Superselective Craniofacial Nerve Decompression in a Pediatric Patient: A Case Report.

Peripheral nerve release has emerged as a viable treatment option for pharmacologically refractory migraines secondary to craniofacial compression neuropathies. Although occipital nerve decompression is well described, management of supraorbital and zygomaticotemporal nerve compression is less characterized despite its relevance in patients with multi-nerve compression. Conventional techniques rely on long, blepharoplasty-like incisions to address both supraorbital and temporal compression sites. However, extensive areas of dissection may increase complication rates and reduce aesthetic outcom

The yield of CT head scans for intracranial haemorrhage in anticoagulated patients presenting with head injury.

AIM: Anticoagulated patients presenting with head injury have previously undergone routine CT imaging, but the 2023 NICE guidelines now permit a more selective approach. Real-world data on CT yield in this cohort, particularly in the absence of other clinical risk factors, remain limited. We aim to evaluate the diagnostic yield of CT head imaging for intracranial haemorrhage (ICH) in anticoagulated patients presenting with head injury. MATERIALS AND METHODS: A retrospective cohort study was conducted at a large NHS Trust, identifying all CT head scans between January-March 2025 with clinical

Pilot Study: Endoscopic Anatomical Study of the Lower Premasseter Space and Buccal Space for Sub-superficial Musculo-aponeurotic System Face Lift: Observations Based on Live Dissections.

BACKGROUND: Sub-superficial musculo-aponeurotic system dissections of the lower premasseteric space (LPMS) and buccal space (BS) performed under direct visualization are challenging because of the close relationship between neurovascular bundles and the fascioligamentous system, making these structures prone to injury. This pilot study aims to document the endoscopic anatomy of these spaces through intraoperative visualization, enabling clearer identification and description of key structures. METHODS: Five representative cases were selected from a series of 80 sub-superficial musculo-aponeur

Cytotoxic Lesion of the Corpus Callosum After Hospital-Administered Intravenous Fentanyl for Trigeminal Neuralgia: A Case Report and Focused Review.

OBJECTIVES: Fentanyl-associated neurotoxicity is increasingly recognized across illicit, inhalational, transdermal, pediatric, and delayed posthypoxic exposures, but isolated splenial CLOCC after hospital-administered intravenous fentanyl has not been well characterized. METHODS: This is a single-patient case report with a focused review of fentanyl-associated neurotoxic syndromes and proposed mechanisms. RESULTS: A 41-year-old woman with trigeminal neuralgia presented with severe facial pain. After droperidol, metoclopramide, and metamizole failed to control symptoms, intravenous fentanyl (

Delayed repair of the facial nerve and its negative impacts on nerve and muscle regeneration.

BACKGROUND: In this experimental protocol, we evaluated the immediate and delayed repair of the buccal branch of the facial nerve (BBFN) with heterologous fibrin biopolymer (HFB) as a coaptation medium and the use of photobiomodulation (PBM), performing functional and histomorphometric analysis of the BBFN and perioral muscles. METHODS: Twenty-eight rats were divided into eight groups using the BBFN bilaterally (the left nerve was used for PBM), namely: G1 - control group, right BBFN (without injury); G2 - control group, left BBFN (without injury + PBM); G3 - Denervated right BBFN (neurotmesi

DRESS Syndrome: Renal Involvement in Two Cases - A Comprehensive Analysis and Literature Review of Improved Diagnosis and Treatment.

BACKGROUND Drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome is a rare hypersensitivity reaction involving the skin and various visceral organs; the kidneys are the second most affected organ. Many drugs are reported to be associated with DRESS, particularly antiepileptic agents and allopurinol. Certain human leukocyte antigen (HLA) haplotypes, in combination with a particular drug, can further contribute to an increased risk of DRESS. Symptoms often develop 2 to 8 weeks after drug initiation. If diagnosis is delayed, DRESS can be a life-threatening condition. CASE REPORT

Post-Traumatic Intracranial Pseudoaneurysm Presenting as Epistaxis.

BACKGROUND: Epistaxis is a frequent presenting complaint in the Emergency Department (ED). Roughly 60% of the population will suffer from epistaxis in their lifetime. The most common causes of epistaxis include nose picking, facial trauma, foreign bodies, and coagulopathies. There are other causes that are much less common, such as intracranial pseudoaneurysms. There are multiple causes that precipitate intracranial pseudoaneurysm formation, with head trauma accounting for less than 1% of inciting events. CASE REPORT: A 24-year-old female with history of traumatic brain injury with associated

Prevention of Superior Petrosal Vein Injury during Microvascular Decompression for Trigeminal Neuralgia: Operative Nuances.

