[Sleep apnea and heart failure: pathophysiology, diagnosis and therapy].

Sleep apnea, defined as a pathologic pause in breathing during sleep >10 s, promotes the progression of chronic heart failure and may be a predictor of poor prognosis. It causes, in fact, several mechanical, hemodynamic, chemical and inflammatory changes that negatively compromise cardiovascular homeostasis of heart failure patients. Sleep apnea is recognized as sleep apnea syndrome when specific symptoms, such as sleepiness and headache during the daytime and snoring, are present and is diagnosed with an overnight test called polysomnography. There are two different forms of sleep apnea, cent

Evaluation of tonsils and adenoids in Sleep Apnea syndrome.

Peripheral Sleep Apnea syndrome has been associated with enlarged tonsils and adenoids as well as other abnormalities which may cause upper airway obstruction in children. A multidisciplinary approach is used at the Step Disorder Center of Cincinnati General Hospital to evaluate the role of tonsils and adenoids in sleep apnea. Polysomnographic techniques combined with cine sleep studies of the upper airway document the degree, site and type of obstruction. This presentation highlights the entity of obstructive sleep apnea and provides a rational clinical approach in the management of these pat

Sleep-disordered breathing and childhood asthma: clinical implications.

PURPOSE OF REVIEW: Asthma morbidity in children remains high despite comprehensive evidence-based guidelines for evaluation and treatment. The objective of this review is to examine the evidence that obstructive sleep-disordered breathing often co-exists with asthma and is associated with asthma severity, and to discuss the clinical implications of this relationship, focusing particularly on studies published within the past year. RECENT FINDINGS: Obstructive sleep-disordered breathing is highly prevalent in children with severe or poorly controlled asthma. Data from an observational study su

Improvement in sleep apnea associated with closure of a patent foramen ovale.

Recent reports have documented an association between patent foramen ovale and obstructive sleep apnea. We report on a 51-year-old man with obstructive sleep apnea and recent stroke who was enrolled in a clinic trial evaluating the efficacy of closure of patent foramen ovale following ischemic stroke. He was randomly assigned to device closure. There was subjective dramatic improvement in sleep-apnea symptoms and objective improvement in polysomnographic testing after device implantation. Aside from a drop in apneas and hypopneas from 181 and 8 on the first polysomnogram to 19 and 0 on the sec

Electrical stimulation of the hypoglossal nerve: a potential therapy.

Obstructive sleep apnea is characterized by recurrent episodes of pharyngeal collapse, which result from a decrease in pharyngeal dilator muscle tone. The genioglossus is a major pharyngeal dilator that maintains airway patency during sleep. Early studies in animal and humans have demonstrated that electrical stimulation of this muscle reduces pharyngeal collapsibility, increases airflow, and mitigates obstructive sleep apnea. These findings impelled the development of fully implantable hypoglossal nerve stimulating systems (HGNS), for which feasibility trial results are now available. These p

Sleep apnea physiological burdens and markers of white matter injury: the Multi-Ethnic Study of Atherosclerosis.

STUDY OBJECTIVES: Obstructive sleep apnea is associated with cognitive impairment; however, the underlying mechanisms remain incompletely understood. Obstructive sleep apnea is characterized by periods of interrupted ventilation (ventilatory burden), leading to hypoxemia (hypoxic burden) and/or arousal (arousal burden) from sleep. Although hypoxemia is considered a key mechanism underlying white matter injury, its measurement has been limited. In our primary analysis, we assessed the association of hypoxic burden, a quantitative measure of hypoxemia, with white matter hyperintensity volume, a

Monitoring respiration during sleep.

The sleep-related breathing disorders have been categorized in various ways. The most basic schema divides them into obstructive or central apneic events. An American Academy of Sleep Medicine (AASM) Task Force Report published in 1999 defined four separate syndromes associated with abnormal respiratory events during sleep among adults, namely, obstructive sleep apnea-hypopnea syndrome (OSAHS), central sleep apnea-hypopnea syndrome, Cheyne-Stokes breathing syndrome, and sleep hypoventilation syndrome. In this classification, the upper airway resistance syndrome was not regarded as a distinct s

[Sleep apnea syndrome in patients with cardiac disease].

