Influence of phenylephrine bolus administration on left ventricular filling dynamics in patients with coronary artery disease and patients with valvular aortic stenosis.

BACKGROUND: Left ventricular diastolic function is known to be impaired in patients with coronary artery disease and patients with valvular aortic stenosis. Phenylephrine is frequently administered as an intravenous bolus in these patients perioperatively to increase coronary perfusion pressure. Although this is common practice, there is no information about the effect of phenylephrine bolus administration on left ventricular filling dynamics. METHODS: Twenty patients with coronary artery disease (group 1), 15 patients with valvular aortic stenosis (group 2), and 10 subjects without cardiovas

Antiemetic prophylaxis with promethazine or droperidol in paediatric outpatient strabismus surgery.

This randomized, double-blind study evaluated the antiemetic efficacy and the side-effects of promethazine pretreatment (0.5 mg.kg-1 IV + 0.5 mg.kg-1 IM) versus droperidol + placebo pretreatment (droperidol, 0.075 mg.kg-1 IV + physiological saline, 0.02 ml.kg-1 IM). One hundred unpremedicated ASA physical status I children ranging from two to ten years, and undergoing outpatient strabismus surgery were studied. All children received inhalational anaesthesia with halothane, nitrous oxide and oxygen. Neither opioids nor muscle relaxants were used. The incidence of vomiting and/or retching and th

Results of a feasibility trial to achieve total immobilization of patients in a neurosurgical intensive care unit with etomidate.

In the intensive care unit of Leiden University Neurosurgical Department 52 critically ill patients were given continuous infusions of etomidate to achieve 'total immobilization'. This can be defined as a state whereby there is complete elimination of all psychic, sensory and motor unrest. Etomidate administration varied from 2 hours to 25 days and dosages began at 20 micrograms/kg/minute and were increased to 110 micrograms/kg/minute according to need and indications for varying periods of time. Attempts were made to control unrest, convulsive seizure activity and raised intracranial pressure

Reduction of psychotomimetic side effects of Ketalar (ketamine) by Rohypnol (flunitrazepam). A randomized, double-blind trial.

A double-blind controlled trial based on 140 women undergoing abortus provocatus was employed to study whether the frequency of side effects after administration of the anaesthetic Ketalar (ketamine) could be reduced by a con-current dose of Rohypnol (flunitrazepam). The control group was given ketamine alone. The dosage of ketamine was 2 mg/kg body weight, supplemented if necessary by 1 mg/kg, in combination with either 2 mg flunitrazepam or placebo. No other anaesthetics were used. On several counts, the combination of ketamine and flunitrazepam was proved to reduce the adverse reactions see

Adverse reactions to intravenous anaesthetics.

One hundred consecutive reports of reactions to intravenous anaesthetics Althesin, thiopentone and Epontol are reviewed and analysed. Ten reactions are attributed to causes other than the anaesthetic drug, and four are believed to have been caused by the muscle relaxant employed. The remaining 86 reactions were grouped according to their clinical presentation: histaminoid reactions ( 19), histaminoid with bronchospasm (33), bronchospasm (12), cardiovascular collapse (uu), delayed histaminoid reactions (6), and clonic contractions (5). None of the first four reaction types was associated with o

Influence of glucose on the development of experimental metastases.

The effect of 10% glucose solution and narcosis upon the blood borne cancer cells was studied on Wistar rats, with Walker 256 carcinosarcoma inoculated intravenously. Our experiment reveals obvious differences between the control group and the groups treated with glucose concerning incidence, localization and tumour extension, differences which suggest that there is a risk in using glucose in the intra- and post-surgical resuscitation of patients with cancer.

Use of optiflow THRIVE high-flow nasal oxygen system versus a non-rebreather oxygen delivery device in patients with increased risk of airway obstruction in interventional radiology procedures: a randomized pilot study.

BACKGROUND: The configuration of interventional radiology (IR) suites often limits simultaneous access to the patient's airway and anesthesia equipment. This constraint may lead to suboptimal sedation in patients at increased risk of airway obstruction due to patient safety concerns. We conducted a randomized pilot trial to evaluate whether high-flow nasal oxygen (HFNO; F&P Optiflow™) reduces hypoxic events compared with a standard non-rebreather mask in this high-risk population undergoing deep sedation. METHODS: Patients at increased risk of airway obstruction or hypoxia during deep sedatio

Pain Management in Staged Osseointegration Procedures: A Retrospective Study and Foundation for Future Optimization.

