Heart transplantation in Ukraine during wartime: A retrospective cohort study of standard vs marginal donor heart transplantation outcomes.

BACKGROUND: Heart transplantation (HTx) is the gold standard for end-stage heart failure treatment, yet donor shortages limit its availability. In Ukraine, legislative reforms in 2019 spurred HTx growth, but the ongoing war halted progress and disrupted logistics. This study evaluates outcomes of orthotopic HTx, focusing on use of marginal donor hearts to expand the traditional donor pool. METHODS: We conducted a retrospective cohort study of 106 consecutive HTx cases at the Heart Institute of the Ministry of Health of Ukraine from December 2019 to October 2024. Data included donor and recipi

Kidney transplant from donation after circulatory death: a novel pre-transplant combined clinico-pathological predictive score.

BACKGROUND: Kidney donation after circulatory death is a valid alternative to donation after brain death. However, ischemia-reperfusion injury can damage the graft and make it non-viable. The Karpinski score, currently used to evaluate kidney quality, does not account for this type of damage and is, therefore, insufficient for predicting expected functional recovery. METHODS: Sixty-seven kidney biopsies from donors  after circulatory death and seventy-two kidney biopsies from donors after brain death were analyzed histologically. A Karpinski score was assigned to all samples. Additionally, bi

Post-transplant cyclophosphamide versus anti-thymocyte globulin in haploidentical stem cell transplantation: a systematic review and meta-analysis.

Post-transplant cyclophosphamide (PTCy) and anti-thymocyte globulin (ATG) are mainstay prophylactic treatment options for graft-versus-host disease (GVHD), widely used in haploidentical stem cell transplantation. Due to a lack of prospective studies, a number of retrospective comparisons have yielded different conclusions as to which prophylaxis regimen is superior. We performed a meta-analysis of these studies to get more informed and comprehensive decisions from clinicians. Nine studies were eligible, and a total of 1674 patients were included. The combined hazard ratio (HR), relative risk (

Predictive value of tacrolimus concentration/dose ratio in first post-transplant week for CYP3A5-polymorphism in kidney-transplant recipients.

BACKGROUND: Tacrolimus (TAC) is metabolized primarily by the CYP3A-encoded enzyme family (CYP3A4, CYP3A5, and CYP3A7). Individuals expressing the CYP3A51 allele are considered fast metabolizers and generally require higher TAC doses to reach therapeutic levels. AIM: To evaluate the predictive value of the TAC concentration-to-dose (C0/D) ratio for identifying CYP3A5 polymorphisms in renal transplant recipients. METHODS: Eighty-six de novo kidney transplant recipients with TAC-based immunosuppression from the Department of Nephrology and Dialysis at Military Hospital 103 (Hanoi, Vietnam) were

Artificial intelligence assisted risk prediction in organ transplantation: a UK Live-Donor Kidney Transplant Outcome Prediction tool.

Introduction: Predicting the outcome of a kidney transplant involving a living donor advances donor decision-making donors for clinicians and patients. However, the discriminative or calibration capacity of the currently employed models are limited. We set out to apply artificial intelligence (AI) algorithms to create a highly predictive risk stratification indicator, applicable to the UK's transplant selection process. Methodology: Pre-transplant characteristics from 12,661 live-donor kidney transplants (performed between 2007 and 2022) from the United Kingdom Transplant Registry database wer…

Deceased donor kidney transplantation in candidates with pre-transplant hematological malignancies: a literature review and recipient allocation proposal in Singapore.

Deceased donor kidneys are a scarce national resource, and principles of utilitarianism and justice govern allocation. Kidney transplant recipients with a prior history of cancer show an increased risk of malignancy- and non-malignancy-related mortality compared to their counterparts without a previous history of malignancy. The inferior survival of a recipient with pre-transplant malignancy questions the allocation of a scarce resource to a population at anticipated poorer patient and graft survival. However, patient survival has significantly improved with advances in therapeutics for hemato

Evolving Ethical Challenges After a Decade of Uterus Transplantation: Recommendations From the International Society of Uterus Transplantation Ethics Committee.

Uterus transplantation (UTx) became a clinical reality with the birth of the first baby in 2014. Following increased success, the procedure has now transitioned to clinical practice in many institutions throughout the world. With a rising number of donors, recipients, and babies born from this procedure, and with more institutions offering UTx, ethical challenges have evolved while novel aspects gained prominence. Here, the Ethics Committees of the International Uterus Transplantation Society, a section of The Transplantation Society, summarize current and future ethical challenges in UTx and

Pre-transplant T-cell clonal analysis identifies CD8+ donor reactive clones that contribute to kidney transplant rejection.

