Pathogenesis of neurogenic pulmonary oedema.

Evidence suggests that the initial phase of neurogenic pulmonary oedema results from a centrally mediated, massive, sympathetic discharge. It is postulated that this produces intense, generlised, but transient, vasoconstriction with a resultant shift of blood from the high-resistance systemic circulation to the low-resistance pulmonary circulation. Pronounced increases in pulmonary vascular pressures and blood-volume then produce pulmonary oedema because of the hydrostatic effect of increased pulmonary capillary pressure. In addition, pulmonary hypertension and hypervolaemia injure pulmonary b

Early Evaluation of Thermal Radiofrequency vs. Chemical Neurolysis for T2 and T3 Sympathectomy in Post-Mastectomy Pain Syndrome Using Oximetry-Based Perfusion Index Assessment.

BACKGROUND: Post-mastectomy pain syndrome (PMPS) is a chronic neuropathic condition thought to be mediated mainly by the sympathetic nervous system. Effective treatment options for PMPS include T2 and T3 sympathectomy, performed through either thermal radiofrequency (TRF) or chemical neurolysis. OBJECTIVES: This trial compares the efficacy of pulsed radiofrequency (RF) to that of neurolysis for post-mastectomy pain relief. SETTING: This double-blinded, randomized trial was conducted in the National Cancer Institute of Cairo, Egypt. METHODS: Fifty-four female patients with PMPS that did not …

Phenotyping Dysautonomia in Unexplained Syncope: Diagnostic Yield and Therapeutic Implications in a Moroccan Cohort.

Background and objective Unexplained syncope remains a frequent and challenging clinical problem, even after guideline-directed cardiovascular and neurological evaluation. In a substantial proportion of patients, syncope remains without an identified etiology after conventional investigations, suggesting alternative underlying mechanisms. Autonomic nervous system (ANS) dysfunction plays a central role in cardiovascular regulation and has been increasingly recognized as a key contributor to unexplained syncope. ANS testing allows objective evaluation of cardiovascular autonomic reflexes, includ

Dynamic changes in cardiac autonomic function persist in the postacute phase after SARS-CoV-2 infection in a hamster model of COVID-19.

BACKGROUND: Autonomic nervous system (ANS) dysfunction is a central feature of long coronavirus disease syndrome, yet little is known about how it develops during and after severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. OBJECTIVE: This study aimed to characterize the time course of dynamic changes in linear and nonlinear heart rate variability measurements as indices of ANS function for up to 2 months after infection. METHODS: Hamsters implanted with radiotelemetry devices were inoculated with SARS-CoV-2. Electrocardiograms and subpleural pressures were recorded befo

Adrenergic receptors: a key determinant of outcomes in bacterial pneumonia and bacterial sepsis-associated acute lung injury/acute respiratory distress syndrome through participation in pulmonary innate immune response.

Acute lung injury (ALI) and its most severe form, acute respiratory distress syndrome (ARDS), are life-threatening inflammatory syndromes dr … ARs are G protein-coupled receptors (GPCRs) that serve as the core molecular interface between the sympathetic nervous system (SNS) and pulmonary innate immunity, modulating inflammatory responses, barrier integrity, and pathogen clearance within the lung. Recent seminal studies have delineated central neural circuits governing lung inflammation via sympathetic-adrenergic …

Effects of Slow Breathing Exercises on Cardiac Autonomic Functions in Anxiety Disorder-A Randomised Control Trial.

BACKGROUND: Anxiety disorders are commonly associated with a higher risk of fatal cardiovascular diseases (CVD). Anxiety disorders lead to dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis, thus weakening the key neuronal components of the autonomic nervous system (ANS) that are involved in cardiovascular functions, leading to increased cardiovascular risk. PURPOSE: Impaired ANS activity, as reduced parasympathetic tone is strongly associated with an increased risk of CVD in anxiety disorders. Slow pranayama influences the ANS by activating the parasympathetic tone and deactivati

Plasma-free metanephrines, nerve growth factor, and renalase significance in patients with PCOS.

PURPOSE: Polycystic ovarian syndrome (PCOS) is a common heterogeneous condition with probably multifactorial genesis. Animal studies have proven the essential role of the sympathetic nervous system in the syndrome development, while human studies are still contradictory. The present study aims to investigate the possible influence of plasma-free metanephrine (MN), and normetanephrine (NMN), nerve growth factor (NGF), and renalase (RNL) on the hormonal and metabolic parameters in women with PCOS and healthy controls. METHODS: Fifty patients with PCOS and 30 healthy women participated in the st…

Sympathetic Nervous Hyperactivity Impairs Microcirculation Leading to Early Brain Injury After Subarachnoid Hemorrhage.

BACKGROUND: Although early brain injury (EBI) is recognized as a critical step following subarachnoid hemorrhage (SAH), its pathophysiology and underlying mechanisms remain poorly understood. Herein, we investigated the role of cerebral circulation in the acute phase using patient data and a mouse SAH model and evaluated its regulation via the sympathetic nervous system. METHODS: The cerebral circulation time and neurological outcomes in the human body were retrospectively examined in 34 SAH cases with ruptured anterior circulation aneurysms and 85 cases with unruptured anterior circulation c…

Pralidoxime improves the hemodynamics and survival of rats with peritonitis-induced sepsis.

Several studies have suggested that sympathetic overstimulation causes deleterious effects in septic shock. A previous study suggested that pralidoxime exerted a pressor effect through a mechanism unrelated to the sympathetic nervous system; this effect was buffered by the vasodepressor action of pralidoxime mediated through sympathoinhibition. In this study, we explored the effects of pralidoxime on hemodynamics and survival in rats with peritonitis-induced sepsis. This study consisted of two sub-studies: survival and hemodynamic studies. In the survival study, 66 rats, which survived for 10 …

Rad-GTPase contributes to heart rate via L-type calcium channel regulation.

Sinoatrial node cardiomyocytes (SANcm) possess automatic, rhythmic electrical activity. SAN rate is influenced by autonomic nervous system input, including sympathetic nerve increases of heart rate (HR) via activation of β-adrenergic receptor signaling cascade (β-AR). L-type calcium channel (LTCC) activity contributes to membrane depolarization and is a central target of β-AR signaling. Recent studies revealed that the small G-protein Rad plays a central role in β-adrenergic receptor directed modulation of LTCC. These studies have identified a conserved mechanism in which β-AR stimulation resu

Macrophage beta2-adrenergic receptor is dispensable for the adipose tissue inflammation and function.

OBJECTIVE: Neuroimmune interactions between the sympathetic nervous system (SNS) and macrophages are required for the homeostasis of multiple tissues, including the adipose tissue. It has been proposed that the SNS maintains adipose tissue macrophages (ATMs) in an anti-inflammatory state via direct norepinephrine (NE) signaling to macrophages. This study aimed to investigate the physiological importance of this paradigm by utilizing a mouse model in which the adrenergic signaling from the SNS to macrophages, but not to other adipose tissue cells, was disrupted. METHODS: We generated a macroph…
