In vitro radiosensitivity of fibroblasts derived from patients with retinoblastoma and abnormalities of chromosome 13.

In vitro x-ray survival experiments were performed on fibroblast strains derived from nine patients with sporatic unilateral retinoblastoma and 26 patients with hereditary retinoblastoma. Fibroblasts derived from patients with hereditary retinoblastoma were significantly more radiosensitive than those derived from patients with sporatic retinoblastoma. The authors hypothesize that the increased in vitro radiosensitivity observed in some hereditary retinoblastoma cell strains is a reflection of an, as yet, uncharacterized defect in DNA repair or post-irradiation DNA replication. X-ray sensitivi

Autosomal dominant preretinal vascular loops.

Preretinal vascular loops are a congenital malformation usually occurring in otherwise normal fundi. These anomalies consist of vessels, most commonly arteries, that spiral into the vitreous and return to the otherwise normal retinal vascular system. The authors present two members of a family with preretinal vascular loops and two members with other vascular anomalies. The inheritance pattern in this group is autosomal dominant.

Presumed tuberculous maculopathy.

A solitary choroidal mass underlying the macula of a patient with active pulmonary tuberculosis involuted under antituberculosis therapy. Angiographic study revealed large blood vessels within the mass and secondary changes in overlying retinal vessels. These retinal vessels, despite their ophthalmoscopic configuration, had no demonstrable anastomosis with the vascular tree of the choroid.

Advanced pigmentary retinal degeneration: an ultrastructural study.

A case of advanced pigmentary retinal degeneration is described. Posterior subcapsular cataract and a remnant of the anterior hyaloid system on the posterior lens surface were found. Extensive dispersion of pigment and organization of the vitreous base were present. Gliosis was seen in the retina, and no photoreceptors were evident. The retinal blood vessel walls were extensively hyalinized, with cell debris and deposition of basement-membrane-like material, which may have originated from perivascular pigment-laden cells of pigment epithelial origin. The pigment epithelium showed extensive pro

Microsurgical management of retinal detachment in eyes with pupillary membranes.

A one-stage approach to the combined problem of retinal detachment and pupillary membranes, aided by fiberoptic endoillumination and microscopic viewing through a corneal contact lens, permits simultaneous removal of the pupillary membrane and cortical vitreous along with microsurgical examination of the peripheral retina and closure of all breaks. Four cases of complicated retinal detachment that were treated in this manner are presented. In two, the presence of pupillary membranes was further complicated by vitreoretinal traction. Although it is a departure from traditional techniques of ret

Uveal melanoma in association with multiple malignancies. A case report and review.

A 66-year-old man had a pigmented choroidal lesion in his right eye. Over a subsequent period of 8 1/2 years, the lesion showed slight growth. At autopsy, the diagnosis was spindle B melanoma of the choroid without evidence of systemic metastasis. A very unusual feature of this case was the presence in this patient of seven primary malignant tumors and five benign neoplasms. In addition to the choroidal melanoma, these included three distinct pulmonary carcinomas, leiomyosarcoma of the stomach, adenocarcinoma of the sigmoid colon, and basal cell carcinoma. The literature on multiple malignanci

Long-term follow-up of ocular histoplasmosis treated by argon laser photocoagulation.

Sixty-eight eyes in 67 patients with a histoplasmic neovascular membrane treated with Argon laser photocoagulation between 1972 and 1978 are reviewed. Factors affecting visual outcome and causes of visual failure are discussed. Twenty of 68 eyes (nearly 30%) experienced a recurrent or new SRNVM after initially successful treatment. Final evaluation of Argon laser photocoagulation for histoplasmic neovascular membrane must await the outcome of the Macular Photocoagulation Study.

Visual results following scleral buckling for retinal detachments sparing the macula.

Scleral buckling was performed on 116 eyes with rhegmatogenous retinal detachments that spared macular function preoperatively. The retina was reattached in 97% of these eyes after one operation. The remaining cases were repaired successfully by a second procedure. Eight percent of the 90 eyes with preoperative vision of 20/30 or better experienced some visual loss postoperatively despite successful retinal reattachment. In 4% of these 90 eyes, the final vision was less than 20/40. Macular pucker was the most common cause of postoperative reduction in vision.

A safe temporary transcameral IOL stay suture.

