Auricular reconstruction using Medpor combined with different hearing rehabilitation approaches for microtia.

BACKGROUND: Congenital microtia-atresia affects patients in two specific ways: severe conductive hearing loss and difficulty in integrating into social environments due to auricle malformation. AIMS/OBJECTIVES: To investigate the safety and efficacy of single-stage auricular reconstruction and hearing rehabilitation in children with microtia and external auditory canal atresia. MATERIAL AND METHODS: From January 2016 to December 2019, we included 32 patients with microtia and external canal atresia who received auricle reconstruction with high-density polyethylene (Medpor) framework and thre

Prevalence of Semicircular Canal Hypoplasia in Patients With CHARGE Syndrome: 3C Syndrome.

IMPORTANCE: CHARGE syndrome refers to a syndrome involving coloboma, heart defects, atresia choanae, retardation of growth and development, genitourinary disorders, and ear anomalies. However, Verloes revised the characteristics of CHARGE syndrome in 2005 to define this syndrome more broadly. Deficiency of the semicircular canals is now a major criterion for CHARGE syndrome. OBJECTIVE: To characterize patients with CHARGE syndrome at our center using Verloes' criteria and to reevaluate the nomenclature for this condition. DESIGN, SETTING, AND PARTICIPANTS: We performed a medical chart review

[Prevalence of conductive hypoacusis in children aged 5-9 years old from rural area in Poland--prospective screening of healthy subjects].

Otitis media with effusion (OME) is a process, which goes on by inner side of intact tympanic membrane. Oligosymptomatic and deceitful course is characteristics for this disease. Fluid filling middle ear structures causes long-lasting hypoacusis. Chronic consequences of the OME are morphologic changes within the middle ear, especially in the tense part of the tympanic membrane, causing strengthened conductive hypoacusis. The OME morbidity peak falls in 3-6 year of life, which is the period of the most intense development of the child and speech creation. Discreet symptoms, like fullness and fe

Early catarrhine evolution in the light of semicircular canal and vestibule shape.

The phylogenetic relationships of early catarrhines (Old World monkeys and apes) are contentious, particularly regarding pliopithecoids, which are considered a clade of Miocene stem catarrhines more derived than most of their Oligocene counterparts. However, their debated affinities and the uncertain phylogenetic relationships of the Oligocene catarrhine Saadanius leave their systematic position unresolved. Notably, Saadanius displays a mosaic morphology, coupling primitive craniodental traits with a fully ossified ectotympanic (as opposed to the annular/incompletely ossified ear tube of stem

Benign Paroxysmal Positional Vertigo in Oral Surgery and Implantology: A Review.

Benign paroxysmal positional vertigo (BPPV) is a common inner ear disorder characterized by intense dizziness often triggered by sudden head movements. Certain dental and maxillofacial surgeries may induce BPPV due to vibrational trauma from instrumentation, extended hyperflexion of the head, and existing vestibular conditions. This study aimed to analyze certain oral and maxillofacial surgical procedures associated with BPPV and evaluate prevention and treatment strategies. A review of relevant literature was conducted through Google Scholar and an extensive search on PubMed for the period 20

Deep learning-based severity grading of Meniere's disease using 2D MRI.

BACKGROUND: Meniere's disease (MD) is a complex inner ear disorder characterized by symptoms such as vertigo, hearing loss, tinnitus, and ear fullness, significantly impacting patients' quality of life. Current diagnostic methods lack sufficient accuracy and consistency, highlighting the need for reliable segmentation tools integrated with advanced deep learning techniques. PURPOSE: To develop and evaluate the multi-stage severity assessment system (MSAS), a novel deep learning-based framework designed for precise segmentation and accurate stratification of MD severity using 2D magnetic reson

AAV-mediated transgene delivery targeting spiral ganglion nonsensory cells.

In-situ neuronal reprogramming in the cochlea through gene therapy offers an avenue to restore hearing loss caused by neuronal damage. One possible source of neuronal conversion is the nonspiral ganglion cells (NSGCs), which include satellite cells, Schwann cells, and otic mesenchyme cells. A major obstacle for this approach is the vector-mediated transgene delivery toward NSGCs. Herein, we sought to assess the transduction profile of adeno-associated virus (AAV) serotypes with peripheral glial cell tropism in the murine inner ear. AAV-1, AAV-DJ, and AAV-PHP.eB with a cytomegalovirus promoter-

A new 3D-printed temporal bone: 'the SAPIENS'-specific anatomical printed-3D-model in education and new surgical simulations.

