Accelerated rehabilitation compared with a standard protocol after distal radial fractures treated with volar open reduction and internal fixation: a prospective, randomized, controlled study.

BACKGROUND: There are relatively few studies in the literature that specifically evaluate accelerated rehabilitation protocols for distal radial fractures treated with open reduction and internal fixation (ORIF). The purpose of this study was to compare the early postoperative outcomes (at zero to twelve weeks postoperatively) of patients enrolled in an accelerated rehabilitation protocol with those of patients enrolled in a standard rehabilitation protocol following ORIF for a distal radial fracture. We hypothesized that patients with accelerated rehabilitation after volar ORIF for a distal r

Clinical results after different operative treatment methods of radial head and neck fractures: a systematic review and meta-analysis of clinical outcome.

INTRODUCTION: There is no consensus on optimal treatment strategy for Mason type II-IV fractures. Most recommendations are based upon experts' opinion. METHODS: An OVID-based literature search were performed to identify studies on surgical treatment of radial head and neck fracture. Specific focus was placed on extracting data describing clinical efficacy and outcome by using the Mason classification and including elbow function scores. A total of 841 clinical studies were identified describing in total the clinical follow-up of 1264 patients. RESULTS: For type II radial head and neck fractu

Galeazzi fracture with volar dislocation of the distal radioulnar joint.

Galeazzi fracture dislocations are fractures of the distal one-third of the radial diaphysis with traumatic disruption of the distal radioulnar joint (DRUJ). This injury results in subluxation or dislocation of the ulnar head. We present a case of a Galeazzi fracture with a volar dislocation of the DRUJ. Open reduction of the DRUJ with Kirschner wire fixation in pronation was necessary to reduce the joint and maintain anatomic alignment. Repair of the triangular fibrocartilage complex was also necessary to maintain stability of the DRUJ.

A fracture of the radial styloid associated with proximal avulsion of abductor pollicis longus and extensor pollicis brevis.

The authors present the case of a right hand dominant adult who sustained a fracture of the distal radial styloid with complete avulsion of the abductor pollicis longus and extensor pollicis brevis tendons at their musculotendinous junctions. The authors could not find any reports of these injuries occurring together in the literature. No formal tendon reconstructive procedure was performed but full functional use of the injured extremity has been regained.

Taylor spatial frame in the treatment of upper extremity conditions.

BACKGROUND: Taylor spatial frame (TSF) is a modern multiplanar external fixator that combines ease of application and computer accuracy; it provides the capability of 1 to 6 axes of deformity correction sequentially or simultaneously by adjusting 6 connecting struts between 2 circular rings. Previous reports have documented the effectiveness of the TSF in acute fracture care, nonunion treatment, and in bone lengthening and deformity correction in the lower extremity. To the authors' knowledge, no previous case series in the English literature have documented the use of the TSF in treating uppe

Cannulated cancellous screw fixation for capitellum fractures in adolescents.

PURPOSE: To review the outcome of 9 adolescents treated with 2 to 3 cannulated cancellous screws for type-1 capitellum fractures. METHODS: Records of 8 boys and one girl aged 12 to 14 (mean, 14) years with type-1 capitellum fractures were reviewed. The mechanism of injury was a fall on the flexed elbow. No patient had associated injuries or neurovascular compromise. In 2 patients, the fracture was minimally displaced and treated with a cast. The remaining 7 patients with displaced fractures underwent open reduction and internal fixation using 4.0-mm cannulated partially threaded cancellous sc

Fluoroscopic evaluation of intra-articular screw placement during locked volar plating of the distal radius: a cadaveric study.

PURPOSE: To evaluate specific fluoroscopic views for assessment of intra-articular screw placement during locked volar plating of the distal radius. METHODS: The distal radius of a cadaver forearm was plated with a fixed-angle volar plate according to the surgical technique guide of the manufacturer. A goniometer was used to place the specimen at various described angles in the fluoroscope including standard posteroanterior (PA), tilt PA (11 degrees ), standard lateral, and tilt lateral (15 degrees , 23 degrees , 30 degrees ) views. Radiographic images of each screw individually and in variou

Terrible triad of the elbow--role of the coronoid process: a case report.

Posterior dislocation of the elbow joint with fracture of both the radial head and coronoid process, the so-called 'terrible triad', is a complex and difficult-to-treat injury. We report a case of a 34-year-old man with terrible triad of the elbow and associated injuries secondary to a fall. The patient was treated with radial head osteosynthesis, resection of the coronoid process fragments, and stabilisation of the ulnohumeral joint by Kirschner wire transfixation. The sequence of treatment steps is provided. The priority for treating multiple injuries should be established. The integrity of

Complete dislocation of the radial head following fracture of the radial neck in children: the Jeffery type II lesion.

The authors report four cases of Jeffery type 2 lesions in children. Despite the probable mechanism of such lesions, none of the cases reported here displayed a dislocation of the elbow. In the first observation the diagnosis was suspected only 15 days after injury. Two other cases were initially treated by closed external manipulations, one with percutaneous Kirschner wire leverage and the other with closed intramedullary pinning, but both cases displayed a 180 degree reversal of the radial head, suspected only secondarily. The first exhaustive review of the literature on the subject, shows t

[Carpal tunnel syndrome in children. Apropos of 4 personal cases].

