Risk factors for hepatitis C on the Texas-Mexico border.

OBJECTIVES: Our clinical experience suggested that hepatitis C virus (HCV) infection in this Texas-Mexico border area might have features, especially risk factors, that differ from some other areas of the United States. Therefore, we conducted a prospective analysis to investigate the epidemiology, risk factors, and certain other characteristics of HCV infection in the El Paso region. METHODS: During a 2-yr period, individuals with a positive HCV serology were considered as "patients" and those with a negative hepatitis serology panel were "controls." A questionnaire survey was conducted in p

An analysis of a community-acquired pathogen in a Kentucky community: methicillin-resistant Staphylococcus aureus.

CONTEXT: Community-acquired methicillin-resistant Staphylococcus aureus (CA-MRSA) infections have been reported in patients with recognized predisposing risk factors in several cities in the United States and across the world. Reviewing risk factors in adult patients with CA-MRSA in Kentucky has not been reported. OBJECTIVE: To determine the risk factors of 15 patients with CA-MRSA in Louisville KY, to compare the sensitivities of each pathogen and to recommend management. SETTING: An infectious diseases private practice in Louisville, KY. MATERIALS AND METHODS: This is a case series of pat

Characteristics of self-cutters among male inmates: association with bulimia and dissociation.

It was examined whether bulimia and dissociation are common in male self-cutters, as has been found in female self-cutters. The subjects were 796 male inmates of a juvenile prison. A self-reporting questionnaire was used to assess self-cutting, histories of psychoactive substance use, problem behaviors, and traumatic life events in the subjects. The Adolescent Dissociative Experience Scale and the Bulimia Investigatory Test of Edinburgh were also used. Subjects were divided into two groups: self-cutting and non-cutting. Questionnaire responses and dissociation and bulimia assessments were comp

Bacteremic pneumococcal pneumonia: a prospective study in Edmonton and neighboring municipalities.

Bacteremic pneumococcal pneumonia (BPP) is an important disease that should be frequently re-evaluated due to changes in demographics and recommended treatment. We conducted a prospective study from 2000 to 2002 in adults aged 17 years and over who presented to any of 6 hospitals and 1 freestanding emergency room in Edmonton, Alberta, with signs and symptoms compatible with pneumonia, a chest radiograph interpreted as pneumonia by the attending physician, and a positive blood culture for Streptococcus pneumoniae. We identified 129 patients with BPP, for an overall incidence of 9.7/100,000 pers

Self-reported personality traits and disorders (DSM-IV) and risk of criminal recidivism: a prospective study.

Assessment and management of criminal offenders require valid methods to recognize personality psychopathology and other risk and protective factors for recidivism. We prospectively explored the association between dimensional and categorical measures of personality disorder (PD) measured with the DSM-IV and ICD-10 Personality Questionnaire (DIP-Q, Ottosson et al., 1995) and registered reconvictions in adult offenders. One hundred and sixty-eight offenders consecutively referred for pre-sentencing forensic psychiatric evaluation in Sweden during 1995-1996 completed DIP-Q self-reports. The subj

Homelessness and high-dosage buprenorphine misuse.

AIM: To determine whether intravenous drug users (IDUs) are more likely to misuse high dosage buprenorphine (HDB) if they are homeless. DESIGN: We carried out a cross-sectional study using data collected from HDB users between 1998 and 1999. Data were collected by use of a structured questionnaire with questions about demographic characteristics, and use of HDB and other substances. IDUs were considered to be homeless if they did not live on their own or with their parents or friends. SETTING: IDUs were recruited from three centers for the treatment of drug users, three health care networks,

Poverty, social exclusion and health in Portugal.

People in Portugal have never been so healthy. Nevertheless, there are great differences in health status between social groups and regions. In 1994, Portugal was the country with the second worst level of inequality in terms of income distribution and with the highest level of poverty in the European Union (EU). Poverty in Portugal affects mainly the elderly and women (especially in single parent families). Beyond these groups, there are the children, the ethnic minorities and the homeless. Substance abusers, the unemployed, and ex-prisoners are also strongly affected by situations of social

High-risk behaviors during incarceration in African-American men treated for HIV at three Los Angeles public medical centers.

OBJECTIVES: This paper describes research that examined the association between high-risk sexual and drug-using behaviors during incarceration and HIV infection for African-American men receiving HIV care at three public medical centers in Los Angeles County (LAC), California. METHODS: A case-control study was conducted in which 305 HIV-infected African-American men and 305 neighborhood controls, ages 20 to 49, were frequency-matched by age. RESULTS: After controlling for anal sex while not incarcerated, we found no association between anal sex during incarceration and HIV (odds ratio [OR],

Women inmates' risky sex and drug behaviors: are they related?

