Upper limb pediatric fractures in 22 tertiary children's hospitals, China: a multicenter epidemiological investigation and economic factor analysis of 32,832 hospitalized children.

BACKGROUND: Fractures are the most common type of unintentional injury in children, with traumatic upper limb fractures accounting for approximately 80% of all childhood fractures. Many epidemiological investigations of upper limb fractures in children have been conducted, but with the development of society, the patterns of childhood fractures may have changed. This study aimed to analyze the epidemiology and economic cost factors of upper limb fractures in Chinese children. METHODS: We retrospectively reviewed children with upper limb fractures or old upper limb fractures hospitalized betwe

Assessment of the Damage Index and Its Dynamics in Patients with Non-Systemic Variants of Juvenile Arthritis during the Treatment with Biologics.

UNLABELLED: Juvenile idiopathic arthritis (JIA) is characterized by a high risk of joint destruction, extra-articular changes, and rapidly progressive disability. The CHAQ (Childhood Health Assessment Questionnaire) and  X-ray are often used to assess functional impairment and the degree of irreversible damage, but both methods have limitations. The Juvenile Arthritis Damage Index (JADI) has been developed to comprehensively assess the biological prognosis of JIA, which includes articular (JADI-A) and extra-articular (JADI-E) damage evaluation. The use of the JADI may be an effective tool for

Intramedullary Ulnar Fixation for the Treatment of Monteggia Fracture.

BACKGROUND: Although many pediatric Monteggia fractures can be treated nonoperatively, the presence of any residual radiocapitellar subluxation following ulnar reduction mandates a more aggressive approach to restore and maintain ulnar length. In younger children, restoration and maintenance of ulna length may be achieved through intramedullary fixation of the ulnar shaft. DESCRIPTION: A Steinmann pin or flexible intramedullary nail is introduced percutaneously through the olecranon apophysis and advanced within the medullary canal to the ulnar fracture site. If necessary, the ulnar length an

Open Reduction and Pin Fixation of Pediatric Lateral Humeral Condylar Fractures.

BACKGROUND: Lateral condylar fractures of the humerus are the second most common elbow fracture seen in pediatrics, behind supracondylar humeral fractures. All practicing orthopaedic surgeons should be able to identify and treat these injuries appropriately, as the intra-articular and transphyseal nature of this fracture pattern contributes to the relatively high complication rate. Treatment has evolved with time as classification systems better characterize the different types of lateral condylar fractures, aiding in decision-making for management. This video article reviews the diagnostic fi

Hyperselective Neurectomy for Spastic Flexion-Pronation Deformity of the Forearm and Wrist Using Local Anesthesia without a Tourniquet.

BACKGROUND: Spasticity in the upper limb following stroke or traumatic brain injury substantially impairs function and quality of life by affecting the flexor and adductor muscles. Spasticity commonly presents with deformities such as shoulder adduction and internal rotation, elbow flexion, forearm pronation, wrist flexion, finger flexion, and thumb adduction-commonly referred to as "thumb-in-palm" deformity. As the spasticity is incurable, surgical interventions aim to improve function and aesthetics, and facilitate daily care. Procedures such as tendon transfers and fractional lengthening ca

Evaluation of the torque-velocity test of the BTE-Primus as a measure of sincerity of effort of grip strength.

An inverse linear relationship exists between torque and velocity in the mid-ranges of an isotonic maximal contraction in a single joint movement (such as the elbow and knee). We hypothesized that submaximal effort does not produce a linear torque-velocity relationship because replicating a submaximal isotonic contraction requires an enormous amount of proprioceptive feedback and the nervous system may not be able to accurately replicate both force and speed of contraction. If this hypothesis is true, the torque-velocity test of the BTE-Primus would be an effective method for assessing sinceri

What factors influence outcomes in olecranon fractures in older adults? A cohort study of operative and nonoperative management.

BACKGROUND: Olecranon fractures occur more commonly in older individuals. For patients aged ≥70 years, operative management is often considered standard of care, although recent evidence has supported nonoperative treatment in frail and/or elderly patients. With evolving treatment indications, more granular evidence is beneficial to guide patient-specific decision-making. The aim of this investigation was to explore patient and treatment factors associated with outcomes after displaced, closed olecranon fractures in older individuals. METHODS: This retrospective cohort study with prospective

Clinical outcomes and return-to-play in dynamic anterior glenohumeral capsular ligament tensioning procedure for overhead-throwing athletes with traumatic shoulder dislocation on the throwing side.

