Malnutrition in tuberculosis.

Tuberculosis has a dramatic effect on nutritional state and this has been borne out in all the studies that have investigated body composition in affected patients. I have included some of the key studies in this review; those I have not cited generally reach the same conclusions. Such malnutrition undoubtedly contributes to the morbidity of the disease and may also contribute to mortality, particularly in resource-poor settings where nutritional state, even in the "healthy," may be parlous. The extent to which such malnutrition also contributes to pathology remains unclear. Certainly, in othe

Risk factors for chronic lung disease in the surfactant era: a North Carolina population-based study of very low birth weight infants. North Carolina Neonatologists Association.

OBJECTIVE: To identify risk factors for chronic lung disease (CLD) in a population-based cohort of very low birth weight infants, born in an era of surfactant usage. We specifically investigated the effects of antenatal steroids, nosocomial infection, patent ductus arteriosus (PDA), fluid management, and ventilator support strategies. METHODS: Data were prospectively collected on 1244 infants born in North Carolina in 1994 with birth weights 500 to 1500 g, and treated at 1 of the 13 intensive care nurseries across the state. The outcome of interest was CLD, defined as dependency on supplement

Digital therapeutic combining exercise and cognitive-behavioural therapy reduces pain in patients with chronic patellofemoral pain: a multicentre randomised controlled trial.

OBJECTIVES: To evaluate whether an 8-week smartphone-based multidisciplinary digital therapeutic (DTx) combining exercise and cognitive-behavioural therapy reduced usual knee pain at 8 weeks compared with education and exercise instruction in chronic patellofemoral pain (PFP). METHODS: This multicentre, parallel-group randomised controlled trial included 216 participants with chronic PFP, defined as anterior or retropatellar pain lasting at least 3 months with activity-related provocation, recruited from orthopaedic outpatient clinics at 10 hospitals in the Republic of Korea. Participants wer

Altered parietal multisensory integration in chronic tinnitus during closed-loop real-time fMRI auditory downregulation.

Chronic subjective tinnitus is the most common form of tinnitus and refers to an internal persistent phantom auditory perception. Evidence from neuroimaging studies has established tinnitus and its associated distress as a brain network disorder, with multiple brain regions displaying dysregulated activity or connectivity extending way beyond the auditory pathway. Hyperactivity in the auditory cortex has been associated with perceived tinnitus loudness, and somatosensory-auditory interactions have recently been involved in promising treatment avenues through their disruption using non-invasive

Enhanced External Counterpulsation Versus Standard Medical Therapy for Chronic Refractory Angina: A Systematic Review and Meta-Analysis.

Background: Chronic refractory angina is a disabling syndrome of persistent ischemic symptoms despite guideline-directed medical therapy and, in many patients, prior coronary revascularization. Enhanced external counterpulsation [EECP] is a noninvasive therapy that augments diastolic coronary perfusion, reduces systolic afterload, improves endothelial function, and may promote collateral recruitment. Although developed for obstructive coronary artery disease, its vascular and microvascular effects may also be relevant to contemporary ischemic syndromes, including ischemia with non-obstructive

Comparative analysis of methods for identifying multimorbidity patterns among people with opioid use disorder: A retrospective single-cohort study.

BACKGROUND: Multimorbidity, the presence of two or more (2+) chronic conditions, presents significant challenges for healthcare delivery, particularly among populations with opioid use disorder (OUD). Multimorbidity patterns among individuals with OUD are not well established, and minimal research exists examining the impact of clustering methods on identifying these patterns. OBJECTIVE: Our study aimed to assess multimorbidity prevalence, explore associated sociodemographic and clinical characteristics, and determine multimorbidity patterns using hierarchical cluster analysis (HCA) and K-mea

Financial Toxicity and Its Determinants in Individuals Living With Inherited and Acquired Neuromuscular Disorders: The BIND Study.

BACKGROUND AND OBJECTIVES: Individuals living with neuromuscular disease (NMD) often face substantial financial strain due to the chronic and progressive nature of these conditions. Despite growing awareness, the extent and determinants of financial toxicity (FT) in this population remain poorly understood. We assessed FT among individuals with NMD or their caregivers in Canada using a validated patient-reported outcome measure. METHODS: We conducted a national, cross-sectional survey of individuals with NMD or their caregivers registered with Muscular Dystrophy Canada (MDC). The survey, avai

Burden of multiple high-risk factors in pregnancy before and after the universal two-child policy in Chinese women: An observational study.

