Headache, pituitary lesion and panhypopituitarism in a pregnant woman: tumor, apoplexy or hypophysitis?

Pituitary dysfunction during pregnancy and its differential diagnosis and treatment can be challenging, as illustrated by the following case. A 22-year-old woman underwent a C-section at 32 weeks of gestation because of preterm labor. She had headache, vision disturbance, polyuria, polydipsia, hypernatremia, diabetes insipidus and a pituitary lesion with findings compatible with apoplexy. Hormonal testing revealed panhypopituitarism. The peripartum presentation, magnetic resonance imaging findings, autoimmunity and global pituitary dysfunction led to the clinical diagnosis of autoimmune lympho

[Successful treatment of clinical manifestations of Muckle-Wells syndrome with anakinra].

HISTORY: A 58-year-old woman and her 18-year-old daughter were first seen in our rheumatology centre after having experienced many years of periodic fever, arthralgia and urticaria. Some months ago a diagnosis of Muckle-Wells syndrome (MWS) had been made and confirmed by genetic testing. The mother had developed partial deafness and substantial loss of vision. Her daughter had been suffering mainly from urticaria and fever at least once daily. THERAPY: Because of the established role of interleukin (IL)-1 in this hereditary disease and some positive case reports, we decided to treat these pat

Possible role of corticosteroids in nervous system plasticity: improvement in amblyopia after optic neuritis in the fellow eye treated with steroids.

OBJECTIVE: Amblyopia consists of reduced visual function in the absence of organic disease, caused by deficient visual stimulation, most commonly due to squint or refractive error. Amblyopia is thought to be reversible up until the age of approximately 8 years (critical period) and is usually treated with occlusion of the fellow eye. There is recent evidence for visual system plasticity extending beyond the critical period, supported by reports of improvement in visual acuity in the amblyopic eye after loss of vision in the contralateral eye. This suggests that the adult visual system exhibits

Outcome of traumatic optic neuropathy. Comparison between surgical and nonsurgical treatment.

34 patients with indirect traumatic optic neuropathy were studied to identify factors affecting outcome and surgical indications. 12 cases (13 eyes = group A) underwent surgery and 24 patients (24 eyes = group B) were managed without surgery. Age, optic canal fracture, visual acuity before treatment (initial visual acuity) and days until surgery (only group A) were employed as variables. Visual acuity improved significantly more in patients with initial visual acuity, hand movement (HM) or better than in those with initial visual acuity for light perception (LP) only or worse. High incidence of visual recovery among four Japanese patients with Leber's hereditary optic neuropathy with the 14484 mutation.

The 14484 mutation in the ND6 gene of mitochondrial DNA (mtDNA) is a genetic mutation associated with Leber's hereditary optic neuropathy (LHON) in Caucasian patients who show a high incidence of visual recovery. We evaluated four Japanese patients with LHON associated with the 14484 mutation who were negative for eight proposed secondary mutations. There was no family history of optic atrophy in three of the four patients. All four patients were initially diagnosed as having optic neuritis, either anterior (Cases 1 and 3) or retrobulbar (Cases 2 and 4), based upon their fundus findings and cl

Orbital pseudotumor: case report and overview.

BACKGROUND: The most common cause of a sudden onset of painful proptosis with diplopia in an otherwise healthy adult is orbital pseudotumor. However, there are other conditions that mimic this presentation and must be ruled out with laboratory testing and imaging studies. CASE REPORT: A 41-year-old Hawaiian man sought treatment for an acute, progressive, painful, left ophthalmoplegia without exophthalmos. During the next week, a loss of accommodation and associated pupillary reaction, decreased visual acuity, color vision deficits, and a paracentral scotoma developed. CT and MRI revealed a ma

Stargardt's disease is not allelic to the genes for neuronal ceroid lipofuscinoses.

Stargardt's disease is an autosomal recessive condition characterised by a rapid and bilateral loss of central vision at around 7 to 12 years, with typical changes in the macular and perimacular region. It is one of the most frequent causes of macular degeneration in childhood and accounts for 7% of all retinal dystrophies. Considering that inclusions of lipofuscin-like substances are observed in retinal pigmentary cells of patients with Stargardt's disease on the one hand, and that the early symptoms of neuronal ceroid lipofuscinosis (CLN3) are suggestive of Stargardt's disease on the other h

[Tuberculum sellae meningioma and hypophyseal adenoma in a woman].

The case of a 61 years old female patient with coincidental tuberculum sellae meningeoma and pituitary adenoma is demonstrated. Because of deteriorating vision of the left eye and bitemporal hemianopia the patient was investigated. Primary arranged computertomographical investigation showed a tall intra- and suprasellar solid homogeneous tumor. The plain skull film revealed sellar enlargement; angiographically craniolateral displacement of Aa. cerebri anteriores was found. Only MRI cleared up two coexisting tumors, an intrasellar lower enhancing tumor close to another extra-/parasellar. Neuroe

Adaptive disorders of accommodation and vergence in binocular dysfunction.

Disturbances of binocular vision are described clinically by the Duane-White classification in terms of the magnitude of the accommodative-convergence ratio (AC/A). Convergence excess and convergence insufficiency are assumed to result from high and low AC/A ratios respectively. It is assumed that the abnormal AC/A ratio is an independent variable that underlies abnormal phorias. However, recent studies have demonstrated that the AC/A ratio is inversely related to the adaptability of tonic accommodation (lens adaptation) and directly related to adaptability of tonic vergence (prism adaptation)

[Indications for studying evoked potentials in childhood. Methods--indications--value].

Evoked potentials (EP) represent a valuable addition to currently applied diagnostic methods in neuropediatrics. Profound knowledge of the neurophysiological conditions producing EP-alterations allows basic conclusions, that cannot be gained or replaced by other investigations. EP-investigation demonstrate the existence but not the nature of a lesion in the CNS. Further diagnostic work-up usually will be necessary. Proved EP-alterations produce reproducible diagnostic results and give clues regarding its localization. Evoked potentials can be used as a screening-method for neuropediatric disea

Paracentral Acute Middle Maculopathy Associated With Dengue Fever.

Newer imaging technique systems have led to an increased interest in the anatomical localization of retinal disease. The clinical and optical coherence tomography (OCT) features of paracentral acute middle maculopathy (PAMM) have been increasingly described and are classically defined by a hyperreflective band at the level of the inner nuclear layer (INL). Symptoms typically include blurred central vision with a paracentral scotoma. Potential causes include retinal vascular diseases and systemic disorders. We present the clinical and OCT findings of a 33-year-old male patient with PAMM followi
