It is important to identify the cause of hypercalcaemia to give the best possible treatment. The primary treatment is to remove the underlying cause and rehydration. When the parathyroid hormone (PTH) level is elevated the most common causes are primary hyperparathyroidism and thiazides. The treatment is operation and discontinuation of the medicine. Cinacalcet can be used to lower the serum calcium level preoperatively. When the PTH level is low the most common causes are dehydration, malignant disease and treatment with calcium. The hypercalcaemia in malignant disease can be treated with pre…

Ectopic calcifications and even bone formation have been linked to GNAS gene mutations. A 51-year-old Caucasian female had been diagnosed of pseudo-pseudohypoparathyroidism (PPHP) in 1989. She has always had normal serum parathyroid hormone, calcium, and phosphorus levels. A non-contrast computed tomography of the head was done in 2013 and it showed finely speckled subcutaneous calcifications in the high convexity of the head. Cutaneous exploration did not show any abnormality. We herein report an unusual case of late-onset scalp calcifications in a patient with PPHP.

Minimally invasive parathyroidectomy (MIP) is an operative approach for the treatment of primary hyperparathyroidism (pHPT). Currently, routine use of improved preoperative localization studies, cervical block anesthesia in the conscious patient, and intraoperative parathyroid hormone analyses aid in guiding surgical therapy. MIP requires less surgical dissection causing decreased trauma to tissues, can be performed safely in the ambulatory setting, and is at least as effective as standard cervical exploration. This paper reviews advances in preoperative localization, anesthetic techniques, an…

This report describes a 59-year-old man who presented 10 days after surgery for hyperparathyroidism with pulmonary edema, mildly abnormal echocardiogram, and elevated free T4 level with suppressed thyroid-stimulating hormone level. Follow-up documented resolution of the elevated free T4 level with 24-hour thyroid scan and uptake at 60 days showing reduced uptake and normal gland anatomy. Previous case reports have been made of thyroiditis at variable time intervals after parathyroidectomy and are here reviewed. This entity represents an under appreciated potential morbidity associated with par

Calciphylaxis is a rare, but life-threatening, disease, mostly seen in patients with renal failure, especially those undergoing dialysis. It is characterized by violaceous tender areas of cutaneous plaques, necrosis, and eschar formation, mostly involving toes and fingers, but rarely the penis. Peripheral pulses are mostly preserved. The parathyroid hormone (PTH) level is elevated, along with raised calcium phosphorus product. There is radiological evidence of blood vessel and soft tissue calcification. Predisposing factors are obesity and diabetes. It is rarely encountered by a urologist and …

Idiopathic hypoparathyroidism is a rare endocrinopathy whose origin is unknown, and which is characterized by a deficiency of parathyroid hormone causing low serum calcium and high serum phosphorus concentrations. Disorders of ectodermal tissues are common in this pathology, and enamel hypoplasia is the most frequently found dental pathology. This article describes the case of a 9-year-old child with hypoparathyroidism that had gone undiagnosed for 6 years. The permanent dentition showed enamel hypoplasia, delayed eruption, and cessation of root formation (i.e., shortened roots), resulting fro…

Primary hyperparathyroidism is due to abnormal parathyroid hormone production of various causes. Bone and kidney are target organs of the disease (1). A 40 years old female patient from Dire Dawa presented with leg pain of three months duration. Serum calcium, alkaline phosphatase and creatinine were elevated X-rays show renal stone, bone resorption and brown tumor. Abdominal ultrasound showed left hydronephrosis. Color and power Doppler ultrasound of the neck showed hypo echoic retro thyroid vascular mass. The epidemiological and clinical feature, x-ray and ultrasound findings are briefly dis…

Parathyroidectomy cures the vast majority of patients with primary hyperparathyroidism (HPT). While bilateral neck exploration has been the mainstay of therapy, recent advances in technology have revolutionized the field making a focused approach to parathyroidectomy a reality. The development of accurate pre-operative localization with sestamibi scanning, effective unilateral, endoscopic and video-assisted techniques, and rapid intra-operative parathyroid hormone (PTH) testing have made it possible to cure patients with the least amount of surgical dissection possible. In this paper, we will …

The patient (age 46; female) had hypocalcemia without characteristic physical findings of Albright hereditary osteodystrophy in July, 1998. She had no evidence of tetany. Her blood test revealed elevated serum concentration of parathyroid hormone (PTH) in spite of hypocalcemia and hyperphosphatemia. Her Ellsworth-Howard test presented decreased excretion of both phosphate and cAMP in urine after PTH injection compared with normal range of response, indicating end-organ resistance to the actions of PTH. Finally, Ellsworth-Howard test, together with lack of clinical manifestations, resulted in d…

