A Comprehensive Analysis of Percutaneous Screw Fixation for Metastatic Lesion of the Pelvis: Outcomes of 107 Cases.

BACKGROUND: Minimally invasive techniques such as percutaneous screw fixation have previously been shown to be mostly successful for pain relief and functional improvement in patients with pelvic metastases. In this study, we retrospectively reviewed the largest single-center cohort to date to further characterize the impact of this treatment on pain palliation, ambulation, and function; the predictors of suboptimal outcomes; and complications. METHODS: Electronic medical records were reviewed. The primary outcome measures were pain, as assessed with use of the visual analog scale (VAS) score

Orbital wall restoration with primary bone fragments in complex orbital fractures.

BACKGROUND: Complex orbital fractures are impure orbital fractures because they involve the orbital walls and mid-facial bones. The author reported an orbital wall restoration technique in which the primary orbital wall fragments were restored to their prior position in complex orbital fractures in 2020. As a follow-up to a previous preliminary study, this study retrospectively reviewed the surgical results of complex orbital wall fractures over a 4-year period and compared the surgical outcomes by dividing them into groups with and without balloon restoration. METHODS: Data of 939 patients w

Extensile Lateral Approach for the Operative Management of a Displaced Intra-Articular Calcaneus Fracture.

Calcaneal fractures account for approximately 1.2% of all fractures and 60% of all tarsal bone fractures. Almost 75% present as displaced, intra-articular fractures. Because of the complex articular and osseous anatomy, the vulnerable soft-tissue envelope, and the technically challenging approach needed for fixation, these fractures are often treated nonoperatively, resulting in poor outcomes. These poor outcomes can include entrapment of the posterior tibial and sural nerves, impingement, dislocation or entrapment of peroneal tendons, a widened heel with a loss of height, hindfoot varus or va

New intramedullary nail for the surgical neck fracture of the proximal humerus in elderly patients.

BACKGROUND: Fractures of the proximal humerus are common and the repair of displaced fractures generally requires an operative approach. In elderly patients, osteoporosis makes internal fixation problematic and frequently contributes to failed fixation and poor clinical results. We have developed a new intramedullary nail (pin lock nail) for the repair of surgical neck fractures of the proximal humerus in patients with osteoporotic bones. A retrospective review is presented of the cases of 19 elderly patients with two-part or three-part fractures of the proximal humerus treated using the pin l

Stability of midface fracture repair using absorbable plate and screw system pilot holes drilled and pin placement at angles other than 90°.

IMPORTANCE: Conventional plating systems use titanium plates for fixation of fractures, with benefits of strength and biocompatibility. However, titanium plates require that screws be placed at a 90° angle to the pilot holes. In the midface, this becomes extremely difficult. Today, a variety of craniomaxillofacial osteosynthesis systems are available, including resorbable plating systems. Specifically, the KLS Martin Sonic Weld system ultrasonically fuses the plate and the head of the pin when placed and will fill the pilot hole grooves completely even at less than 90° angles, which provides a

Isolated screw fixation of posterior wall fractures.

INTRODUCTION: Posterior wall (PW) fractures are common acetabular fractures and are most often treated by open reduction and internal fixation. The gold-standard of surgical stabilization is screw fixation of large wall fragments with additional buttress plating. Some reports focussed on screw fixation alone presenting adequate results. MATERIAL AND METHODS: From a total of 208 PW-fracture treated between 1972 and 2008, 57 patients were identified with open reduction and internal fixation (ORIF) using isolated screw fixation. These patients were analyzed regarding demographical data (patient

Biomechanical assessment of the maxillary position after Le Fort I osteotomy using rapidly biodegradable miniplates and screws: an experimental study.

Absorbable plates have become increasingly common in orthognathic surgery. Multiple plate types are available, including poly-L-lactic acid (PLLA) and PLLA/polyglycolic acid (PGA), the latter of which is expected to be absorbed more rapidly. However, the application of these materials in Le Fort I osteotomy is not clearly established. In this study, the strength and biomechanical properties of PLLA and PGA-co-PLLA absorbable plates were compared using an in vitro LeFort I osteotomy model. Basic physical strength was evaluated using tensile, bending, indentation, and handling test s. Biomechani

Which implant for Pauwels type 2 femoral neck fractures: a biomechanical study.

Femoral neck fractures (FNF) in young patients are relatively rare but can lead to severe complications if not treated appropriately. Pauwels type 2 fractures, characterized by moderate inclination, require surgical stabilization. However, no consensus exists regarding the optimal implant that provides superior biomechanical stability. This study aims to compare the biomechanical properties of 3 different fixation systems for the surgical treatment of Pauwels type 2 FNF.  Methods: A total of 21 bovine femurs of similar age and size were selected and divided into 3 fixation groups: dynamic hip

Comparative biomechanical evaluation of medial support plating in Pauwels type III femoral neck fractures: A finite element analysis.

OBJECTIVES: This study aims to evaluate the biomechanical contribution of medial support plating compared to isolated screws and to determine the impact of plate design (one-third tubular vs. mini-plate) and distal screw configuration (unicortical vs. bicortical) on construct stability. MATERIALS AND METHODS: Five finite element models were analyzed in this study: three cannulated screws (CS) alone (control), and CS augmented with either a one-third tubular plate or a 2.4-mm mini-plate. For the plated models, the distal fragment was fixed using either unicortical or bicortical screws to isola

Title of the study: finite element analysis of five internal fixation modalities for Schatzker type IV‒C tibial plateau fractures.

BACKGROUND: This study aims to evaluate the biomechanical performance of five different internal fixation strategies for Schatzker type IV-C tibial plateau fractures using three-dimensional finite element analysis. By analyzing stress distribution and displacement patterns under physiological load, we seek to identify optimal fixation modalities for clinical application. METHODS: We established a three-dimensional finite element model of Schatzker IV-C tibial plateau fractures and evaluated five internal fixation modalities via finite element analysis. These modalities included a medial plate
