Learning curve for cytoreductive surgery and perioperative intraperitoneal chemotherapy for peritoneal carcinomatosis.

BACKGROUND: Combined cytoreductive surgery (CRS) and perioperative intraperitoneal chemotherapy (PIC) has been considered as a standard approach for peritoneal surface malignancy. This study aims to assess the learning curve of this combined approach. METHODS: Prospectively collected data of our first 800 consecutive patients treated by CRS and PIC between January 1996 and July 2014 were retrospectively reviewed. Eight hundred patients were divided into two groups and eight groups for comparison. A significant difference was defined as P < 0.05. RESULTS: A significant increase in the rate of

Poorly differentiated cecal adenocarcinoma showing prominent rhabdoid feature combined with appendiceal mucinous cystadenoma: A case report and review of the literature.

Extrarenal rhabdoid tumors (ERRTs) are extremely rare neoplasms; of these, colorectal ERRTs are the most rare, and only nine cases have been previously described in the English language literature. The current study reports the pathological features of a case of poorly differentiated cecal adenocarcinoma with prominent rhabdoid feature, which was combined with mucinous cystadenoma of the appendix in a 73-year-old male, and additionally reviews the previously reported cases. Microscopically, the majority of tumor cells were non-cohesive or loosely cohesive, with a polygonal morphology and promi

[Pulmonary embolism during palliative chemotherapy including cetuximab for metastatic colorectal cancer].

In May 2007, a 48-year-old woman was admitted to our hospital for acute intestinal obstruction, and she was subsequently diagnosed with metastatic colorectal cancer in the sigmoid colon. Jejunum-ileum anastomosis and colostomy were performed as palliative surgery because the locally-advanced primary tumor had involved the ileum and other surrounding organs and formed huge mass. After placement of a central venous port, palliative chemotherapy mFOLFOX6 was commenced. In May 2008, mFOLFOX6 was replaced with FOLFIRI because of progression of both the metastasized and the primary tumors. On Novemb

Cystadenocarcinoma of the appendix: an incidental perioperatory finding in a patient with adenocarcinoma of the ascending and sigmoid colon: case report and review of literature.

BACKGROUND: Primary adenocarcinomas of the appendix are uncommon, constituting 1% of all colorectal malignancies. Appendiceal malignancies often present atypically, thus creating diagnostic challenges. Although there are many pathology reports of primary cystadenocarcinoma of the appendix, only a limited number of cases have appeared in the radiological or surgical literature. We present a unique case of primary cystadenocarcinoma of the appendix occurring concurrently with adenocarcinoma of the colon, and overview the clinical and therapeutic difficulties posed by this rare entity. Beyond total operative time: phase-specific robotic efficiency and postoperative morbidity following colorectal resection - a multisurgeon cohort study.

Robotic colorectal surgery is associated with longer operative times than conventional laparoscopic approaches, yet total operative duration is a composite measure comprising fundamentally different procedural components. It remains unknown as to whether postoperative morbidity is attributable to overall operative length or to specific phases of the robotic procedure. A retrospective single-centre cohort study at York and Scarborough NHS Foundation Trust, UK, included consecutive robotic colorectal resections performed by eight consultant surgeons (Surgeons A-H). Two prespecified cohorts were …

Technical risk factors for benign anastomotic strictures in colorectal and/or coloanal anastomosis: A retrospective case-control study.

AIM: Anastomotic stricture occurs in up to 30% of colorectal resections; however, evidence on risk factors and preventive measures remains scarce. This study aimed to identify technical factors responsible for increasing the risk for colorectal and coloanal anastomotic strictures. METHOD: This was a retrospective cohort study of patients with anastomotic stricture who underwent resection and/or redo anastomosis between January 1, 2011 and August 1, 2021 in a tertiary referral centre. Patients with anastomotic stricture were compared with an equal number of randomly selected patients without a

Elevated temperatures and longer durations improve the efficacy of oxaliplatin- and mitomycin C-based hyperthermic intraperitoneal chemotherapy in a confirmed rat model for peritoneal metastasis of colorectal cancer origin.

