The effects of alcoholic beverages on urate metabolism in gout sufferers.

The purine contents of commercial, low-alcohol and alcohol-free beers were determined. Four gout sufferers were studied under controlled conditions before and after ingestion of four different beverages containing alcohol, alcohol and purine, purine and neither alcohol nor purine. The results show a significant increase in purine excretion with a fluid load alone and impairment or reversal of this response with the other three beverages. These results are difficult to interpret on the basis of the alcohol and purine contents of the beverages alone. Isohumulones are present in all beers. Their

Thermal threshold determination in alcoholic polyneuropathy: an improvement of diagnosis.

Reports on the incidence of alcoholic polyneuropathies are variable depending on diagnostic tools. In this study, 50 chronic alcoholics with positive MALT (Munich Alcoholism Test) and greater than seven years history of excessive alcohol abuse were examined neurologically. Tibial and peroneal motor and sural nerve conduction velocities (NCV) were studied. Warm and cold perception was evaluated in the area behind the internal malleolus using a Somedic-Thermotest. Thresholds were determined by the method of limits. The effect of a slow, medium and fast temperature change rate on thermal percepti…

Remission of severe alcoholic polyneuropathy after liver transplantation.

Alcoholic polyneuropathy presents with variable severity. Only few very severely disabled patients showed good clinical and electrophysiological improvement after cessation of alcohol abuse. We report a 55 year old patient with polyneuropathy who underwent orthotopic liver transplantation for decompensated alcoholic cirrhosis. After an initial postoperative exacerbation of her neurological disorder the patient developed an impressive remission of her clinical condition during the 4 year post transplantation follow up period. Nine months after surgery the initially wheel chair bound patient reg

Vibrameter testing facilitates the diagnosis of uremic and alcoholic polyneuropathy.

The diagnostic sensitivity of Vibrameter and tuning fork examination towards uremic and alcoholic neuropathy was tested in 75 patients. In 40 uremic and 35 alcoholic patients, we compared the sensitivity of neurological examination, nerve conduction studies (NCS) and vibration thresholds assessed at the malleoli by means of Vibrameter and scaled tuning fork. Vibrameter results were correlated with NCS. Polyneuropathy was diagnosed in 52 patients, but in 16 patients diagnosis depended upon inclusion of Vibrameter testing in the examination protocol. In uremic patients, Vibrameter (47.5%) showed

Inactivation of human immunodeficiency virus type 1 by alcohols.

Alcohols are commonly used as disinfectants for skin, surfaces and immersion of some medical instruments. Measurements of the activity of alcohols against human immunodeficiency virus type 1 (HIV-1) must take account of the compatibility of neutralizers used to stop the disinfectant reaction, and of toxicity to the cell line used to detect residual virus. We have developed protocols to measure the efficacy of alcohols against HIV in suspension and dried onto surfaces in the presence of high and low protein concentrations. High titres of HIV in suspension were rapidly inactivated by 70% ethanol

Trypsin activity. A new marker of acute alcoholic pancreatitis.

A normal serum amylase level is found in up to 32% of patients with acute alcoholic pancreatitis. This underlines the need for more sensitive diagnostic tests in this frequent cause of pancreatitis. Animal and human studies have shown that chronic alcohol consumption leads to important modifications in trypsinogen metabolism. The present work has prospectively analyzed admission serum trypsin activity with a new biochemical test and usual markers such as amylase, lipase, and immunoreactive trypsin in 32 attacks of acute pancreatitis. Seventeen were due to alcohol and 15 to other causes, includ

Hepatitis C virus, alcoholic cirrhosis, and hepatocellular carcinoma.

We undertook a retrospective study to determine the prevalence of hepatitis C virus (HCV) and hepatitis B virus (HBV) infection in 81 Caucasian patients with confirmed hepatocellular carcinoma (HCC). Besides HBV and HCV serological markers, HCV RNA and HBV DNA were detected in serum and liver tissue by polymerase chain reaction. Overall, HCV RNA was found in 20 cases (25%), HBV DNA in 21 patients (26%), and coinfection in 3 patients (3%). HCV RNA in liver tissue was not found without virus in serum, whereas HBV DNA was found in the liver tissue of one patient without viremia. In an additional

Alcohol disorders among patients with burns: crisis and opportunity.

A burn injury so severe that inpatient treatment is necessary is a crisis in any patient's life. For patients who also suffer from an alcohol use disorder, hospitalization may offer a unique opportunity to facilitate entry into appropriate treatment. In this study, 442 hospitalized patients with burns were evaluated, and 50 (11%) were diagnosed with an alcohol use disorder by DSM-III-R criteria. All but one of the injuries were deemed preventable. The average length of stay in hospital was 9 days longer for the alcohol group compared with the average stay in the burn center, resulting in addit

An introduction to alcohol-induced brain damage and its causes.

