Hallucinations in Parkinson disease.

Patients with Parkinson disease (PD) can experience hallucinations (spontaneous aberrant perceptions) and illusions (misinterpretations of real perceptual stimuli). Of such phenomena, visual hallucinations (VHs) and illusions are the most frequently encountered, although auditory, olfactory and tactile hallucinations can also occur. In cross-sectional studies, VHs occur in approximately one-third of patients, but up to three-quarters of patients might develop VHs during a 20-year period. Hallucinations can have substantial psychosocial effects and, historically, were the main reason for placin

Nonmotor manifestations of Parkinson's disease.

Parkinson's disease (PD) is the second most common neurodegenerative disease after Alzheimer's disease. Traditionally, attention has focused on the motor symptomatology of PD, but it is now appreciated that the nonmotor symptoms affecting neuropsychiatric, sleep, autonomic, and sensory domains occur in up to 88% of PD patients and can be an important source of disability. Nonmotor manifestations of PD play a significant role in the impairment of disease-related quality of life. The cause of nonmotor manifestations of PD is multifactorial, but to a large extent, these manifestations are related

Actigraphic and parental reports of sleep difficulties in children with attention-deficit/hyperactivity disorder.

OBJECTIVES: To describe actigraphically detected and parent-reported sleep problems in nonmedicated children with attention-deficit/hyperactivity disorder (ADHD); to clarify whether or not comorbid oppositional defiant disorder contributes to sleep difficulties; and to compare objectively measured sleep with the parents' observations of sleep. DESIGN: Case-control study. SETTING: A child and adolescent psychiatric department of a teaching hospital. PARTICIPANTS: Two hundred six children aged 5 to 11 years, including 45 with a diagnosis of ADHD, 64 with a diagnosis of other psychiatric diagn

Disturbed sleep in post-traumatic stress disorder: secondary symptom or core feature?

Sleep disturbances are often viewed as a secondary symptom of post-traumatic stress disorder (PTSD), thought to resolve once PTSD has been treated. Specific screening, diagnosis and treatment of sleep disturbances is therefore not commonly conducted in trauma centres. However, recent evidence shows that this view and consequent practices are as much unhelpful as incorrect. Several sleep disorders-nightmares, insomnia, sleep apnoea and periodic limb movements-are highly prevalent in PTSD, and several studies found disturbed sleep to be a risk factor for the subsequent development of PTSD. Depressive symptoms, intrusive thoughts, sleep quality and sexual quality of life in women co-infected with human immunodeficiency virus and human papillomavirus.

BACKGROUND: Women infected with human immunodeficiency virus (HIV) experience major challenges and often report marked decreases in sexual functioning and quality of life (QOL). HIV-infected women also face challenges concerned with other commonly observed concomitant sexually transmitted organisms, such as human papillomavirus (HPV), which may further affect sexual QOL. Despite advances made in understanding factors that predict sexual functioning and QOL in men with HIV, relatively little is understood about the role of behavioural and emotional factors in women. METHODS: As a preliminary i

Treatment of chronic nightmares in adjudicated adolescent girls in a residential facility.

PURPOSE: To evaluate imagery rehearsal therapy for the treatment of chronic nightmares in a sample of adolescent girls. METHODS: Adolescent girls ranging in age from 13 to 18 years were recruited from the Wyoming Girls School in Sheridan, Wyoming (treatment group, n = 9; control group, n = 10). These girls had previously suffered a high prevalence of unwanted sexual experiences in childhood and adolescence, and thus many suffered from nightmares, sleep complaints, and posttraumatic stress symptoms. Imagery rehearsal therapy was provided in a 1-day (6-h) workshop. Imagery rehearsal consists of

Sleep disorders in Tourette syndrome: a primary or unrelated problem?

Sleep disturbances are a common complaint in children with either Tourette syndrome (TS) or attention-deficit hyperactivity disorder (ADHD). Because a significant number of individuals with TS also have ADHD, we attempted to determine whether sleep difficulties reported in TS are a primary problem or are related to the co-occurrence of ADHD. Using a parent-completed sleep questionnaire, data were collected on boys, ages 7-14 years. Three groups, TS-only (N = 57), ADHD-only (N = 21), and TS+ADHD (N = 89), were compared to an age-matched control population (N = 146). The complaint of "poor sleep

Monosymptomatic bedwetting.