There has been a shift in the referral patterns in recent years that has resulted in increasing numbers of patients being referred to surgeons with a diagnosis of hypercalcemia rather than primary hyperparathyroidism. The surgeon must perform a thorough history, including medications, and laboratory assessment, including serum calcium and parathyroid hormone measurements. A 24-hour urinary calcium excretion should be routinely ordered to exclude FHH. After the diagnosis of primary hyperparathyroidism is made, preoperative localization studies will benefit 78% to 90% of patients, with sestamibi…

Renal bone disease results in significant morbidity in patients with end-stage renal failure. Renal osteodystrophy is a mixture of different conditions with different pathogenetic factors involved. Most recently a new form of renal bone disease, adynamic bone disease, has emerged as the most frequent finding on bone biopsy of patients on dialysis therapy. The etiology of this new entity is not fully understood, but relatively low levels of intact serum parathyroid hormone are frequently associated with this disorder and may play an important role in its pathogenesis.

A 34-month-old girl presented with a clinical picture of Kenny syndrome. The clinical manisfestations included growth retardation, persistent open anterior fontanelle, prominent forehead, mid-facial dysplasia, hypocalcemic tetany and characteristic radiologic skeletal abnormalities. Serum levels of immunoreactive parathyroid hormone (PTH) remained inappropriately low during hypocalcemic episodes in the neonatal period; indicating that hypocalcemia was a consequence of the hypoparathyroid state. This is the first reported case of Kenny syndrome in Taiwan. The literature on the pathogenesis, eti…

The primary hyperparathyroidism (PHP) is a complex of nosological symptoms associated with disturbances of calcium-phosphate equilibrium and bone metabolism, caused by excessive secretion of parathyroid hormone. In the past regarded as rare entity, today it is arousing great interest due to ever more frequent diagnosis of it in the stage of subtle psychoneurological changes or in the asymptomatic period. Untreated PHP leads to the development of a number of organ complications, and even to a life-threatening state, that is hypercalcemic crisis. Surgical operation is the treatment of choice.

Common thyroid and parathyroid disorders present with reversible neurologic signs and symptoms affecting the central and peripheral nervous system, musculature, and mental function. Patients with thyrotoxicosis may have myopathy, spasticity, seizures, and multiple psychiatric symptoms. A deficiency of thyroid hormone also causes muscle weakness and may be accompanied by reversible muscle hypertrophy or movement disorders. The chronic hypercalcemia that develops secondary to hyperparathyroidism produces many psychiatric and cognitive symptoms, as well as a reversible myopathy. Calcium deficienc

We report a rare case of Turner's syndrome coexisting with thyrotoxicosis and parathyroid adenocarcinoma resulting in hyperparathyroidism. This 45-year-old female patient presented with primary amenorrhea and poor development of secondary sexual characteristics. She also experienced repeated bone fractures, and body weight loss. Multiple risk factors of osteoporosis developed simultaneously in this patient. The interesting interactive relationship of hypercalcemia between thyrotoxicosis and primary hyperparathyroidism was noted. The best choice was surgery for both cases. Calcium supplements a

Pseudohypoparathyroidism (PHP) is a heterogenous syndrome resulting from bone and renal resistance to parathyroid hormone (PTH). Hypocalcemia and hyperphosphatemia occur despite elevated PTH levels. Head and neck manifestations of PHP include tetany, laryngeal spasm, and ectopic calcification in the eye, dentition, subcutaneous tissue and brain. The first case of malleus head fixation caused by ectopic calcification in a PHP patient is reported. Normalization of serum calcium and phosphate levels does not appear to retard or correct this abnormality. Ossiculoplasty or amplification is currentl…

Aluminium is present in senile plaques and in neurofibrillary tangle-bearing neurones in brains of cases of Alzheimer's disease. Evidence linking aluminium absorption to calcium metabolism prompted a case-control study of physiological markers of calcium homeostasis in patients with Alzheimer's disease. Mean plasma 25-hydroxyvitamin D concentration was lower in cases than in controls but no difference in median parathyroid hormone concentrations or other evidence of altered calcium metabolism was found. The difference in 25-hydroxyvitamin D concentrations may be secondary to differences in exp…

In a patient with hyperparathyroidism and chronic renal failure due to polycystic kidney disease, a finding of destroyed sellar and parasellar structures and hyperprolactinemia suggested the diagnosis of invasive pituitary prolactinoma. At surgery no tumor was found, and pathological examination of the sphenoid bone revealed a parathyroid bone lesion (brown tumor) as well as ectopic prolactinoma in the clivus. This patient demonstrates that a tumor may develop in ectopic pituitary tissue. The combination of radiographically abnormal sellar structures with pituitary hormone hypersecretion shoul