INTRODUCTION: Responses to allogeneic human leukocyte antigen (HLA) molecules limit the survival of transplanted organs. The changes in T-cell alloreactivity that contribute to this process, however, are not fully understood. We defined a set of donor reactive T-cell clones (DRTC) with the goal to elucidate signatures of kidney allograft rejection. METHODS: DRTC were identified pretransplant using an anti-donor mixed lymphocyte reaction assay: CFSE-diluting CD4+ and CD8+ DRTC were flow-sorted, and the TCR sequences were identified using Adaptive Immunosequencing. DRTC were then tracked in pos

Porto-sinusoidal vascular disease-like alterations in transplant liver biopsies: A clinicopathologic observation of transplant-associated portal vasculopathy.

OBJECTIVE: Non-cirrhotic portal hypertension (NCPH) encompasses hepatic vascular disorders without cirrhosis, now grouped under porto-sinusoidal vascular disease (PSVD). While PSVD features are well described in native livers, their significance in the liver transplant setting remains unclear and may present diagnostic challenges. This study aimed to investigate PSVD-like histologic alterations in liver allografts and their clinical relevance. DESIGN: We retrospectively analyzed 95 liver biopsies from 43 transplant recipients (2020-2022). Biopsies were reviewed for PSVD-specific features (obl

Are we disadvantaging smokers by excluding them from kidney transplantation? A single-centre experience and survey of kidney transplantation units.

BACKGROUND: Smoking has been shown to have detrimental effects on KT outcomes and survival. Most units and guidelines advocate for the cessation of smoking prior to a kidney transplant and consider it a general contraindication to listing. Smoking prevalence is higher in disadvantaged groups. Smoking cessation is complex and often takes many years. For those suffering from the burden of chronic kidney disease, a delay in transplantation with a longer dialysis time may result in worse outcomes and accentuate the difficulty of cessation. AIM: The objective of this study was to describe the coho

Post-Transplant Pain and Paralysis: Neurologic Amyotrophy as an Atypical Cause of Shoulder Dysfunction Following Hematopoietic Stem Cell Transplant.

BACKGROUND: Neuralgic amyotrophy (NA), also known as Parsonage-Turner syndrome, brachial neuritis, and idiopathic brachial plexopathy, is a rare and potentially debilitating peripheral nerve disorder characterized by acute-onset shoulder pain followed by progressive motor deficits. It is often under-recognized, with an estimated incidence of 1 to 3 per 100,000 annually, though some studies suggest the actual prevalence may be significantly higher. The condition typically progresses through three phases, an acute painful phase, a phase of weakness, and a recovery phase, with sensory disturbance

Post-transplant cyclophosphamide improves survival compared to antithymocyte globulin in HLA-mismatched unrelated donor stem cell transplantation.

Allogeneic hematopoietic stem cell transplantation (alloHSCT) from mismatched unrelated donors (MMUD) carries high risks of non-relapse mortality (NRM) and graft-versus-host disease (GvHD). Post-transplant cyclophosphamide (PTCY) has emerged as an alternative to antithymocyte globulin (ATG) for GvHD prophylaxis. This single-center retrospective study compared PTCY (N=41) to high-dose ATG and low-dose ATG in 155 MMUD alloHSCT recipients. PTCY was associated with better overall survival with a 1-year overall survival of 78.7% versus 56.5% in the PTCY and high-dose ATG groups (P=0.007) and 64.8%

HOPE in every breath and heartbeat: Understanding transplant eligibility, access barriers, and clinical outcomes in thoracic transplantation in persons with HIV.

BACKGROUND: Persons with human immunodeficiency virus (PWH) are living longer, leading to increased end-organ disease from chronic illness. Organ transplantation improves survival in patients with end-organ disease yet remains a constrained resource due to limited access. We aimed to better understand barriers to transplantation for thoracic end-organ disease in PWH. METHODS: We performed a retrospective study of patients referred to our center for consideration of thoracic organ transplant and assessed the factors associated with transplant eligibility in candidates with human immunodeficien

The rate of muscle wasting in liver transplant recipients on waiting list: post-transplant outcomes and associated serum metabolite patterns.

BACKGROUND: Sarcopenia at the time of liver transplantation (LT) is an established risk factor for mortality following LT. However, most studies in this context have defined sarcopenia by one-time, static measurements. The aims of this study were (I) to determine the impact of the rate of muscle loss in waitlisted LT recipients on post-LT outcomes and (II) to identify patterns of serum metabolites associated with patients with more progressive sarcopenia. METHODS: Patients undergoing liver transplant from 2008 to 2018 who received more than one computed tomography (CT) scans within 12 months

Randomized controlled trial of pre-transplant zoledronate versus observation for prevention of bone loss in allogeneic hematopoietic cell transplantation.

BackgroundApproximately half of allogeneic hematopoietic cell transplantation (HCT) recipients experience significant bone loss in the early post-HCT period. Only recently have international guidelines started recommending early screening. However, the guidance for intervention remains conservative. In this study, we sought to evaluate the efficacy of pre-transplant prophylactic zoledronate in preventing early bone loss in allogeneic HCT recipients.MethodsThis was an open-label, investigator-initiated, phase 2 randomized controlled trial (RCT) of prophylactic zoledronate versus observation to