Background  The superior petrosal vein (SPV) often obscures the surgical field or bleeds during microvascular decompression (MVD) for the treatment of trigeminal neuralgia. Although SPV sacrifice has been proposed, it is associated with multiple complications. We have performed more than 4,500 MVDs, including approximately 400 cases involving trigeminal neuralgia. We aimed to describe our operative technique and nuances to avoid SPV injury. Methods  We have provided a detailed description of our institutional protocol, including the anesthesia technique, neurophysiologic monitoring, patient po

A Rare Case of Oculomotor Nerve Palsy after Endovascular Treatment in a Patient with Internal Carotid Artery Dissection.

BACKGROUND: The oculomotor nerve has two major fibers: the outer autonomic fiber innervates the ciliary muscles and sphincter pupillae involved in pupil constriction, and the inner somatic fiber innervates the levator palpebrae superioris in the eyelid and four extraocular muscles involved in oculomotor dysfunction. We present a rare case of oculomotor nerve palsy with internal carotid artery (ICA) dissection and discuss the vascular anatomical feature of interest to be considered during treatment. CASE REPORT: A 56-year-old man presented with language impairment, right visual field loss, and

Frequency of Cervical Spine Injuries in Pediatric Craniomaxillofacial Trauma.

PURPOSE: In pediatric patients with craniomaxillofacial (CMF) trauma, evaluation for cervical spine injury (CSI) is critical, but there are no studies investigating CSI in this unique population. The aim of this study was to measure the frequency of CSI in the pediatric CMF fracture population. MATERIALS AND METHODS: A retrospective cohort study of all pediatric patients who presented to the Johns Hopkins Hospital Emergency Department (Baltimore, MD) with CMF fractures were examined for concurrent CSIs. Patient charts were reviewed for mechanism of injury, type and level of CSI, type and loca

Increased brain glucose metabolism in chronic severe traumatic brain injury as determined by longitudinal 18F-FDG PET/CT.

Little is known about changes in glucose metabolism in patients with chronic severe traumatic brain injury (sTBI). It remains to be elucidated how neurological manifestations of sTBI are associated with brain glucose metabolism during longitudinal follow-up. We show here that neurological manifestations are associated with changes of brain glucose metabolism by using two serial 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) images. In this longitudinal observational study, two serial 18F-FDG PET/CT images from each of 45 patients were analyzed for whol

Prognostic Indices for Predicting Facial Nerve Outcome following the Resection of Large Acoustic Neuromas.

This study analyzes the simple ratio of anterior-to-posterior extension of large (>2.5 cm) acoustic neuromas relative to the internal auditory canal (ICA; anterior-posterior [A/P] index) as a tool for predicting risk of facial nerve (FN) injury. In total, 105 patients who underwent microsurgical resection for large acoustic neuromas were analyzed retrospectively. House-Brackmann (HB) scores were assessed immediately postoperatively, at 1 month, and at 1 year. Lateral-medial, inferior-superior, A/P, and maximum diameters were measured from preoperative magnetic resonance images. These measureme

Recipient vessel selection in the difficult neck: Outcomes of external carotid artery transposition and end-to-end microvascular anastomosis.

BACKGROUND: Selection of recipient vessels for head and neck microvascular surgery may be limited in the previously dissected or irradiated neck. When distal branches of the external carotid artery (ECA) are unavailable, additional options for arterial inflow are needed. Here we propose high ligation of the ECA and transposition toward the lower neck as an alternative. METHODS: After obtaining institutional approval, patients who underwent head and neck tumor resection and simultaneous free flap reconstruction were identified over a 5-year period. Patients whose recipient artery was listed in

Acute injury of the neck: anatomical and pathological basis of pain.

Headache, upper torso and upper limb pain can all originate in lesions of the cervical spine. The pain distribution is similar whether the cause is traumatic or degenerative. Nociceptive fibres are present in many cervical spine structures including the disc annulus, facet joint capsules, muscles, meninges, arteries, nerve roots and dorsal root ganglia. Such nerves are not present in the disc nucleus, facet articular cartilages and the ligamentum flavum. Central nervous connections between the trigeminal nucleus and upper cervical sensory input provide an explanation for some forms of headache
