Anterior cruciate ligament reconstruction does not induce further gamma loop abnormalities on the intact side of the quadriceps femoris: A longitudinal study.

This study aimed to investigate the effect of surgery on the gamma loop of the quadriceps on the side with an intact knee in patients with anterior cruciate ligament (ACL) injuries. We compared longitudinally the response of alpha motor neurons to vibration stimulation of the quadriceps on the side with an intact knee before and after ACL reconstruction. To evaluate alpha motor neuron response, we measured the maximal knee extension strength and integrated electromyography of the vastus medialis, vastus lateralis, and rectus femoris. After obtaining pre-vibration data from each subject, vibrat

Functional results of low median and ulnar nerve repair with intraneural fascicular dissection and electrical fascicular orientation.

Twenty-eight low median nerve injuries and 23 low ulnar nerve injuries were repaired using intraneural fascicular dissection and electrical fascicular orientation. Eleven freshly lacerated nerves were seen within 48 hours after injury; 40 nerve lacerations were chronic. Fascicular orientation between sensory and motor fascicles at the proximal nerve end could be accurately differentiated in 47 nerves (92%) independent of whether it was acute or chronic. At the distal nerve end in fresh lacerations, the motor fascicles could be determined conclusively by muscle contraction with sequential elect

Immediate internal fixation of high-energy open forearm fractures.

Eighteen patients with Gustilo and Anderson Grade III A (7), III B (8) or III C (3) open diaphyseal forearm fractures were treated with a protocol consisting of extensive primary debridement, immediate open reduction, dynamic compression plate fixation, and vascular repair when indicated. This was followed by routine redebridement at 24 to 48-h intervals until wound status allowed completion of soft tissue reconstruction. Bone grafting was performed at 8 to 10 weeks following obtainment of a closed soft tissue envelope for injuries with extensive comminution or bone loss (5 patients, 7 fractur

Total Flavonoids of Rhizoma Drynariae Ameliorate Tendon-Bone Healing Following Anterior Cruciate Ligament Reconstruction in Rats by Activating the FSP1/CoQ10 Pathway To Inhibit Osteoblast Mitochondrial Dysfunction and Ferroptosis.

Objective Poor tendon-bone healing after anterior cruciate ligament reconstruction (ACLR) is often accompanied by impaired osteogenesis and apoptosis, while iron overload and mitochondrial dysfunction may exacerbate this process through ferroptosis. Although total flavonoids of Rhizoma Drynariae (TFRD) are known to have osteogenesis-promoting effects, whether they improve tendon-bone healing by regulating ferroptosis remains unclear. Methods The impact of TFRD on bone formation, ferroptosis, and mitochondrial function was evaluated using an ACLR rat model and a Dexamethasone (Dex)-induced oste

Surgical technique: repair of forefoot skin and soft tissue defects using a lateral tarsal flap with a reverse dorsalis pedis artery pedicle: a retrospective study of 11 patients.

BACKGROUND: Various authors have proposed flaps to reconstruct traumatic forefoot skin and soft tissue defects, especially with exposure of tendon and/or bone although which is best for particular circumstances is unclear. DESCRIPTION OF TECHNIQUE: The indications for the technique were a forefoot defect area of no more than 8-cm × 8-cm and a well-preserved lateral tarsal (LT) donor site. The injured tendons were repaired using tendon grafts. The free dorsalis pedis flap was outlined by centering it on the cutaneous branch of the LT artery and tailoring it to the size of the wound, allowing 0

Severe Cubital Tunnel Syndrome: Considerations for Nerve Transfer Surgery.

PURPOSE OF REVIEW: Cubital tunnel syndrome is the second most common compressive neuropathy, next to only carpal tunnel syndrome in its incidence. Severe states of disease do not respond to nonoperative management. Likewise, functional outcomes of cubital tunnel surgery decline as the disease becomes more severe. The relatively long distance from site of nerve compression at the elbow to the hand intrinsic muscles distally makes it a race between reinnervation of the muscle and irreversible motor endplate degeneration with muscle atrophy. Loss of intrinsic function can lead to severe functiona

Mid-palm replantation. Long-term results.

16 patients with complete mid-palm amputations underwent replantation between 1987 and 1991. 14 were successful. Young patients with guillotine type mid-palm amputations who could be treated with end-to-end suturing of the tendons and nerves, presented with the highest degree of function when evaluated both objectively and subjectively. In contrast, less return of function was noted in patients with high energy or avulsion type injuries of the nerves.

Injury to the suprascapular nerve associated with anterior dislocation of the shoulder: case report and review of the literature.

A young man with persistent pain beginning 2 days following reduction of an anterior subcoracoid dislocation was found to have a suprascapular nerve injury. The patient described was felt to have a neurapraxia of the suprascapular nerve. Possible causes of post-reduction subluxation are interposition of the shoulder joint capsule or biceps tendon, or rupture of the rotator cuff. EMG in 3 weeks, followup examinations, and active assisted motion 4 to 6 weeks postinjury are recommended.

Shear wave elastography-based predictive model for early functional recovery following extensor tendon repair of the hand.

OBJECTIVE: To investigate whether early postoperative shear wave elastography (SWE) parameters-specifically absolute stiffness and novel elasticity ratios reflecting regional stiffness gradients-can serve as early biomechanical proxies to predict 12-week functional outcomes. METHODS: This prospective study included 65 patients (81 tendons) who underwent extensor tendon surgical repair. Conventional ultrasound and SWE were performed one week postoperatively. Absolute shear wave velocities (SWV) were measured at the distal tendon (ROI-1), suture site (ROI-2), and proximal tendon (ROI-3). Elasti

Acute Achille Tendon Rupture After Treatment of Levofloxacin for Helicobacter pylori Eradication: A Case Report.

This case report aims to demonstrate a rare occurrence of tendon rupture attributable to levofloxacin use in Helicobacter pylori eradication. On the seventh day of treatment, the patient experienced severe foot pain and difficulty in walking, leading to a diagnosis of Achilles tendon rupture confirmed through magnetic resonance imaging. Levofloxacin-induced tendinopathy and/or rupture are rare complications that are often linked to age and sex. Health-care providers should exercise caution when prescribing fluoroquinolones, especially to at-risk patients, and should inform them about potential

Techniques for extensor mechanism reconstruction after total knee arthroplasty: Is there a clear winner?

Extensor mechanism (EM) disruption after total knee arthroplasty (TKA) is rare, but can result in devastating outcomes for patients including inability to ambulate. Disruption can occur at the quadriceps tendon, patella, or patellar tendon. This complication can be traumatic, but is often atraumatic from an iatrogenic or degenerative etiology. Primary repair for treatment of EM disruption has led to poor results with high failure rates and has mostly been abandoned. Most commonly accepted techniques center around Achilles tendon allograft reconstruction, synthetic mesh reconstruction, or other