We examined the incidence of sleep-apnea syndrome (SAS; 5 or more episodes of apnea/hypopnea in 1 hour) in 213 patients (152 male, 67.8 +/- 10.9 years) with various cardiac diseases by a modified sleep polygraph (morpheus; Teijin Pharma, Tokyo) from July 2005 to April 2007. Mild sleep disturbance was defined as 5< or = AHI<20, moderate sleep disturbance as 20< or = AHI<40, and severe sleep disturbance as 40< or = AHI. SAS was seen in 87.3% of the patients. This high incidence sharply contrasts with 7.5% reported in factory workers in Japan. Body mass index, though significant, was scarcely cor

Sleep apnea in patients receiving growth hormone.

Among 145 patients treated with recombinant human growth hormone (GH), four developed sleep apnea (two obstructive, two mixed) associated with tonsillar and adenoidal hypertrophy in three. These four patients had no local risk factors predisposing to upper airway obstruction (i.e., frequent pharyngitis or sinusitis). Clinical and/or polysomnographic features of sleep apnea improved following cessation of GH therapy in one patient, and following tonsillectomy and adenoidectomy in all patients. The present observations indicate that, albeit rarely, obstructive and/or central sleep apnea may occu

Chest wall motion in sleep apnea.

Patterns of rib cage (RC) and abdomen (ABD) motion obtained from a respiratory inductive plethysmograph were studied during disordered breathing events - obstructive apnea, obstructive hypopnea, mixed apnea, and central apnea - in 54 patients with sleep apnea. The type of disordered breathing event was verified by esophageal pressure and bias-flow mask recordings. Obstructive apnea occurred to a variable extent in all patients and was characterized by stereotyped paradoxical motion of the RC or ABD or both in 49 patients (91%). There was no paradox in 2 patients with "feeble" inspiratory effor

Effect of cardiac pacing on sleep-related breathing disorders: a systematic review.

Sleep-related breathing disorders are commonly encountered in the middle-aged population, negatively affecting quality of life. Central sleep apnea is associated with congestive heart failure, whereas obstructive sleep apnea is related to different pathophysiologic mechanisms, such as the total or partial occlusion of upper airway tract. Both sleep-related disorders have been associated with increased morbidity, and hence, they have been a target of several treatment strategies. The aim of this systematic review is to evaluate the effect of different types of cardiac pacing on sleep-related br

Evaluating the impact of different message strategies about OSA employed in the American Academy of Sleep Medicine Count on Sleep campaign.

UNLABELLED: Health campaigns hold promise for promoting general awareness about obstructive sleep apnea. In 2023, the American Academy of Sleep Medicine developed a series of messages as part of their "Count on Sleep" campaign. Three distinct messaging strategies were employed in posts disseminated on Facebook: (1) positive outcome (benefits of obstructive sleep apnea treatment); (2) negative outcome (consequences of untreated obstructive sleep apnea); and (3) partner-focused. We evaluated Facebook analytics to explore which of the 3 strategies was most impactful. The impressions were highest

Cognitive behavioral therapy for insomnia is associated with reduced sleep apnea severity but not its endotype traits in those with comorbid insomnia and sleep apnea.

STUDY OBJECTIVES: Cognitive behavioral therapy for insomnia (CBT-I) improves obstructive sleep apnea (OSA) severity in comorbid insomnia and sleep apnea, though the mechanisms underlying this change are unstudied. CBT-I, which promotes sleep continuity and reduces hyperarousal, may improve OSA by raising the respiratory arousal threshold. We aimed to investigate the effect of CBT-I on OSA severity and its impact on the arousal threshold and other endotype traits. METHODS: In this single-arm trial, 25 patients with comorbid insomnia and sleep apnea (13 females and 12 males, mean age [standard

Central sleep apnea after stroke: prevalence, mechanisms, and clinical implications.

Central sleep apnea (CSA) and Cheyne-Stokes respiration (CSR) are common but underrecognized forms of sleep-disordered breathing after stroke. While obstructive sleep apnea dominates post-stroke research, CSA and CSR are neglected due to the fact that they rarely appear in isolation.

Sleep stage determines the expression pattern of sleep apneas.

We report on an obese man with sleep stage-dependent apnea. His type of apnea was predominantly central during non-rapid eye movement (NREM) sleep, while it was obstructive during rapid eye movement (REM) sleep. We found significantly more and more severe apneas during REM sleep compared to NREM sleep. His apneas were remarkably reduced by position adjustment, suggesting that upper airway collapse played a key role in the induction of both types of apnea. As his apneic patterns were dependent on the sleep stages, we assumed that the balance between upper airway obstruction and instability of c
