Diagnosing and treating hair loss.

Physicians should be careful not to underestimate the emotional impact of hair loss for some patients. Patients may present with focal patches of hair loss or more diffuse hair loss, which may include predominant hair thinning or increased hair shedding. Focal hair loss can be further broken down into scarring and nonscarring. Scarring alopecia is best evaluated by a dermatologist. The cause of focal hair loss may be diagnosed by the appearance of the patch and examination for fungal agents. A scalp biopsy may be necessary if the cause of hair loss is unclear. Alopecia areata presents with smo

Topical corticosteroids in dermatology.

Topical corticosteroids are the most commonly prescribed agents in the treatment of dermatologic conditions. They are used primarily as monotherapy or in combination with other agents for enhanced efficacy. Several stronger preparations are now available since their first introduction. They are also available in various vehicles altering the potency and giving the option of tailoring them for use based on specific anatomic locations, area of involvement, age of the patient, and most importantly, severity of the condition. Several local and systemic side effects have been associated with their

[The use of autologous serum in the treatment of the ocular surface diseases].

In the literature, there is evidence suggesting that autologous serum eyedrops can be helpful in the treating various ocular conditions. Using eyedrops containing the 20-100% solution of patient's own serum, centrifuged 3000-4000 rpm, diluted in either saline, BSS or chloramphenicol solution, is a relatively new therapeutic strategy. Nevertheless, in some publications, a positive influence of using autologous serum topically has been proven in numerous ocular diseases. It has been found, that using autologous serum eyedrops is safe, such eyedrops can be stored in temperature ranging from -30 d

Effects of topical application of fipronil spot-on on dogs against the Chagas disease vector Triatoma infestans.

We assessed the insecticidal effects of fipronil spot-on applied to experimental dogs on the blood-feeding success and other vital parameters of the Trypanosoma cruzi vector Triatoma infestans. In the first trial, the cumulative mortality of 30 third or fourth instar nymphs exposed to eight fipronil-treated dogs differed significantly from those exposed to untreated dogs at 1 week post-treatment, but not at baseline or at 2-6 weeks post-treatment. In the second trial, the effects of multiple exposures to fipronil-treated dogs on bug population dynamics were assessed. A population of 80-84 bugs

Pityriasis rubra pilaris exacerbation with topical use of imiquimod.

The role of immune response modifiers is increasing in the treatment of dermatologic diseases. Imiquimod, a toll-like receptor agonist, results in up-regulation of proinflammatory cytokines for improved immune surveillance. Although topical use is generally well-tolerated, imiquimod can potentially result in systemic effects and exacerbate generalized inflammatory papulosquamous diseases of the skin. We report the case of a 67-year-old man who was treated with imiquimod for actinic keratosis and developed fever and a progressive erythematous papulosquamous eruption that was histologically cons

Comparative efficacy and tolerability of Ketomousse (ketoconazole foam 1%) and ketoconazole cream 2% in the treatment of pityriasis versicolor: results of a prospective, multicentre, randomised study.

Ketomousse (K), a new thermophobic formulation (ketoconazole 1%), has proven its efficacy in the treatment of dandruff, caused by the same agent as pityriasis versicolor (PV). The objective of this study was to compare the efficacy and tolerability of K thermophobic foam vs. ketoconazole cream 2% (N) in the treatment of PV. Forty-six patients (22 in K and 24 in N group) with PV involving no more than 15% of the total trunk surface were randomly assigned for treatment either with K or N once daily for 14 days. Three weeks after the completion of treatment, improvement rate and side-effects were

Topical pimecrolimus in the treatment of vitiligo.

Although the treatment of vitiligo has improved during the last decade, therapy is still not satisfactory for many patients. Recently topical calcineurin inhibitors were introduced in the treatment of atopic dermatitis. Considering the autoimmune hypothesis of vitiligo pathogenesis, the use of these topical calcineurin inhibitors seems reasonable. Most clinical vitiligo trials have been performed with tacrolimus and show beneficial effects. Concerning the value of pimecrolimus in the treatment of vitiligo only few data are available. Therefore we performed an open pilot study in 26 patients to

Two therapeutic challenges: periocular and genital vitiligo in children successfully treated with pimecrolimus cream.