PURPOSE: Otology and neuro-otology surgeries pose significant challenges due to the intricate and variable anatomy of the temporal bone (TB), requiring extensive training. In the last years 3D-printed temporal bone models for otological dissection are becoming increasingly popular. In this study, we presented a new 3D-printed temporal bone model named 'SAPIENS', tailored for educational and surgical simulation purposes. METHODS: The 'SAPIENS' model was a collaborative effort involving a multidisciplinary team, including radiologists, software engineers, ENT specialists, and 3D-printing expert

Dual stimuli-responsive and sustained drug delivery NanoSensoGel formulation for prevention of cisplatin-induced ototoxicity.

Cisplatin (CisPt)-induced ototoxicity (CIO) is delineated as a consequence of CisPt-induced intracellular generation of reactive oxygen species (ROS) which can be circumvented by Bucillamine (BUC; an antioxidant drug with sulfhydryl groups) and Diltiazem (DLT, L-type calcium channel blocker). However, its effective accumulation in the Organ of Corti and cell cytoplasm is desired. Therefore, a biocompatible BUC- and DLT-nanoparticles (NPs)-impregnated dual stimuli-responsive formulation (NanoSensoGel) presented here with ROS- and thermo-responsive properties for the sustained and receptive deli

The caspase-inhibitor Emricasan efficiently counteracts cisplatin- and neomycin-induced cytotoxicity in cochlear cells.

Cisplatin is a chemotherapeutic agent widely used to treat solid tumors. However, it can also be highly ototoxic, resulting in high-frequency hearing loss. Cisplatin causes degeneration of hair cells (HCs) and spiral ganglion neurons (SGNs) in the inner ear, which are essential components of the hearing process and cannot be regenerated in mammals. As the affected cells primarily die by apoptosis, we tested several anti-apoptotic small molecules to protect these cells from drug-induced toxicity. We found that the general caspase inhibitor Emricasan could significantly counteract the toxic effe

Gram-negative Bacteria are Associated With Sensorineural Hearing Loss in Chronic Otitis Media.

OBJECT: Chronic otitis media (COM) is an inflammatory disease that commonly presents with otorrhea and hearing loss. Bacteria-induced inflammation can cause inner ear damage, leading to sensorineural hearing loss (SNHL). This study aimed to compare the prevalence and severity of SNHL in patients with gram-negative versus gram-positive cultures and examine associations between the concentrations of circulating monocytes and neutrophils with bacteria species and SNHL. METHODS: This was a retrospective study. Cholesteatoma or chronic suppurative otitis media patients with otorrhea were enrolled.

Physiologic Effects of Microneedle-Mediated Intracochlear Dexamethasone Injection in the Guinea Pig.

OBJECTIVES: Oral or intratympanic corticosteroids are commonly used to treat sudden sensorineural hearing loss (SSHL), tinnitus, and Meniere disease. Direct intracochlear delivery has been proposed to overcome the variability in bioavailability and efficacy of systemic or middle ear delivery. In this study, we aim to characterize the physiologic consequences of microneedle-mediated direct intracochlear injection of dexamethasone through the round window membrane (RWM). METHODS: In Hartley guinea pigs (n = 5), a post-auricular incision followed by bullostomy was made to access the round window

Secondary autoimmune immune ear disease (AIED): a systematic review and meta-analysis on vestibular manifestations of systemic autoimmune and inflammatory disorders.

Secondary autoimmune inner ear disease (AIED) is often bilateral and asymmetric in patients presenting with audiovestibular symptoms due to a systemic autoimmune disease. This systematic review and meta-analysis are aimed at identifying and highlighting patterns in prevalence of vestibular dysfunction, symptom presentation, and diagnostic methods in extant literature by combining clinical context from case reports with quantitative analyses from cohort studies. Screening of articles by title, abstract, and full text was completed by four reviewers (K.Z., A.L., S.C., and S.J.). In this study, w

Cytotoxicity of Tetracyclines in Human Tympanic Membrane Fibroblasts.

HYPOTHESIS: Tetracyclines are less cytotoxic to tympanic membrane (TM) fibroblasts than quinolones. BACKGROUND: Use of quinolone ear drops after tympanostomy tube placement and for acute otitis externa has been linked to an increased risk of TM perforation. This has been verified in animal models. Cell culture studies have shown quinolones to be highly toxic to TM fibroblasts. Tetracyclines are a potential alternative to quinolones as they have been used to treat acute otitis externa and are thought to be nontoxic to the inner ear. We aimed to determine if tetracyclines are cytotoxic to TM fi
