Comparison of augmentation methods for internal fixation of osteoporotic ankle fractures.

BACKGROUND: Internal fixation of osteoporotic ankle fractures is associated with failure of fixation. This study compared different augmentation methods biomechanically. MATERIALS AND METHODS: In nine paired fresh-frozen cadaver legs, an identical supination-external rotation type II ankle fracture was created. Fractures were stabilized using an eight-hole locking plate. In four pairs, two screws were inserted across the syndesmosis for purchase in the tibial metaphysis. One leg from each pair was randomly selected for injection of calcium sulphate-calcium phosphate graft into the screw holes

Arthroscopic-assisted locking compression plate clavicular hook fixation for unstable fractures of the lateral end of the clavicle: a prospective study.

The aim of this prospective study was to assess the clinical outcomes of an unstable fracture of the lateral end of the clavicle treated with an arthroscopic-assisted locking compressive plate (LCP) clavicular hook plate. Twenty-three patients underwent arthroscopic assisted LCP clavicular hook plate fixation for these fractures. All patients achieved clinical and radiological union over a mean of 4.2 months (range, 3.4-5 months). Four patients (17%) showed some degree of acromial osteolysis. Three patients (13%) showed radiological signs of arthrosis of the acromioclavicular joint. In one pat

[Surgical treatment in cervical spondylosis].

The most common cause for progressive cord and nerve root deterioration is cervical spondylosis. Symptoms often develop insidiously and are characterized by neck stiffness, arm pain, hand numbness, weakness in the hands and legs, gait disturbances--recognized in the clinical practice as cervical spondylotic myelopathy (CSM), cervical radiculopathy or both of them. The imaging diagnosis has to begin with plain radiographs of cervical spine, but magnetic resonance imaging (MRI) is a procedure of choice. Narrowing of the spinal canal caused by osteophytes, herniated disc, lig. flavum hypertrophy

Injury to the axillary artery, a complication of fixation using a locking plate.

The proximal humerus internal locking system is an internal fixation device consisting of a low-profile plate and locking screws, which is used for the fixation of three- and four-part fractures of the proximal humerus. We describe a case in which the screws cut out of the humeral head causing injury to the axillary artery necessitating urgent removal of the implant.

Correction of massive hydrocephalus and brain wound in holoprosencephaly.

Holoprosencephaly is a rare disorder of embryologic development that denotes an incomplete or absent division of the embryonic forebrain (prosencephalon) into distinct lateral cerebral hemispheres. In most cases of holoprosencephaly, the fetus fails to survive to term, and those that do survive have severe functional limitations, including mental retardation. We present a case report of cranial vault remodeling in a 20-month-old female who was born with holoprosencephaly and a severe number 0/14 cleft. The patient developed chronic nonhealing ulcers secondary to hydrocephalus, ventriculoperito

Management of mandible fractures.

LEARNING OBJECTIVES: After studying this article, the participant should be able to: 1. Review the incidence and etiology of mandible fractures. 2. Discuss indications and techniques for closed and open treatment of mandible fractures. 3. Review complications of mandible fractures. BACKGROUND: Mandible fractures are among the most common types of facial fractures treated by plastic surgeons. They must be managed carefully to maintain the function of the mandible, reestablish proper occlusion, and minimize secondary complications. METHODS: Current methods of management include combinations of

Internal fixation of distal radius fractures with novel intramedullary implants.

UNLABELLED: Fracture of the distal radius is a common injury and treatment varies depending on the fracture, patient factors, and surgeon preference. Internal fixation has grown in popularity because it affords increased stability and early motion of the extremity. The desire to provide stable fracture support while minimizing soft tissue dissection and complications has led to the development of new intramedullary implants. These implants use distal screw divergence for subchondral support, fixed-angle screws locked to the implant, and minimally invasive technique to allow early patient rehab

Influence of fracture pattern on consolidation after metacarpal plate fixation.

