Surgical treatment of complex scapular fractures: a single centre prospective observational study.

BACKGROUND: Conservative management is traditionally preferred for scapular fractures, but recent studies suggest that surgical intervention may improve outcomes in selected patients. This study evaluates functional and imaging outcomes, complication rates, and under-reported factors, such as duration of surgery and time from injury to definitive fixation, in surgically treated patients. METHODS: This prospective observational study included 12 patients who underwent surgery for scapular fractures between 2016 and 2022. Inclusion criteria were complex scapular fractures, defined as displaced

Lipoprotein(a) reduction with PCSK9-targeting pharmacotherapy: A prospective real-world registry study.

BACKGROUND AND AIMS: Lipoprotein(a) is an independent cardiovascular risk factor with limited therapeutic options. Proprotein convertase subtilisin/kexin type 9 inhibitors (PCSK9i) reduce lipoprotein(a), but comparative real-world data across available agents are scarce. We aimed to evaluate lipoprotein(a) reduction with PCSK9i in a large prospective registry. METHODS: We analysed 1112 patients from a prospective single-centre PCSK9i registry with available baseline lipoprotein(a) measurements and follow-up data. Patients received alirocumab, evolocumab, or inclisiran according to clinical pr

Fracture Risk Stratification by REMS: A 5 Years Prospective Longitudinal Multicentre Study.

Osteoporosis is a skeletal disorder characterized by low bone mass and deterioration of bone microarchitecture. Current predictive tools, mainly based on DXA-BMD, do not full capture all the factors contributing to bone deterioration. By combining information on bone quantity (T-score) and skeletal fragility (Fragility Score, FS), the Radiofrequency Echographic Multi-Spectrometry provides an integrated measure of fracture susceptibility known as fracture risk stratification. We aimed To evaluate the ability of REMS fracture risk stratification through a 5 years prospective longitudinal multice

Pulmonary exacerbations in patients with genetically confirmed PCD: a prospective observational multicentre study.

BACKGROUND: Primary ciliary dyskinesia (PCD) is a rare genetic disorder characterized by deficient mucociliary clearance and development of chronic lung disease. Pulmonary exacerbations (PEx) in chronic lung diseases increase morbidity and lung function decline, but their frequency in PCD has been understudied. We aimed to prospectively determine the annual frequency of PEx in PCD and identify related risk factors. METHODS: In a multicentre, observational study conducted in 11 centres from seven countries, we prospectively collected data from a well-described patient cohort with genetically c

Camera-Seismocardiography Based Cardiac Triggering for Magnetic Resonance Imaging: A Prospective Study.

Cardiac Magnetic Resonance (CMR) imaging relies on electrocardiogram (ECG) triggering to eliminate motion artifacts caused by cardiac contractions, but ECG triggering faces challenges such as waveform distortion caused by magneto-hydro-dynamic effects and switched magnetic gradient fields. Although camera-based photoplethysmography (Camera-PPG) triggering is unaffected by the magnetic field, there is a significant physiological delay caused by the pulse arrival time between cardiac contraction and blood volume changes at the facial skin, hence it cannot reserve more time for scanning preparati

ProstNFound+: A Prospective Study using Medical Foundation Models for Prostate Cancer Detection.

PURPOSE: Medical foundation models (FMs) offer a path to build high-performance diagnostic systems. However, their application to prostate cancer (PCa) detection from micro-ultrasound ( μ US) remains untested in clinical settings. We present ProstNFound+, an adaptation of FMs for PCa detection from μ US, along with its first prospective validation. METHODS: ProstNFound+ incorporates a medical FM, adapter tuning, and a custom prompt encoder that embeds PCa-specific clinical biomarkers. The model generates a cancer heatmap and a risk score for clinically significant PCa. Following training on m