The aim of the symposium on alcohol-induced brain damage is to review current opinion and recent advances concerning factors which are thought to play a significant role in this disorder. The three principal factors are: alcohol specific neurotoxicity, associated vitamin B1 (thiamine) deficiency (the Wernicke-Korsakoff syndrome) and liver failure secondary to alcoholic cirrhosis. There is a complex interaction of these and other factors and it is difficult to dissect out the relative importance of each in the pathogenesis of alcohol-related brain damage. Moreover recent molecular and biochemic

Thiamine utilization in the pathogenesis of alcohol-induced brain damage.

There is increasing evidence for the role of thiamine deficiency in ethanol neurotoxicity and in development of alcoholic organic brain disorders other than Wernicke-Korsakoff syndrome [WKS] and cerebellar degeneration. Investigations in humans and in animal models have implicated a reduction in the activities of thiamine-utilizing enzymes as the metabolic basis of tissue injury due to thiamine deficiency. We have investigated the interactions of the thiamine-utilizing enzyme transketolase [Tk], derived from human fibroblasts, lymphoblasts, and various brain regions, with its cofactor, thiamin

The effect of ethyl alcohol on the sensitivity of upper airway reflexes.

The effect of ethyl alcohol on upper airway reflex sensitivity (UARS) has not been previously investigated in humans. Using a technique that we have previously described, intermittent breaths of low concentrations of ammonia vapour were used to measure the effect of ethyl alcohol 0.55-0.66 g/kg on UARS in ten healthy male volunteers. The depression of upper airway reflexes that occurred following ingestion of ethyl alcohol was maximum at 60 min and returned to baseline by 150 min. This dose of ethyl alcohol was insufficient to produce statistically significant depression of UARS. The blood alc

[Reversible neurological complications in chronic alcohol abuse with hypophosphatemia].

Severe hypophosphatemia is rare, usually affecting chronic alcoholics and patients under total parenteral nutrition. The most important clinical features are rhabdomyolysis and neurological deficits. The latter may take various forms and can affect the peripheral as well as the central nervous system. Symptoms of polyradiculitis with progressive paresis or cerebellar symptoms such as dysarthria, dysphagia and ataxia are frequent manifestations. Rarely, hypophosphatemia can cause confusional states, epileptic seizure or coma. The differential diagnosis includes Guillain-Barré polyradiculitis, d…

[The role of acetaldehyde in the pathogenetic mechanisms of the development of chronic alcoholism].

Experiments on white rats were carried out to confirm an important role of acetaldehyde in pathogeny of alcoholism. It is evidenced by results of experiments when animals were given acetaldehyde and medichronal (a drug which combines acetaldehyde) and by data obtained in the course of studies of changes in the content of endogenic ethanol and acetaldehyde, activity of alcohol dehydrogenase and aldehyde dehydrogenase, concentration of biogenic amines (catecholamines and serotonin) in the blood and brain structures.

Hyperactivity in preweanling rats following postnatal alcohol exposure.

Neonatal alcohol exposure in rats has been used as a model to study the effects of third-trimester alcohol consumption in humans. In the present research, male and female rat pups were artificially reared (AR) and received condensed alcohol exposure (6 g/kg/day or 4 g/kg/day) on postnatal days (PNs) 4 through 9. Controls consisted of both artificially reared animals receiving maltose-dextrin substituted for alcohol and normally reared animals. These rats were tested for open-field activity at 18 days of age for four days. Both male and female rat pups that received the high dose of alcohol (6

[Reversible hypokalemia and hypomagnesemia during alcohol withdrawal syndrome].

The alcohol-withdrawal syndrome was examined in 76 persons suffering from chronic alcohol dependency. Twenty persons showed only slight withdrawal symptoms. Thirty-two showed pronounced predelirium. Twenty-four patients developed delirium tremens. Commencing on the day on which alcohol was last consumed, potassium and magnesium levels in the blood serum were measured for 8 days until the 7th day after withdrawal from alcohol. The more pronounced the alcohol withdrawal syndrome, the sharper the decline in the level of potassium and magnesium in the blood serum. In each case, the decline in the

Prenatal alcohol and offspring development: the first fourteen years.

This report summarizes findings from a prospective longitudinal study of the effects of prenatal alcohol exposure on a birth cohort of 500 offspring selected from 1,529 consecutive pregnant women in prenatal care by mid-pregnancy at two representative community hospitals. Effects of prenatal alcohol observable on size measures at birth were insignificant after 8 months. Morphometric analysis of facial features identified effects only at the very highest alcohol exposure levels. By contrast, dose-dependent effects on neurobehavioral function from birth to 14 years have been established using pa