A serious problem arises in retinal detachment surgery when intravitreous gas is needed and an iris-supported artificial lens is in place. Using a very small-gauge long needle to cross the anterior chamber just anterior to the lens, monofilament suture can safely be brought across the chamber by threading it through the lumen of the needle and then removing the needle. The process can be repeated in a parallel position to create a mattress suture with no risk of lens-endothelial touch. The suture can be removed easily in the office when the gas has been absorbed. This suture may be useful also

Siderosis from a retained intraocular stone.

A penetrating intraocular stone caused a retinal detachment that was repaired successfully. The patient returned 18 months later with siderosis bulbi. The patient was followed by serial neurosensory tests for the next three years. The electro-oculogram (EOG) showed the greatest abnormality, while the electroretinogram (ERG) and dark adaptation, although initially affected, showed no further deterioration. Surgical procedures on this eye included linear extraction of the siderotic lens, anterior vitrectomy, and irrigation of a hyphema. Fluorescein angiogram revealed a siderotic deposit on the r

A histopathologic study of macular cysts and holes.

Forty-four eyes from 39 patients with either lamellar macular holes (17 eyes), full-thickness macular holes (18 eyes), and/or macular cysts (9 eyes) were studied histopathologically. Lamellar and full-thickness holes were frequently found in eyes with a history of trauma or ocular surgery. Diabetes mellitus was the most common condition associated with macular cysts. Four lamellar holes and four full-thickness holes occurred in eyes with senile macular degeneration. Six lamellar holes, eight full-thickness holes, and one macular cyst developed on an idiopathic basis. Residual cystoid macular e

Laser treatment of choroidal neovascular membranes in angioid streaks.

Eight patients with active extrafoveal choroidal neovascular membranes related to angioid streaks were studied. One eye of each of four patients was treated with argon laser. Two patients received bilateral treatment. Four patients received multiple treatments at different times for recurrence of neovascular membranes. Two patients were not treated. The visual acuity of the treated eyes remained the same or improved posttreatment, with one exception. The majority of untreated eyes lost vision. The vast majority of the treated eyes showed a marked improvement in central visual field posttreatme

Proliferative retinopathy associated with incontinentia pigmenti.

An 8-year-old white female presented with retinal vascular changes of fibrovascular proliferation and avascularity of the peripheral retina that suggested a diagnosis of incontinentia pigmenti. These findings prompted a systemic evaluation, and the features of this disease were confirmed, including a history of recurring vesicles on the lower extremities of her sister in infancy. Associated systemic findings in the patient included alopecia, apigmented skin lesions, conical teeth, and a confirmatory biopsy. This case illustrates that a clinical recognition of eye findings can be of assistance

Ocular manifestations of relapsing polychondritis.

The clinical, ultrasonographic, fluorescein angiographic, and immunological features of a case of relapsing polychondritis in a 46-year-old female are described. Acute bilateral sclerouveitis with multiple retinal pigment epithelial defects and sensory retinal detachment were present on initial examination. Antibodies to type II collagen were present in serum samples taken from the patient during the acute and convalescent phase of the disease. A possible relationship of this disorder to the phylogenetic development of the eye is discussed.

Pars plana vitrectomy for asteroid hyalosis.

Asteroid hyalosis is a common, usually asymptomatic, benign vitreous disease affecting older individuals. A case report of an aphakic patient who developed anterior segment complications with no visualization of the fundus is presented. A pars plana anterior and posterior vitrectomy was performed using the Ocutome system. An excellent technical result with moderate restoration of vision was achieved. Preoperative evaluation with ultrasound was helpful. Pathology confirmed the diagnosis of calcium-containing lipids. Other indications for vitrectomy for asteroid hyalosis include retinal detachme

Status of high-dose chemotherapy for breast cancer: a review.

The purpose of this review is to analyze the current status of high-dose chemotherapy (HDCT) with autologous stem cell transplantation for patients with breast cancer. Current results from the major prospective phase 2 and phase 3 trials in metastatic breast cancer (MBC) and high-risk primary breast cancer (HRPBC) are reviewed. Prognostic factors and future research directions are also discussed. The encouraging results of phase 2 trials suggested a benefit for HDCT in HRPBC and some categories of patients with MBC. Some investigators have argued that patient selection might have been a critic
