparable with early range of motion. METHODS: Index, long, ring, and small finger metacarpals were harvested from matched pair cadaveric hands. A midshaft 1.75 mm transverse gap was created to simulate a comminuted fracture. Specimens were randomized into two groups for fixation with either a retro