Vitiligo is characterized by the selective destruction of melanocytes resulting in patches of skin depigmentation. Vitiligo is a therapeutic challenge. Eyebrows, eyelids and genital vitiligo are a therapeutic dilemma, especially in children. The possible side effects of topical corticosteroids and the difficulties for choosing any other adequate treatment option are a major concern. We present two children, one with vitiligo of the eyelids and the other with genital vitiligo, both treated with pimecrolimus 1% cream with almost full repigmentation of the lesions, showing pimecrolimus could be a

Positively charged microemulsions for topical application.

The study reports pig-skin permeation and skin accumulation of miconazole nitrate (MCZ) from positively charged microemulsions containing water, 1-decanol/1-dodecanol (2:1, w/w), lecithin and/or decyl polyglucoside at different weight ratios, propylene glycol, 1,2 hexanediol and a cationic charge-inducing agent (stearylamine (ST), l-alanine benzyl ester (ALAB) or cetyltrimethylammonium bromide (CTAB)). Zeta-potential values of the positively charged microemulsions ranged from 14.2 to 37.5 mV and mean droplet size from 6.0 to 16.8 nm. In vitro pig-skin permeation of MCZ after a single 24h appli

Bilateral periorbital necrotizing fasciitis following exposure to Holi colors: a case report.

Holi festival is celebrated in India traditionally by applying colors on one another. Various ocular adverse effects of these colors have been reported including conjunctivitis and corneal abrasion. We report a case of bilateral periorbital necrotizing fasciitis, following exposure to Holi colors. General physicians might encounter more such cases after exposure to Holi colors. In India, these colors are prepared on a small scale and lack any quality checks. Use of such toxic colors should be discouraged, and all doctors should caution people against using synthetic dyes. This case report high

Acrodermatitis continua of Hallopeau on toes successfully treated with a two-compound product containing calcipotriol and betamethasone dipropionate.

OBJECTIVE: To investigate the efficacy of treating acrodermatitis continua of Hallopeau (ACH) with a new two-compound ointment. BACKGROUND: ACH is a rare pustular eruption on the distal portions of the fingers and less often, on the toes. It is nowadays classified as a form of acropustular psoriasis that tends to be resistant to treatment. METHODS: A 75-year old woman presented with pustules and hyperkeratotic erythematous papules, coalescing in plaques on the dorsal aspect of both great toes and on the perionychium of their nails. Diagnosis of ACH was based on clinical and histologic findin

Potential new indications of topical calcineurin inhibitors.

The topical calcineurin inhibitors pimecrolimus (Elidel) and tacrolimus (Protopic) were initially developed for the treatment of atopic eczema (atopic dermatitis), a chronic or chronically relapsing skin condition most prevalent in infants and children. Their main advantages compared with conventional topical corticosteroid therapy are that they are more selective in their mode of action, do not induce skin atrophy and are not associated with significant systemic absorption. In addition, topical calcineurin inhibitors may represent a useful alternative to topical corticosteroids for the treatm

Molluscum contagiosum infection associated with pimecrolimus use in pityriasis alba.

Atopic dermatitis is an inflammatory, chronically relapsing, highly pruritic skin disorder that has considerable impact on the quality of life. As corticosteroids are not ideal for long-term control of the disease because of their potential side effects, steroid-free topical immunomodulators have recently been developed for the treatment of atopic dermatitis. Although clinical trials do not indicate an increased risk of cutaneous infection, the clinician must be careful about viral infections such as eczema herpeticum or molluscum eczema that can be observed during treatment with pimecrolimus

Erosive papulonodular dermatosis associated with topical benzocaine: a report of two cases and evidence that granuloma gluteale, pseudoverrucous papules, and Jacquet's erosive dermatitis are a disease spectrum.

Granuloma gluteale infantum/adultorum, pseudoverrucous papules, and Jacquet's erosive diaper dermatitis are fairly uncommon manifestations of irritant contact dermatitis historically thought to be distinct clinical entities. Here we report two informative cases of an erosive papulonodular dermatosis in the genital and suprapubic area associated with topical benzocaine. We review granuloma gluteale infantum, pseudoverrucous papules and nodules, and Jacquet's erosive diaper dermatitis, and propose that these disorders may all be variants of genitocrural irritant dermatitis.