An update of the pathogenesis and treatment of monosymptomatic bedwetting is presented. This frequently occurring entity seems to have a multifactorial pathogenesis incorporating arousal disturbances and disturbances to the circadian rhythm of diuresis modulating hormones. It has recently been substantiated that the bladder reservoir function in monosymptomatic bedwetting is normal. This is further underlined by the fact that treatment of instability of the bladder has proven futile. In a substantial part of the monosymptomatic bedwetters the changes in circadian rhythm of antidiuretic hormone

[Sleep disorders in Parkinson syndromes].

About 90% of neurodegenerative diseases with parkinsonism are associated with sleep disorders including daytime sleepiness, sleep-related breathing disorders and parasomnias. It is hard to define what ratio of insomnia and daytime hypersomnia is caused by the antiparkinsonian treatment, by the somatic and mental-emotional symptoms of the neurodegenerative disease and by the neurodegenerative brain process itself. Recent research suggests that the latter group is more important than expected. In Parkinson syndromes the structures included in sleep regulation--mainly within the brainstem--are al

Epidemiologic study of sleep disturbances and psychiatric disorders. An opportunity for prevention?

As part of the National Institute of Mental Health Epidemiologic Catchment Area study, 7954 respondents were questioned at baseline and 1 year later about sleep complaints and psychiatric symptoms using the Diagnostic Interview Schedule. Of this community sample, 10.2% and 3.2% noted insomnia and hypersomnia, respectively, at the first interview. Forty percent of those with insomnia and 46.5% of those with hypersomnia had a psychiatric disorder compared with 16.4% of those with no sleep complaints. The risk of developing new major depression was much higher in those who had insomnia at both in

Sleep and nocturnal breathing of individuals working over 1-week periods at high altitude: a cross-sectional polysomnographic study.

STUDY OBJECTIVES: To investigate objective sleep architectures, sleep fragmentation, and sleep-disordered breathing (SDB) indices in high-altitude workers rotating a week of altitude followed by a week of rest schedules from the Atacama Large Millimeter Array (ALMA) Observatory, Atacama, Chile. METHODS: A total of 122 high-altitude workers (males/females: 70/52) completed a polysomnographic sleep study at the Operation Support Facility (OSF, 2950 m). The majority of the participants (78 [63.9%]) were low-altitude residents. Most of the workers (74 [60.7%]) worked at the Array Operation Site (

Return of memory and sleep efficiency following moderate to severe closed head injury.

OBJECTIVE: Sleep disturbance is common in the subacute recovery phase following brain injury. A previous study from the authors' group found 68% of patients with closed head injury (CHI) had disrupted sleep on a rehabilitation unit. In the present study, the authors investigated whether improvement in sleep efficiency correlates with duration of posttraumatic amnesia (PTA) after CHI. METHODS: Fourteen CHI patients were enrolled and followed prospectively. Mechanism of injury included motor vehicle accident, fall, and blunt assault. An actigraph was placed on each subject's wrist within 72 hou

Vornorexant versus lemborexant for insomnia disorder: a systematic review and network meta-analysis.

OBJECTIVE: This frequentist network meta-analysis was conducted to compare vornorexant (VOR) and lemborexant (LEM) in terms of efficacy, tolerability, and safety outcomes in adults with insomnia disorder. METHODS: Efficacy outcomes evaluated at weeks 1 and 2 included subjective time to sleep onset (sTSO), which served as the primary outcome at week 2, as well as subjective total sleep time (sTST), subjective wake after sleep onset (sWASO), and subjective sleep efficiency. Tolerability outcome was treatment discontinuation due to adverse events. Safety outcomes included the incidence of death,

Male and female ecstasy users: differences in patterns of use, sleep quality and mental health outcomes.

BACKGROUND: Ecstasy users report a number of adverse effects following use including mood and sleep disturbances. The present study examined differences in characteristics of ecstasy use (amount, frequency of use, reported harm resulting from use) between males and females and assessed relationships between ecstasy use, sleep quality and mental health outcomes. METHODS: An online survey of 268 ecstasy users (54.1% male, 45.9% female) was conducted. Validated sleep instruments assessing sleep quality and excessive daytime sleepiness, as well as questionnaires regarding physical and mental heal

Chronic headaches and insomnia: working toward a biobehavioral model.

BACKGROUND: Sleep disturbances are consistently associated with chronic headaches, yet the mechanisms underlying this relationship remain unclear. One potential barrier to generating new hypotheses is the lack of synthesis between models of headache and models of sleep. The goal of this paper is to present a perspective on the chronification of migraine and tension-type headaches based upon conceptual models used in sleep research. METHODS: We provide a critical review of the literature on sleep and headache, highlighting the limitations in sleep methodology. Models of sleep physiology and in