BACKGROUND: Although arthroscopic Bankart repair for shoulder instability after a traumatic shoulder dislocation is a standard procedure, return-to-play rates for overhead-throwing athletes with shoulder instability on the throwing side have been reported to be lower than those in nonoverhead athletes or in nonthrowing shoulders. A recent technical report showed that the dynamic anterior glenohumeral capsular ligament tensioning (DAGHT) procedure in abducted external rotation, which mimics the late cocking phase of the throwing motion during surgery, may provide optimal tensioning of the anter

Superior clinical results for early arthroscopic treatment of grade IIIb and V acromioclavicular joint instability compared to delayed operative treatment.

BACKGROUND: Arthroscopy-assisted cortical fixation devices have been increasingly used in the operative management of both acute and chronic cases of acromioclavicular joint instability (ACJI). It has been hypothesized that delayed surgical management leads to inferior clinical and radiologic outcomes compared to acute treatment. The purpose of this study is to compare clinical and radiologic outcomes, scapula dyskinesia, and failure or revision rates of arthroscopically treated acute and chronic ACJI. METHODS: This retrospective study of prospectively collected data included all surgically t

The Effects of Fatigue on Muscle Synergies in the Shoulders of Baseball Players.

BACKGROUND: Muscle synergies are defined as the central nervous system's organizational structure for movement. Muscle synergies change after muscular fatigue, with certain synergies assuming the primary role to compensate for fatigue within another muscle synergy. Owing to the high eccentric forces imposed upon the external rotators (ie, posterior rotator cuff), pitchers that continue to throw while fatigued are at a significantly higher risk of shoulder and/or elbow injury; however, the neuromuscular compensation strategies of baseball players in response to fatigue are currently unknown. Short-term Clinical Outcome of Dual Plate Fixation in the Treatment of Proximal Humerus Fractures with Calcar Comminution.

OBJECTIVE: Calcar comminution has been considered to be the main cause of the failure of internal fixation and fracture nonunion in proximal humerus surgery. Anatomical reduction and increasing the strength of internal fixation is the key to success. The purpose of this study was to investigate the short-term clinical effect of dual plate fixation in the treatment of proximal humeral fractures with calcar comminution. METHODS: The data of 37 patients with proximal humeral fractures with calcar comminution, treated in our departments from July 2018 to April 2020, were retrospectively analyzed.

Clinical outcomes of reverse total shoulder arthroplasty for elective indications versus acute 3- and 4-part proximal humeral fractures: a systematic review and meta-analysis.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) has continued to increase in clinical utility and popularity as an effective treatment for cuff tear arthropathy (CTA), irreparable rotator cuff tears (RCTs), osteoarthritis, and acute 3- and 4-part proximal humeral fractures. Performing RTSA for acute proximal humeral fractures presents the unique challenges of tuberosity management, bone loss, and instability compared with elective indications such as CTA or irreparable RCTs. The purpose of this study was to compare the clinical outcomes, active range of motion (ROM), radiographic outcom

Biceps Tenodesis for Superior Labrum Anterior-Posterior Tear in the Overhead Athlete: A Systematic Review.

BACKGROUND: Superior labrum anterior-posterior (SLAP) lesion is a common shoulder injury, particularly in overhead athletes. While surgical management has traditionally consisted of SLAP repair, high rates of revision and complications have led to alternative techniques, such as biceps tenodesis (BT). While BT is commonly reserved for older nonoverhead athletes, indications for its use have expanded in recent years. PURPOSE: To determine functional outcomes and return-to-sport rates among overhead athletes after BT for SLAP tear. STUDY DESIGN: Systematic review. METHODS: A systematic review

Early functional improvements using continuous passive motion therapy after angular-stable plate osteosynthesis of proximal humerus fractures - results of a prospective, randomized trial.

BACKGROUND: The use of continuous passive motion therapy (CPM) has led to promising results in the early phase of rehabilitation after surgical treatment of rotator cuff tears and arthrolysis of the elbow. However, its use has not been proven in other pathologies of the upper extremity. Therefore, the aim of the underlying study was to evaluate the use of CPM therapy after plate osteosynthesis of proximal humeral fractures. METHODS: 95 patients with isolated proximal humerus fractures were enrolled in a prospective, randomized study. Patients were assigned to a treatment group with (n = 48, C