BACKGROUND: The prevalence of high-risk pregnancy increased after the implementation of two-child policy in China, but the impact of this policy change on the burden and profile of multiple high-risk factors in pregnancy (MHFP) has been insufficiently explored. We hypothesised that the profile of MHFP might have changed after the two-child policy was implemented and aimed to estimate the prevalence, intercorrelation, and outcomes of MHFP before and after its introduction. METHODS: We obtained data on the population of pregnant women before (2015) and after (2020/2021) the implementation of un

Cost-utility analysis of four WHO-recommended sofosbuvir-based regimens for the treatment of chronic hepatitis C in sub-Saharan Africa.

BACKGROUND: Although direct-acting antivirals (DAA) have become standard care for patients with chronic hepatitis C worldwide, there is no evidence for their value for money in sub-Saharan Africa. We assessed the cost-effectiveness of four sofosbuvir-based regimens recommended by the World Health Organization (WHO) in Cameroon, Côte d'Ivoire and Senegal. METHODS: Using modelling, we simulated chronic hepatitis C progression with and without treatment in hypothetical cohorts of patients infected with the country's predominant genotypes (1, 2 and 4) and without other viral coinfections, history

Patient-reported outcomes with direct-acting antiviral treatment for hepatitis C in West and Central Africa (TAC ANRS 12311 trial).

BACKGROUND & AIMS: Patient-reported outcomes (PROs) are poorly documented for patients with chronic hepatitis C on direct-acting antiviral (DAA) treatment in low-to-middle-income countries. We documented PROs during and after DAA treatment in participants of the TAC ANRS 12311 trial (West and Central Africa). METHODS: Trial participants received a 12-week regimen containing either sofosbuvir plus ribavirin (HCV genotype 2, n = 40), or sofosbuvir plus ledipasvir (HCV genotypes 1 and 4, n = 80). Health-related quality of life (SF-12), fatigue (Piper Fatigue scale), and self-reported symptoms (3

Persistent Health Disparities Among Adult Cancer Survivors.

Cancer survivors face elevated risks of age- and treatment-related health conditions, possibly due to accelerated biological aging from cancer therapy. The role of social determinants of health (SDOH) in these aging trajectories remains understudied. We conducted a retrospective cohort study of 109,224 curatively treated Danish cancer survivors (lung, breast, prostate, or colon cancer, diagnosed between 1997 and 2018) and 574,022 age- and sex-matched cancer-free individuals. SDOH was assessed using educational level and cohabitation status. Using national registries, we investigated associatio

The association between long-term opioid therapy and composite infection-related dental outcomes.

BACKGROUND: The Food and Drug Administration's warning that transmucosal buprenorphine, a partial opioid agonist used to treat opioid use disorder and chronic pain, may cause dental disease opens questions about potential class-wide adverse effects involving more widely prescribed opioid analgesics. METHODS: This was a retrospective matched national cohort study of patients in care at the Department of Veterans Affairs (VA) between October 2010-September 2019. Patients prescribed LTOT were matched 1:2 to patients not prescribed LTOT on age, sex, service region, and VA dental coverage. Cox reg

The self-reinforcing cycle under dual constraints-a study of the behavioral logic of urban empty-nest older adults substituting self-medication for formal medical treatment.

INTRODUCTION: Against the stark backdrop of China's rapidly aging population and the high prevalence of chronic diseases among the older adults, urban empty-nesters face a dual challenge stemming from inadequate intergenerational support and heightened health vulnerability. A deeper understanding of their illness coping behavior is therefore critically needed. METHODS: This study employs grounded theory methodology, conducting in-depth interviews with 25 empty-nest older adults in H City. A total of 290,000 words of interview texts were collected to generate a theoretical model, systematicall

Post-Transplant Bendamustine as a Platform for Immune Modulation After Allogeneic Hematopoietic Cell Transplantation.

Post-transplant cyclophosphamide (PT-CY) remains the standard of care for graft-versus-host disease (GvHD) prophylaxis in haploidentical hematopoietic cell transplantation (HCT), yet relapse, delayed immune reconstitution, infections, and organ toxicity represent persistent and clinically meaningful limitations. Post-transplant bendamustine (PT-BEN) has been investigated as a biologically distinct alternative. In stringent haploidentical murine models, PT-BEN uncoupled GvHD from graft-versus-leukemia (GvL) effects through mechanisms centered on myeloid modulation and preservation of innate imm

Multidimensional toxicity of snake venom causes multi-organ damage and treatment challenges: a narrative review.

Snakebite envenomation has become a global public health challenge due to the widespread distribution of venomous snakes. Snake venom, a complex mixture containing various bioactive components, exhibits distinct characteristics across different families. The core toxic components of snake venom are mainly Three-Finger Toxins (3FTxs), phospholipase A2(PLA2), and proteases, which together form the material basis of the venom's multidimensional toxicity. Through synergistic effects, they activate common pathological pathways, enabling targeted disruption of multiple human organs., leading to acut