Four cases of Carpal Tunnel syndrome during childhood are reported. Two of them are post traumatic cases; for one child, the syndrome was caused by hypertrophic cal six months after trauma. The other child has been operated because of a median and ulnar nerve deficit syndrome, which appeared one month after fracture. In the third observation, the authors have seen an abnormal muscle just in front of annular anterior ligament of the carpus during surgery. This muscle was assimilated to hypertrophic Palmaris Brevis muscle. In the last observation, the adolescent had a Poland syndrome with Spreng

The outcome of Radial Neck Fracture Treated with Closed/ Open Reduction and Retrograde Kirschner Wires Fixation in the Paediatric Age Group.

Background Radial neck fractures in children are rare injuries, representing 1 to 5% of all elbow pediatric fractures. Most of them are non-displaced or slightly displaced and treated conservatively. Severely displaced or angulated radial neck fractures (Judet type III and IV fractures or O'Brien type III radial neck fractures) requires surgical treatment. Objective To study the clinical and radiological outcomes of fractures following closed or open reduction and percutaneous intramedullary stabilization of the displaced radial neck fracture in children. Method There were 24 children with dis

Low complication rate of elastic stable intramedullary nailing (ESIN) of pediatric forearm fractures: A retrospective study of 202 cases.

Elastic stable intramedullary nailing (ESIN) has been established as state of the art treatment for forearm fractures in children, if operative stabilization is required. Their use has been expanded to single bone shaft fractures, and also more complex injuries such as Monteggia fractures or Monteggia-like lesions. A wide range of complications has been reported in the literature, up to 70% in certain investigations. The purpose of this study was to assess the complication rate after ESIN treatment of forearm fractures in children and adolescents in a representative cohort of patients from a l

Management of severe musculoskeletal injuries of the upper extremity.

Limb salvage was successful in 25 patients treated for severe grade III upper extremity injuries. In a retrospective review of 20 men and five women, follow-up time averaged 26 months. These high-energy injuries were characterized by massive soft-tissue injury, highly comminuted fractures, and significant neurovascular injury. Farm, industrial, and vehicular accidents accounted for 80% of the cases. Over 50% of the patients had concomitant systemic and/or other significant extremity injuries. Initial treatment consisted of irrigation and debridement and fracture stabilization using external an

Treatment of unstable forearm fractures at the metaphyseal-diaphyseal junction in children: antegrade ESIN vs. transepiphyseal intramedullary K-wire fixation.

BACKGROUND: Treatment of unstable forearm fractures in the metaphyseal-diaphyseal junction (MDJ) zone is still a matter of debate. Major drawbacks of all types of fixations include either invasiveness, technical impracticality, or lack of acceptance by patients. This study reports results after antegrade ESIN (a-ESIN) compared to transepiphyseal intramedullary K-wire (TIK) for unstable MDJ forearm fractures. METHODS: The MDJ of the forearm was defined as the square over the joints of both forearm bones subtracted with the square over the metaphysis of the radius alone. The data of 40 consecut

Trauma on eight wheels. A study of roller skating injuries in Seattle.

Thirty-five fractures secondary to roller skating accidents from March 15 to October 1, 1979 were treated in the University of Washington-affiliated hospitals. Twenty-eight involved the wrist and elbow. Only three were in skeletally immature individuals. Sixty-three percent of the patients were female with the 20- to 34-year-old age group the most commonly involved. A fall, usually backwards, onto outstretched arms and hands was the most frequent mechanism of injury. A detailed study of the popular outdoor skating area in Seattle, Green Lake, indicated that more than 75% of the injuries occurr

Apex Posterior Overcorrection of the Ulna Increases Radial Head Stability in Anterior Monteggia Fractures: An In Vitro Biomechanical Study.

PURPOSE: Anterior radial head instability may persist with anatomic restoration of the ulna in anterior Monteggia fractures, leading to poor outcomes. The purpose of this study was to evaluate the effect of apex posterior ulnar overcorrection on radial head (RH) stability in the setting of anterior Monteggia fractures. It was hypothesized that increasing apex posterior angulation of the ulna, as well as a supinated forearm position, would decrease anterior RH translation. METHODS: Sequential static biceps loading was applied to nine cadaveric arms using an elbow simulator with the elbow posit

Patterns of orthopedic injuries associated with increasing e-scooter-related trauma: A retrospective observational study with a 4-year evaluation from a tertiary center in Istanbul, Turkey.

The increasing use of electric scooters (e-scooters) has been accompanied by an increase in the number of traumas. Noncompliance with regulations and the effectiveness of legal measures remain subjects of ongoing debate. This study aimed to investigate the demographic characteristics of patients with e-scooter-related orthopedic injuries, temporal distribution of trauma, and patterns of these injuries. We retrospectively evaluated 534 patients with e-scooter-related injuries who were referred to the Department of Orthopedics and Traumatology between January 2020 and October 2023. Variables suc