INTRODUCTION: In patients with limited peritoneal metastasis (PM) originating from colorectal cancer, cytoreductive surgery (CRS) followed by hyperthermic intraperitoneal chemotherapy (HIPEC) is a potentially curative treatment option. This combined treatment modality using HIPEC with mitomycin C (MMC) for 90 minutes proved to be superior to systemic chemotherapy alone, but no benefit of adding HIPEC to CRS alone was shown using oxaliplatin-based HIPEC during 30 minutes. We investigated the impact of treatment temperature and duration as relevant HIPEC parameters for these two chemotherapeutic

Using a combination of fruquintinib, raltitrexed, and S-1 as a third-line treatment for metastatic colorectal cancer with co-existence of Hodgkin lymphoma: a case report.

BACKGROUND: Patients with metastatic colorectal cancer (mCRC) beyond second line treatment have a poor prognosis. Fruquintinib, regorafenib, trifluridine/tipiracil (TAS-102), panitumumab and cetuximab combined with single-agent chemotherapy regimens are currently recommended as third-line therapies for patients exhibiting disease progression. Effective late-line therapies for mCRC are urgently needed. The FRESCO randomized clinical trial (RCT) prompts fruquintinib as a third-line treatment in advanced colorectal cancer (CRC). A phase II study in our center reported the efficacy and safety of S

The prevalence of sarcopenia and cachexia in older patients with localized colorectal cancer.

INTRODUCTION: The incidence of colorectal cancer (CRC) increases with age. In combination with an ageing population, the number of older patients undergoing surgical treatment for CRC is therefore expected to increase. Sarcopenia and cachexia are potentially modifiable risk factors of a negative surgical outcome. Sarcopenia can be categorized into primary (age-related) and secondary where diseases, such as malignancy, are influential factors. We aimed to investigate the prevalence of preoperative sarcopenia and cachexia in older (≥65 years) vulnerable patients with localized CRC. MATERIALS AN

Adult Descending Colon Duplication Mimicking Colon Cancer: A Rare Case Report.

INTRODUCTION: Intestinal duplication is a rare congenital anomaly characterized by the presence of a segment of intestinal tissue containing both mucosal and muscular layers adjacent to the normal alimentary tract. Intestinal duplication is most commonly diagnosed in infancy and is rarely observed in adults. Colonic duplications are particularly uncommon and may be difficult to distinguish from malignancy. Herein, we report a rare case of an adult descending colon duplication that was suspected preoperatively to be colon cancer. CASE PRESENTATION: A 75-year-old man with a history of surgery f

Lynch Syndrome-Impact of the Type of Deficient Mismatch Repair Gene Mutation on Diagnosis, Clinical Presentation, Surveillance and Therapeutic Approaches.

In today's world, with its continuing advancements in genetics, the identification of Lynch syndrome (LS) increasingly relies on sophisticated genetic testing techniques. Most guidelines recommend a tailored surveillance program, as well as personalized prophylactic and therapeutic approaches, according to the type of dMMR gene mutation. Carriers of path_MLH1 and path_MSH2 genes have a higher risk of developing colorectal cancer (CRC), despite intensive colonoscopic surveillance. Conversely, carriers of path_MSH6 and path_PMS2 genes have a lower risk of developing CRC, which may be due to thei

Automatic segmentation of male pelvic floor soft tissue structures for anatomical simulation and morphological assessment in lower rectal cancer surgery.

BACKGROUND: Pelvic anatomy is a complex network of organs that varies between individuals. Understanding the anatomy of individual patients is crucial for precise rectal cancer surgeries. Therefore, developing technology that can allow visualization of anatomy before surgery is necessary. This study aims to develop an auto-segmentation model of pelvic structures using AI technology and to evaluate the accuracy of the model toward preoperative anatomical understanding. METHODS: Data were collected from 63 male patients who underwent 3D MRI during a preoperative examination for colorectal and u

Splenic flexure mobilization and anastomotic leakage in anterior resection for rectal cancer: A multicentre cohort study.

BACKGROUND AND OBJECTIVE: Some colorectal surgeons advocate routine splenic flexure mobilization (SFM) when performing anterior resection for rectal cancer to ensure a tension-free anastomosis. Meta-analyses of smaller studies suggest that this approach does not influence anastomotic leakage rates, but larger multicentre studies are needed to confirm the safety of a selective strategy. The aim of this study is to evaluate the impact of SFM on anastomotic leakage. METHODS: This is a retrospective multicentre cohort study, comprising 1109 patients operated with anterior resection for rectal can
