Genetic determinants of Venezuelan equine encephalitis emergence.

Following a period of inactivity from 1973-1991, Venezuelan equine encephalitis (VEE) reemerged during the past decade in South America and Mexico. Experimental studies of VEE virus (VEEV) infection of horses with virus strains isolated during these outbreaks have revealed considerable variation in the ability of equine-virulent, epizootic strains to exploit horses as efficient amplification hosts. Subtype IC strains from recent outbreaks in Venezuela and Colombia amplify efficiently in equines, with a correlation between maximum viremia titers and the extent of the outbreak from which the vir

The West Nile virus: a lesson in emerging infections.

Since its identification in New York City in 1999, the West Nile virus has spread to 45 states and caused human infections in at least 44 states. West Nile virus is difficult to correctly diagnose without a high level of clinical suspicion and can cause severe debilitation or death in those with the most severe symptoms. This paper reviews the epidemiology, surveillance, prevention, clinical features, diagnosis, and treatment of this important emerging infection. Lessons learned from West Nile virus are discussed at the conclusion of the paper.

[Emerging and reemerging diseases: a health problem in the Americas].

In the Region of the Americas the emerging and reemerging infectious diseases that had the greatest impact on health, in terms of their incidence and the number of deaths that they caused during the five-year period of 1999-2003, were: malaria, yellow fever, dengue hemorrhagic fever, AIDS, anthrax, and SARS, as well as infection by hantavirus and by West Nile virus. The appearance of epidemics of emerging and reemerging diseases is related to biological, social, and economic factors. Growth in international trade, the movement of large numbers of people across national borders, the variability

Emerging pathways in the development of AIDS-related lymphomas.

The clinicopathological range of AIDS-related non-Hodgkin lymphomas (NHLs) includes systemic lymphomas, primary central-nervous-system lymphomas, primary effusion lymphoma, and plasmablastic lymphoma of the oral cavity. Most AIDS-related NHLs belong to one of three categories of high-grade B-cell lymphomas: Burkitt's lymphoma, centroblastic lymphoma, and immunoblastic lymphoma. The pathological heterogeneity of AIDS-related NHL reflects the heterogeneity of their associated molecular lesions. In AIDS-related Burkitt's lymphoma, the molecular lesions involve activation of c-MYC, inactivation of

Ultrasound and CT evaluation of emergent gallbladder pathology.

Ultrasound is the initial imaging modality of choice for the evaluation of suspected acute gallbladder disorders, and is often sufficient for correct diagnosis. CT also plays a vital role, however, in the evaluation of acute gallbladder pathology. CT is particularly useful in situations where ultrasound findings are equivocal. CT is also extremely valuable in the assessment of suspected complications of acute cholecystitis, particularly emphysematous cholecystitis, hemorrhagic cholecystitis, and gallbladder perforation, which are often very difficult diagnoses to establish at sonography. If CT

Emerging molecular targets in the treatment of bacterial meningitis.

The mortality and morbidity associated with bacterial meningitis have remained significant despite advances in antimicrobial chemotherapy and supportive care. A major contributing factor to this high mortality and morbidity is our incomplete understanding of the pathogenesis of this disease and its associated neurological sequelae. Most cases of bacterial meningitis develop as a result of haematogenous spread, but it is unclear how circulating bacteria cross the blood-brain barrier. Experimental animal studies indicate that two forms of neuronal injury, such as necrotic cortical injury and apo

Mortality associated with emergency abdominal surgery in the elderly.

INTRODUCTION: Elderly patients with life-threatening abdominal disease are undergoing emergency surgery in increasing numbers, but emergency procedures generally are associated with increased morbidity and mortality. We carried out a retrospective and prospective study at a tertiary centre in Spain to analyze the factors contributing to death after emergency abdominal surgery in elderly patients and to determine whether there were differences in the death rate between those aged 70-79 years and those aged 80 years and older. METHODS: The study population comprised 710 patients aged 70 years o

Propofol for procedural sedation in children in the emergency department.

