Impact of cancer-related and primary lymphedema and compression bandaging on limb range of motion: a cross-sectional study.

INTRODUCTION: Lymphedema is chronic and can be a consequence of cancer treatment. Little is known about the range of movement (ROM) of the limbs with lymphedema. We aimed to quantify the ROM in patients with lymphedema to assess the impact of lymphedema and multilayer bandaging on mobility. METHODS: A motion analysis system quantified ROM. The ankle and knee of 22 patients (57 years, 14 females) with lower limb lymphedema (mainly secondary to gynecological or urological cancer) were evaluated. The wrist and elbow of 21 women (58 years) affected by upper limb lymphedema secondary to breast can…

Capitellum Fracture with Distal Triceps Tendon Avulsion Injury: A Rare Presentation with Literature Review.

INTRODUCTION: Capitellum fractures are rare, representing only 1% of all elbow injuries. McKee Type 4 capitellum fractures encompass osteochondral shearing fractures of the capitellum, which involve a significant lateral portion of the trochlear ridge and the trochlea. These fractures may be linked with soft tissue and bone injuries surrounding the elbow, such as distal triceps tendon avulsion (DTTA) injuries. To date, only three case reports in the English literature have documented the correlation between Type 4 capitellum and trochlear fractures with DTTA injuries. Orthopedic surgeons shoul…

Effect of ulnar angulation and soft tissue sectioning on radial head stability in anterior Monteggia injuries: an in vitro biomechanical study.

BACKGROUND: Radial head instability continues to be a challenge in the management of anterior Monteggia injuries; however, there is a paucity of literature on the factors that contribute to this instability. The aim of this biomechanical investigation was to examine the effects of ulnar angulation and soft tissue insufficiency on radial head stability in anterior Monteggia injuries. METHODS: Six cadaveric arms were mounted in an elbow motion simulator. Radial head translation was measured during simulated active elbow flexion with the forearm supinated. After testing the elbows in the intact …

Motor Impairment-Related Alterations in Biceps and Triceps Brachii Fascicle Lengths in Chronic Hemiparetic Stroke.

Poststroke deficits in upper extremity function occur during activities of daily living due to motor impairments of the paretic arm, including weakness and abnormal synergies, both of which result in altered use of the paretic arm. Over time, chronic disuse and a resultant flexed elbow posture may result in secondary changes in the musculoskeletal system that may limit use of the arm and impact functional mobility. This study utilized extended field-of-view ultrasound to measure fascicle lengths of the biceps (long head) and triceps (distal portion of the lateral head) brachii in order to inve…

Effect of trochleocapitellar index on adult patient-reported outcomes after noncomminuted intra-articular distal humeral fractures.

BACKGROUND: Anatomic surgical reduction of intra-articular fractures of the distal humerus is important to achieve the best long-term outcomes and prevent post-traumatic arthritis. In this study we compared the radiographic reduction using the trochleocapitellar index. We also correlated the trochleocapitellar index to the functional outcomes next to the comparison of the triceps brachii lifting approach and olecranon osteotomy approach, 2 common approaches for distal humeral fractures. METHODS: From January 2006 to June 2016, patients with elbow fractures were registered in 4 centers. The tr…

Ulnar Nerve Enlargement at the Medial Epicondyle Negatively Correlates With Nerve Conduction Velocity in Cubital Tunnel Syndrome.

Background: In cubital tunnel syndrome (CuTS), chronic compression often occurs at the origin of the flexor carpi ulnaris at the medial epicondyle. Motor nerve conduction velocity (NCV) across the elbow is assessed preoperatively to corroborate the clinical impression of CuTS. The purpose of this study was to correlate preoperative NCV to the direct measurements of ulnar nerve size about the elbow at the time of surgery in patients with clinical and/or electrodiagnostic evidence of CuTS. Methods: Data from 51 consecutive patients who underwent cubital tunnel release over a 2-year period were r…

Radial head prosthesis: surgical tips and tricks.

Radial head prostheses (RHP) have been developed to decrease the complications rate following a radial head resection surgery. The aim of the RHP is to replicate the physiological radiocapitellar tracking, reproducing the mechanical function of the native radial head: to stabilize the elbow and to shear the forces passing through the elbow along with the other stabilizers. The currently used RHP models try to achieve this target with three different prosthesis' strategies: (a) loose fit stem, (b) bipolar radial head or (c) anatomical radial head. Even if the radial head fixation is the preferr…

Low incidence of flexion-type supracondylar humerus fractures but high rate of complications.

Background and purpose - Supracondylar humerus fractures are the most common type of elbow fracture in children. A small proportion of them are flexion-type fractures. We analyzed their current incidence, injury history, clinical and radiographic findings, treatment, and outcomes. Patients and methods - We performed a population-based study, including all children <16 years of age. Radiographs were re-analyzed to include only flexion-type supracondylar fractures. Medical records were reviewed and outcomes were evaluated at a mean of 9 years after the injury. In addition, we performed a systema…

Coronoid reconstruction with an osteochondral radial head graft.

BACKGROUND: Chronic coronoid deficiency can occur subsequent to coronoid fracture malunion/nonunion or to coronoid hypoplasia or dysplasia resulting from injury during development. Several surgical options have been described to treat this difficult condition, but results are equivocal. We hypothesized that a modified coronoid reconstruction using a radial head osteochondral graft could restore elbow stability and congruity and that a technique involving rigid internal fixation would promote graft union. METHODS: The coronoid was reconstructed using an osteochondral fragment from a frozen all…

Revisiting the 'bag of bones': functional outcome after the conservative management of a fracture of the distal humerus.

The best method of managing a fracture of the distal humerus in a frail low-demand patient with osteoporotic bone remains controversial. Total elbow arthroplasty (TEA) has been recommended for patients in whom open reduction and internal fixation (ORIF) is not possible. Conservative methods of treatment, including the 'bag of bones' technique (acceptance of displacement of the bony fragments and early mobilisation), are now rarely considered as they are believed to give a poor functional result. We reviewed 40 elderly and low-demand patients (aged 50 to 93 years, 72% women) with a fracture of …