INTRODUCTION: Osseointegration is an innovative procedure to attach an external prosthetic device directly to the skeleton. The technique has been shown to improve physical function and quality of life relative to conventional socket prosthetic devices. While much of the research in osseointegration has focused on functional outcomes, less is known regarding perioperative pain management. The purpose of this study was to describe perioperative and postoperative pain management approaches received by patients undergoing osseointegration procedures at a tertiary medical center. Predictors of Good Functional Outcomes in Posterior Circulation Stroke After Mechanical Thrombectomy With Stent Retrievers: An Individual Patient-Data Pooled Analysis From the TRACK and NASA Registries.

BACKGROUND: Recent randomized clinical trials have demonstrated that endovascular therapy for basilar artery occlusion is safe and potentially effective, predominantly in the non-White population. The aim of this study was to identify predictors of good functional outcome in posterior circulation strokes in US population after mechanical thrombectomy from the TRACK (Trevo Stent-Retriever Acute Stroke) and the NASA (North American Solitaire Stent Retriever Acute Stroke) registries from North America. METHODS: Patient-level data from the TRACK and NASA registries were pooled, and patients with

Safety and efficacy of esketamine for postoperative analgesia in pediatric patients with hypospadias.

OBJECTIVE: To explore the safety and efficacy of the combination of continuous intravenous infusion of esketamine with sacral block for postoperative analgesia in pediatric patients undergoing surgery for hypospadias. METHODS: Pediatric patients (n = 77) undergoing surgery for hypospadias were randomized into two groups: a hydromorphone group (H group, initial dose, 0.02 mg/kg; maintenance dose, 0.01 mg/kg/h) or an esketamine group (E group, initial dose, 0.3 mg/kg; maintenance dose 0.15 mg/kg/h). Caudal epidural block involved injection of 0.2% ropivacaine 1 ml/kg through the sacral hiatus.

The analgesic benefit of Pericapsular Nerve Group (PENG) block in hip arthroscopic surgery: a retrospective pragmatic analysis at an academic health center.

INTRODUCTION: The novel pericapsular nerve group (PENG) block has recently been reported to provide effective motor-sparing local anesthetic-based analgesia to the hip joint. We aimed to evaluate the analgesic efficacy and safety of a preoperative PENG block among patients undergoing ambulatory hip arthroscopic surgery where systemic analgesia is the gold standard. METHODS: We conducted a single-center, retrospective pragmatic exploratory cohort study of consecutive outpatient hip arthroscopic surgery cases from January 2017 to March 2019. We identified 164 cases in which patients received ge

The Effect of Intravenous Dextrose Administration on Postoperative Nausea and Vomiting: Systematic Review and Meta-Analysis with Trial Sequential Analysis.

Background and Objectives: Intravenous dextrose administration is a potential nonpharmacological adjunct for preventing postoperative nausea and vomiting (PONV); however, its efficacy and optimal timing remain unclear. This systematic review and meta-analysis, including trial sequential analysis (TSA), evaluated the antiemetic effects of perioperative dextrose administration. Materials and Methods: Electronic databases were searched for randomized controlled trials (RCTs) comparing dextrose-containing fluids with dextrose-free solutions under general anesthesia. The primary outcomes were the i

Intraoperative application of low-dose dexmedetomidine or lidocaine for postoperative analgesia in pediatric patients following craniotomy: a randomized double-blind placebo-controlled trial.

BACKGROUND: Postoperative pain is a common occurrence in pediatric patients following craniotomy, often leading to negative outcomes. Intravenous dexmedetomidine and lidocaine are commonly used adjuvant medicines in general anesthesia to reduce perioperative opioid consumption and relieve postoperative pain in adults. While they show promise for use in pediatrics, the evidence of their application in pediatric craniotomy patients is limited. Therefore, we aimed to compare the effects of dexmedetomidine and lidocaine on postoperative pain in pediatric patients following craniotomy. METHODS: We
