Advanced techniques in cervical spine surgery.

Many disorders of the cervical spine can be effectively treated through a posterior approach. Generally, posterior decompressions such as a laminectomies, laminoplasties, and/or foraminotomies result in fewer postoperative complications compared with anterior approaches for multilevel surgery. Complications can include dysphagia, dysphonia, and pseudarthrosis. Fusion is indicated in spinal conditions that involve cervical instability, subluxation, or severe sagittal imbalance. During the past decade, techniques have been developed for instrumentation of the upper and subaxial cervical spine. S

Chiari malformation: are children little adults?

OBJECTIVES: To discuss the controversies, variability of opinion and approach, and special considerations in pediatric Chiari malformation. METHODS: PubMed-based literature review with focused discussion on clinically relevant pediatric issues. RESULTS: Compared to adults, children with Chiari more often present with congenital anomalies, scoliosis, and deficits in oropharyngeal control. Those with a syrinx present more with scoliosis than with deficits in sensation or pain. Surgical intervention has varied widely in its level of invasiveness though there has been a trend towards less invasi

Pharyngostomy tubes for gastric conduit decompression.

OBJECTIVE: This article illustrates our operative technique for pharyngostomy tube placement and describes our clinical experience with pharyngostomy use for gastric conduit decompression after esophagectomy. METHODS: We retrospectively reviewed patients undergoing pharyngostomy tube placement for gastric conduit decompression after esophagectomy from January 2008 to August 2009. Patients were included if they had a pharyngostomy tube placed at esophagectomy (prophylactic placement) or as a means of decompression after postesophagectomy anastomotic leak (therapeutic placement). We collected o

Saturday night palsy or Sunday morning hangover? A case report of alcohol-induced Crush Syndrome.

Saturday night palsy is a colloquial term given to brachial plexus injuries of the arm resulting from stretching or direct pressure against a firm object, often after alcohol or drug consumption. In most circumstances, this condition gives rise to a temporary plexopathy, which generally resolves. However, if the compression is severe and prolonged, a more grave form of this condition known as 'Crush Syndrome' may occur. Skeletal muscle injury, brought about by protracted immobilization, leads to muscle decay, causing rhabdomyolysis, which may in turn precipitate acute renal failure. This condi

The "sandwich phenomenon": a rare complication in adjacent, double-level X-stop surgery: report of three cases and review of the literature.

STUDY DESIGN: Case reports. OBJECTIVE: To report the atraumatic fracture of the intervening (L4) spinous process (SP) in patients treated with X-Stop at 2 adjacent levels and discuss the potential underlying causes. SUMMARY OF BACKGROUND DATA: Limited evidence on complications in double-level X-Stop surgery is available. METHODS: Three men, 47, 63, and 75 years old, respectively, underwent surgery with insertion of X-Stop at L3-L4 and L4-L5 because of low back pain and neurogenic claudication due to degenerative lumbar spine conditions. Two 10 mm devices were implanted in the first patient

The effect of bilateral laminotomy versus laminectomy on the motion and stiffness of the human lumbar spine: a biomechanical comparison.

STUDY DESIGN: A cadaveric simulation model of the lumbar spine was used to study the intervertebral motion characteristics of the lumbar spine after bilateral laminotomy and facet-sparing laminectomy. OBJECTIVE: To assess differences in motion patterns and lumbar spine stiffness after bilateral laminotomy versus laminectomy. SUMMARY OF BACKGROUND DATA: Spondylolisthesis after facet-sparing laminectomy has been reported with a frequency of 8% to 31%. Bilateral laminotomies have been shown to be effective in decompressing the spine, without resection of the posterior osteo-ligamentous complex.

A long-segment string of bead- like schwannoma of cauda equina: a case report.

Spinal schwannomas are one of the most common intradural extramedullary spinal tumors. However, a string of bead-like intradural extramedullary schwannoma is highly uncommon. In the paper we present a case of a long-segment string of bead-like intradural extramedullary schwannoma of cauda equina from L2 to S2.The patient had been suffering progressive low back pain and sciatica with progressive neurological deficits for three years. She was misdiagnosed as lumbar disc herniation at her first time of hospitalization after a simple lumbar computed tomography (CT) scanning and a discectomy. We fo

Microsurgical nerve root decompression in scoliotic lumbar spines-the relationship between important anatomical structures in correlation to imaging and implications for daily practice: anatomic cadaveric study.

