Brown-Séquard syndrome associated with brachial plexus injury in neck trauma.

A case of adult cervical trauma that resulted in a clinical picture of Brown-Séquard syndrome and unilateral brachial plexus injury is presented. Rotational hyperflexion of the cervical spine is thought to have resulted in subluxation of C5-6 with root avulsions, leading to this unusual combination of findings.

Dislocations of the lower cervical spine.

In 34 cases of cervical spine facet dislocation treated between 1975 and 1979, the dislocations were reduced by closed methods and immobilized in the halo thoracic brace. If closed reduction was unsuccessful, open reduction and fusion were performed. There is a high incidence of failure of closed reduction and halo thoracic immobilization in patients with minimal or no neurologic deficit. Bilateral facet dislocation was more frequently associated with severe neurologic injury than was unilateral facet dislocation, Use of closed reduction is time consuming, often unsuccessful, and may result in

Pathologic fracture of odontoid process secondary to amyloid deposition.

Immunocyte dyscrasias have increasingly been reported in patients with gammopathy-associated systemic amyloidosis. In "primary" amyloidosis, deposits occur in heart, smooth and skeletal muscle, bones and joints, skin, lungs and peripheral nerves. The formation of amyloid fibrils appears related to the lambda light chains in these monoclonal gammopathies. A 72-year-old white man is reported who developed massive deposits of amyloid in joints and bones, as well as other tissues. Twenty percent of his bone marrow population was comprised of plasma cells in various maturational stages, without for

["Crowned" odontoid process and osteoarthrosis of the anterior atlantoaxial joint (author's transl)].

The so-called "peridentale aureole" ("crowned odontoid"), a horseshoe-like calcification around the odontoid process, can occasionally be shown on transbuccal views of the occipito-atlantoaxial region, but is commonly only seen on a.p. tomography in patients with osteoarthrosis of the anterior atlantoaxial joint. Tomographic examinations reveal that these irregular horseshoe-like calcifications around the odontoid peg represent mainly the osteophyte formation on the superior border of the anterior arch of the atlas. These calcifications are often surrounding a smaller calcification on the tip

[Roentgenological measurement of the cervical vertebral bodies in ossification of the posterior longitudinal ligament (OPLL) and cervical spondylosis (CS) (author's transl)].

Many studies of the pathogenesis of ossification of posterior longitudinal ligaments (OPLL) have been reported, and both general and local factors have been pointed out. In case a longitudinal ligament is affected by both the general and local OPLL factors for many years, the shape and size of the cervical vertebral bodies and the arrangement of the cervical spinal column might be affected. The relation between OPLL and disc degeneration, the morphology of the facet joint and the change of longitudinal ligament tension has been reported by many authors. But the relation between OPLL and the sh

Correction of spinal instability and recovery of neurologic loss following cervical vertebral body replacement. A case report.

While replacement of cervical vertebral bodies may be required when they are compromised by infection, tumor, or injury, or when they impinge on the neural elements, this procedure does not add immediate stability and may cause additional problems. In this case, displacement of a large fibular graft into the spinal canal appeared to cause additional spinal cord and nerve root injury. Despite a long delay, decompression and firmly fitted and wired anterior and posterior grafts seemed to enhance the neurologic recovery and permitted rapid mobilization. The diagnostic value of computerized tomogr

The halo-shoulder brace and the mandibular-shoulder brace as postoperative supports following spinal fusion.

Bracing of the cervical spine in patients with rheumatoid arthritis, ankylosing spondylitis and instability due to metastases poses special problems. Because of asymmetry, a tender bony or cutaneous swelling or tender skin, difficulties arise in fitting a mass-produced brace. In order to overcome these difficulties a carefully moulded made-to-measure halo-shoulder brace and also a mandibular-shoulder brace were developed. The halo-shoulder brace (a halo connected with 4 rods to a shoulder girdle) provides an effective means of postoperatively controlling the unstable cervical spine until the g

Strain on the interarticular stress distribution. Measurements regarding the development of spondylolysis.

In discussion the aetiology of spondylolysis, attention has been increasingly directed to the biomechanics of the lumbar vertebral column and the lumbo-sacral transition. The theory of a traumatic induction of spondylolysis was tested with stress distribution measurements on the vertebral column of cadavers. Using stretch measuring strips it was possible to investigate, the strain exerted upon the interarticular portion of the intact lumbar vertebral column preparation. A particular strain upon the pars interarticularis occurs through hyperextension, axial stress and torsion of the lumbar vert

Anterior surgery for cervical disc disease. Part 2: Treatment of cervical spondylotic myelopathy in 32 cases.

The results of anterior cervical surgery for treatment of cervical spondylotic myelopathy (CSM) are assessed 1 to 7 years postoperatively in 32 patients. At follow-up review, 50% were improved after surgery and 50% were unimproved or had deteriorated in spite of surgery. The results could not be statistically linked to the patients' age, duration of symptoms, severity of myelopathy, cervical canal size, or the performance of single- or multiple-level operations. Various anterior surgical techniques were used, but none proved to have superior results. The results in this series failed to surpas

[Treatment of cervical spine injuries by an anterior stable spondylodesis with an H-plate (author's transl)].

A stable spondylodesis of the cervical spine was obtained by an osteosynthesis with metal plates. The H-plate of the A-O in connection with autologous bone-chips from the iliac crest or Kieler bone and bone cement was used in five cases and resulted in a solid fusion of the affected cervical segments. The development of this technique and the advantages over other methods are demonstrated. The results of our cases reflect the superiority of this procedure.

[Atypical case of the Ramsay-Hunt syndrome].

The authors report a case of Ramsay-Hunt syndrome during cervical zoster in the C2--C4 area. The case reserves attention in view of the fact that facial nerve paralysis and paralysis of both parts of the right acoustic nerve occurred as a second exacerbation of the disease 16 days after the onset. Permanent hearing impairment in the right ear after regression of facial nerve paralysis and paralysis of the vestibular part of the acoustic nerve were also exceptionally rare signs.

Dysphagia due to vertical subluxation of the axis in rheumatoid arthritis.

Two patients with rheumatoid arthritis developed swallowing difficulties. These were mild in one case but severe in the other. Associated pyramidal tract signs and striking upward translocation of the dens made it seem probable that dysphagia was attributable to medullary compression despite the absence of other bulbar features. A history of dysphagia may therefore have serious implications for patients with rheumatoid disease.

Cervical osteoid osteoma: a cause of chronic upper back pain.

A case of an osteoid osteoma occurring in the cervical spine and presenting as scapular and arm pain has been described. Correct diagnosis was delayed for 2 years. Plain radiographs may fail to demonstrate a lesion in the spine. Radiographic evaluation, when osteoid osteoma is a possibility, should include bone scanning. If the bone scan is abnormal, tomography of the area is indicated. If the diagnosis is still unclear, selective angiography may reveal the tumor blush typical of osteoid osteoma. In certain instances, a myelogram may be helpful to establish whether there is encroachment on the

Potentially fatal asphyxia following a minor injury of the cervical spine.

The case is reported of an elderly man with asymptomatic cervical spondylosis who sustained a minor hyperextension injury of the cervical spine when the brakes of a car were applied suddenly to avoid a collision. Within six hours marked swelling of the neck and severe dyspnoea developed, and a lateral radiograph showed the pharyngeal shadow displaced far forwards and a crack fracture of an anterior osteophyte. Tracheostomy and evacuation of a massive prevertebral haematoma were performed, with immediate relief. The case draws attention to the possibility of this serious complication of a "whip