STUDY OBJECTIVE: We determine the safety and efficacy of propofol sedation for painful procedures in the emergency department (ED). METHODS: A consecutive case series of propofol sedations for painful procedures in the ED of a tertiary care pediatric hospital from July 2000 to July 2002 was performed. A sedation protocol was followed. Propofol was administered in a bolus of 1 mg/kg, followed by additional doses of 0.5 mg/kg. Narcotics were administered 1 minute before propofol administration. Adverse events were documented, as were the sedation duration, recovery time from sedation, and total

Homeopathic treatment in emergency medicine: a case series.

Following a multiple-casualty construction disaster in Israel, members of The Center of Integrated Complementary Medicine joined in the emergency activity of the Shaare Zedek Medical Center. They administered homeopathic treatment to injured patients to supplement conventional orthopaedic treatment. This was to our knowledge the first time that complementary medicine had been used officially in conjunction with conventional medicine in an emergency situation. Our objective is to report and summarize the rationale, procedures and outcome of the complementary medicine intervention. Fifteen ortho

Emergency management of eclampsia and severe pre-eclampsia.

Eclampsia and severe pre-eclampsia are rare, but potentially life-threatening conditions that emergency physicians must be able to diagnose and treat promptly, because initial presentations to the ED are real possibilities. The treatment of the major complications of this disorder, hypertension and seizures, have been the focus of much research. Magnesium sulphate is now the first line agent for acute treatment and prophylaxis of seizures in eclampsia and pre-eclampsia. Severe pre-eclampsia should be treated with magnesium to prevent progression to eclampsia. Severe hypertension requires treat

[Psychopharmacological treatment in the pre-clinical emergency medicine].

Psychiatric emergencies are a common cause for a call for the pre-clinical emergency physician (EP). However, previous studies reported serious problems in diagnosing and particularly treating these patients. Although evidence-based treatment guidelines and algorithms exist for the most important somatic disorders the EP has to deal with, they do not for psychiatric emergencies. It is the aim of this review, subsequent to an extensive overview on the available literature, to provide the reader with differentiated suggestions for pharmacological crisis intervention as well as with treatment rec…

Emergence of echovirus type 13 as a prominent enterovirus.

In 2001, increased activity of the rarely detected enterovirus echovirus type 13 (E13) was observed in the United States. This article describes the epidemiologic, clinical, and genetic characteristics of E13 activity in the United States in 2001, compared with E13 activity abroad in 2000-2002. In the United States, E13 accounted for 376 (24%) of the 1584 enterovirus isolates reported in 2001 (29% of the reported isolates had a known serotype), compared with 74 isolates reported during 1970-2000. Five states reported aseptic meningitis outbreaks associated with E13, for a total of 521 cases. A

Factors contributing to the occurrence of emerging infectious diseases.

Emerging infectious diseases (EIDs) have been receiving increasing attention for more than two decades. Such attention has resulted from observations of increasing resistance of microorganisms to the usual antibiotics, the identification of formerly unknown disease agents and the diseases they cause, and the realization that the concept of globalization includes global exposure to disease agents formerly confined to small, endemic, or remote areas. Sadly, in the fall of 2001, the potential for using microbial agents as instruments of terror and destruction became obvious with the incidents of

Emerging technologies in surgical planning for breast cancer.

BACKGROUND: Computer-assisted diagnosis (CAD) has been applied to mammography and chest radiography to improve diagnostic accuracy, and has the potential to improve the accuracy of magnetic resonance imaging (MRI) for detecting nodal metastases in women with newly diagnosed breast cancer. METHODS: Six women with newly diagnosed breast cancer underwent MRI of the breast and axilla. Radiologists applied digital tissue recognition (DTR), a form of CAD, to the MR images to detect nodal metastases. The results were compared with clinical staging and to surgical pathology. RESULTS: The accuracy of