The large concentration of female illicit drug users in state correctional facilities prompted an examination of the associations among different types of drug use and sexual risk factors related to human immunodeficiency virus (HIV) among women inmates. A consecutive sample of 805 women felons admitted to the North Carolina Correctional Institution for Women between July 1991 and November 1992 was interviewed. Of these inmates, 651 had complete information on relevant characteristics. Of the women inmates, 73% had used drugs prior to incarceration; most women were crack smokers only (33%), fo

Risk factors for violent behavior among incarcerated male psychiatric patients: a multimethod approach.

A multimethod approach that included demographic, criminal offense, drug use, neuropsychological, Rorschach, psychiatric diagnosis, and psychopathy characteristics was used to evaluate 131 incarcerated male psychiatric inpatients. Each criminal offense was ranked from nonviolent to severely violent, and participants were classified as having lifetime histories of either high or low violent behavior. Univariate analyses revealed 12 characteristics which independently discriminated inmates with lifetime histories of high or low violent behavior (married, non-Caucasian race, Axis I psychotic diag

Antibody to hepatitis E virus in HIV-infected individuals and AIDS patients.

Antibody to hepatitis E virus of IgG class (anti-HEV IgG) is regularly detected in industrialized countries, where HEV is non-endemic, at levels not exceeding 2-3%; seropositive individuals are often found in certain groups of patients and professionals exposed to an increased risk of blood-borne infections. The present study was aimed at the identification of anti-HEV IgG in patients with human immunodeficiency virus (HIV) infection, including acquired immune deficiency syndrome (AIDS), living in Russia and Belarus, an area of low anti-HEV prevalence with a moderate spread of HIV infection an

Spitzka and Spitzka on the brains of the assassins of presidents.

Although four American Presidents have been assassinated (Lincoln, Garfield, McKinley, Kennedy), only the assassins of Garfield (Charles Julius Guiteau) and McKinley (Leon Franz Czolgosz) were tried, convicted, and executed for their crime. In 1882 Edward Charles Spitzka, a young New York neurologist with a growing reputation as an alienist, testified at the trial of Guiteau. He was the only expert witness who was asked, based on his personal examination of the prisoner, a direct question concerning the mental state of Guiteau. Spitzka maintained the unpopular view that Guiteau was insane. In

Liver disease and cell-mediated immunity in hepatitis-associated antigen (HAA) carriers.

As the incidence of liver disease in hepatitis-associated antigen (HAA) carriers has not been defined and it has been postulated that continuing liver disease is associated with incompetence of the cell-mediated immune system, a prospective study was undertaken to examine both these points. An increased incidence of HAA carriers was found in a prison population (1.3%). Eighteen of these subjects gave informed consent to further study with liver function tests, liver biopsy, and testing of cell-mediated immunity with dinitrochlorobenzene (DNCB) skin sensitization. Liver function tests were norma

Adults with problematic opioid use released from jail: Mutual help groups and medication support contribute to a greater likelihood of opioid abstinence.

BACKGROUND: Although medications for opioid use disorder (MOUD) improve outcomes for persons with opioid use disorder (OUD), it is unclear whether augmentation with mutual help groups (MHGs), such as 12-Step programs, enhances abstinence following jail release. This study examined how MHG and MOUD together affect abstinence in persons with problematic opioid use in the 6 months post-incarceration. METHOD: We subset to 328 adult participants who completed baseline, 3- and 6-month follow-ups. All screened positive for problematic opioid use upon discharge from jail, primarily in Chicago. Partic

Influences of paternal incarceration on children's maltreatment and polyvictimization risks.

BACKGROUND: Paternal incarceration is associated with child maltreatment. However, clarity on its influence across types is needed. Additionally, maltreatment exposures may co-occur and how paternal incarceration influences children's polyvictimization is not yet known. OBJECTIVE: To investigate the influences of paternal incarceration on childhood emotional, physical, and sexual maltreatment by caregivers, as well as their exposure to multiple forms. PARTICIPANTS AND SETTING: Adolescents in the National Longitudinal Study of Adolescent to Adult Health were followed from Wave I (1994-5) (ave

Beyond burnout: occupational risk pathways linking stress, sleep, and multisystem morbidity in correctional officers.

BACKGROUND: Correctional officers work in security-intensive environments characterized by organizational strain, threat exposure, shift work, staffing shortages, and restricted autonomy. Psychological outcomes have been most studied, yet sleep disruption, cardiometabolic risk, musculoskeletal pain, and fatigue are accumulating but often examined separately, obscuring multisystem occupational morbidity and prevention priorities. OBJECTIVE: To synthesize mental and emerging physical health risks among correctional officers using an integrated occupational risk pathways framework linking upstre