OBJECT In a retrospective study in cadavers, the authors investigated the anatomical parameters between the convex and concave side in lumbar scoliosis for microsurgical discectomy and the decompression of spinal stenosis. Microsurgical intervention is the gold standard for lumbar herniotomy or the decompression of the spinal stenosis. However, the risk for complications during these procedures is higher due to the anatomical variations caused by the scoliosis. METHODS In 4 adult cadaveric scoliotic lumbar spines, the authors studied the anatomical relationship between important structures for

[Algorithms for neurosurgical treatment of trigeminal neuralgia].

Surgery should be considered only after anticonvulsant medications have failed or if medical treatment is not well-tolerated, including in cases of asthenia or drowsiness. In most reference centers, consensus is that MVD is the first option when patients are in good health. Percutaneous lesioning operations or radiosurgery are preferable in patients with adverse co-morbidity or those who are not willing to undergo open surgery.

Anatomical variability of the lateral femoral cutaneous nerve: findings from a surgical series.

The lateral femoral cutaneous nerve (LFCN) is a branch of the lumbar plexus and supplies the skin of the lateral thigh region. This entrapment-compressive syndrome is named meralgia paresthetica or Roth's meralgia and depends, on a vast majority of cases, on the entrapment of the nerve in proximity of the inguinal ligament. Surgical decompression of the nerve is an option when conservative treatments fail and is usually performed through a 3-cm infrainguinal skin incision. Available data on anatomical variations of the LFCN derive from extensive cadaver dissections and lack many features relev

Arthroscopic release of flexor hallucis longus tendon using modified posteromedial and posterolateral portals in the supine position.

BACKGROUND: The flexor hallucis longus (FHL) is a known site of pathological conditions. FHL stenosing tenosynovitis presents as posterior medial ankle pain or great toe discomfort and is often associated with a catching sensation of the great toe. There are a wide variety of open procedures for the treatment of FHL stenosing tenosynovitis. OBJECTIVE: Arthroscopic treatment may circumvent some of the complications associated with open surgical treatment of the hindfoot. METHODS: Arthroscopic surgery was completed in the supine position using modified posteromedial and posterolateral portals.

[Endoscopic anatomy research related to transsphenoidal optic nerve decompression].

OBJECTIVE: To investigate the anatomical relationship among optic nerve, posterior ethmoid sinus and ophthalmic artery to further provide surgical instruction for endoscopic transsphenoidal optic nerve decompression. METHOD: Messerklinger technique was adopted in eight cases of adult cadaveric head to expose the posterior ethmoid sinus and sphenoid sinus. The optic-carotid recess and optic canal were identified. The adjacent structure of optic canal was observed. After removal of the bony wall of optic canal, the relationship between the optic nerve and the ophthalmic artery was disrupted. R

[Modern opinions on diagnostics and treatment of carpal tunnel syndrome].

Carpal tunnel syndrome is the most common compressive neuropathy of the upper extremity. Because of that many different methods of diagnostics is used to make proper diagnosis. Few of them, like: MRI, ultrasounds or nerve conductivity speed are used more often than others. Also among surgical methods of treatment some gain an advantage over others. In this paper we present opinion on the diagnostics and treatment of carpal tunnel syndrome according to our experience and references.

Complex distal humeral fractures: internal fixation with a principle-based parallel-plate technique. Surgical technique.

BACKGROUND: Severe comminution, bone loss, and osteopenia at the site of a distal humeral fracture increase the risk of an unsatisfactory result, often secondary to inadequate fixation. The purpose of this study was to determine the outcome of treating these fractures with a principle-based technique that maximizes fixation in the articular fragments and stability at the supracondylar level. METHODS: Thirty-four consecutive complex distal humeral fractures were fixed with two parallel plates applied (medially and laterally) in approximately the sagittal plane. The technique was specifically d

Cauda equina syndrome: a comprehensive review.

Cauda equina syndrome (CES) is a rare syndrome that has been described as a complex of symptoms and signs--low back pain, unilateral or bilateral sciatica, motor weakness of lower extremities, sensory disturbance in saddle area, and loss of visceral function--resulting from compression of the cauda equina. CES occurs in approximately 2% of cases of herniated lumbar discs and is one of the few spinal surgical emergencies. In this article, we review information that is critical in understanding, diagnosing, and treating CES.