INTRODUCTION: Consolidation problems may complicate plate fixation of metacarpal fractures. It was our clinical impression that the fracture's morphology and the patient's occupation may influence this complication. METHOD: Retrospective study on 104 extra-articular metacarpal fractures. Time to union, presence of consolidation problems and time to return to work were correlated with fracture pattern (transverse/non-transverse), presence of soft tissue injury, type of patients and type of plate. RESULTS: Twelve patients (15%) experienced consolidations problems: 8 patients within the transve

Anterior locking screw plate fixation for cervical spondylosis.

Thirty consecutive patients with significant cervical spondylotic myelopathy or radiculopathy were treated with anterior decompression, autologous iliac crest graft, and titanium hollow screw plate (THSP). Eight patients had 1 previous surgery, and 1 patient had 2 previous surgeries. Satisfactory fixation was achieved intraoperatively in all instances. At time of final follow-up, graft incorporation was noted in all instances. There were 4 complications: 2 superficial infections, treated nonoperatively; 1 case of dysphagia, which resolved; and 1 case of spasmodic torticollis, which resolved. T

[Is miniplate osteosynthesis for combined mandibular body and bilateral mandibular condyle fractures adequate?].

Between 1980 and 1993 81 patients with a combined mandibular body and bilateral mandibular condyle fractures underwent open reduction and internal fixation of the mandibular body. The following methods were used: 29 x miniplate osteosynthesis, 28 x maxiplate osteosynthesis, 13 x lag screws, 10 x combinations and once titanium mesh. Reoperations because of occlusal disturbances were necessary in two cases of miniplate fixation, in one case of maxiplate fixation and in one case of lag screw fixation. In 1994 28 patients were reviewed. Posterior crossbite as operative sequela was noted in 2 (out

[Intramedullary nailing in multiple trauma].

The indication and best time for intramedullary nailing of long bones in the multiple trauma patient is still controversial. While in the Anglo-American literature primary intramedullary nailing is regarded as the method of choice, differentiated management is preferred in German-speaking countries on account of reports about the pathogenesis of trauma shock. On the one hand, it has been shown that severe thoracic injury greatly influences the course after multiple trauma and, in particular, its combination with femoral fracture represents an increased risk. On the other hand, every surgical i

Variables affecting pedicle screw plate fixation of an unstable L3-L4 defect.

Fresh frozen human cadaveric spinal specimens (T8-S1) were subjected to pure flexion extension bending moment and pure axial torque loadings while intervertebral rotations were recorded at the L3-L4, L2-L3, and Ll-L2 discs. A standardized unstable defect was created at the L3-L4 disc, and loading tests were repeated after application of bilateral Steffee plates in 2 configurations: a short plate with 2 pedicle screws (spanning the defect) and a longer plate with 3 pedicle screws (spanning the defect and 1 disc above). Each plating configuration was tested in the unlocked state (nuts compressin

Management of Perthes disease of late onset in southern India. The evaluation of a surgical method.

We analysed the short-term outcome after varus osteotomy for Perthes' disease in 48 older children from south-west India, comparing them with 30 historical controls. The children were between 7 and 12 years of age at the onset of the disease. All had stage-I or stage-II disease, with half or more of the epiphysis involved. The operated children had an open-wedge subtrochanteric varus osteotomy with derotation or extension and a trochanteric epiphyseodesis. Weight-bearing was avoided until late stage III. The non-operated children had been treated symptomatically by conservative methods. At the

[Growth disturbances after A-O cancellous screw head-neck transfixation for slipped femoral epiphysis].

The impact of head-neck A-O cancellous screw transfixation on subcapital growth plate in 20 children operated for slipped capital femoral epiphysis is presented. It was found, that early fusion of the growth plate may be attributed to the compression by A-O screw used for transfixation (p-0.0001). Resulting shortening of the femoral neck and relative overgrowth of the great trochanter did not influence the final outcome of treatment.

Operative treatment of intra-articular fractures of the calcaneus.

Displaced intra-articular fractures of the calcaneus remain a diagnostic and therapeutic dilemma. The classification of these fractures has been frustrated in the past by limitations of radiographic technique. Because of our need to consistently analyze our results, a CT scan classification was developed, based on the number and location of articular fracture fragments. The authors have treated displaced intra-articular calcaneal fractures according to an operative protocol using a modified lateral approach, a new plate and lag screws, all without the use of bone graft. This article will discu
