Elbow trauma in children is one of the most commonly encountered musculoskeletal injuries in pediatric radiology. However, elbow injuries in children can be misdiagnosed due to secondary ossification centers unique to pediatric patients. Familiarity of the normal elbow anatomy in children is crucial for an accurate diagnosis. This article seeks to improve diagnostic accuracy of these elbow injuries by reviewing the secondary ossification centers in the elbow in the pediatric population, followed by a discussion of commonly encountered acute and chronic fractures, dislocations or other injuries

CASE: A twenty-eight-year-old man presented with a Pipkin type-III femoral head fracture and open floating knee injury. Initially debridement was performed, during which the extensor apparatus could not be salvaged. The top priority in management of the floating knee was achievement of adequate soft-tissue coverage followed by skeletal stabilization as staged procedures. Because the femoral head was split, dislocated, comminuted, and deemed nonreconstructible, arthroplasty was considered the best option. The extensor mechanism damage was addressed by reconstruction using an iliotibial tract fl

The authors describe two cases of perilunate dislocation with transscaphoid fractures both presenting following hyperextension injuries within 4 days of each other at a district general hospital. After initial management in accident and emergency, both patients underwent open reduction and internal fixation under general anaesthetic, with plaster cast immobilisation and careful clinical follow-up. Given the relative rarity of this injury, the significance of the radiological findings, the degree of associated bony and ligamentous damage and the need for prompt intervention may be overlooked. T

The metacarpophalangeal (MP) joint is resistant to injury due to its strong capsuloligamentous structures, which include the volar plate and deep transverse metacarpal and collateral ligaments. Complex MP joint dislocations are, by definition, irreducible by closed means and require open reduction, as the volar plate becomes entrapped between the metacarpal head and proximal phalanx. The dorsal approach may offer the following advantages: 1) reduced risk to palmarly displaced neurovascular structures, 2) facilitated management of osteochondral fractures, and 3) full exposure of the volar plate

An 80-year-old woman was admitted to our hospital with abnormal shadow on chest X-ray 8 years after a chest trauma during which multiple ribs on the right side were fractured causing hemothorax. A diagnosis of right delayed traumatic diaphragmatic hernia was based on the findings on plain X-ray and multislice computed tomography. We performed surgery via thoracic approach with thoracoscopic assist. The transverse colon, liver, and omentum were dislocated into the right thoracic cavity and hardly adhered to the lung. We successfully repaired the ruptured diaphragm. The postoperative course was

Ligamentous injuries at the ankle and subtalar joint range from simple sprains to severe talar dislocations. While lateral ankle sprains are among the most frequently encountered injuries and do not pose a greater diagnostic problem, the surgeon must be suspicious not to overlook associated ligamentous injuries at the subtalar and midtarsal level that may result in chronic painful conditions. Syndesmotic instabilities with or without ankle fractures must be assessed carefully and treated properly, since these are prearthrotic conditions. In the treatment of chronic ankle or subtalar instabilit

From 1985 to 1995, 417 patients with dislocated medial femoral neck fractures (Garden III-IV) were treated with hemiarthroplasty using a Biolox ceramic head. The average patient age at the time of operation was 81.5 years. A total of 140 survivors was available for follow-up examination with a mean prosthesis longevity of 55.8 months. The Harris hip score recorded a mean of 70.6 points. At the time of follow up, 5 patients had severe hip pain, and in 8 the roentgenographic examination revealed protrusio acetabuli. Five of these 8 patients underwent revision surgery for replacement of the cup,

The glenoid labrum contributes to shoulder stability by increasing the glenoid surface and providing attachments for the shoulder capsule and various ligaments and tendons. Overhead arm activities, especially throwing, can stress the shoulder excessively and cause labral injury. Symptoms include poorly localized pain, exacerbated by overhead and behind-the-back motions, and popping, catching, or grinding. Imaging includes AP, outlet, and axillary radiographs, but CT arthrogram or MR arthrogram is also recommended. Fractures and dislocations should be referred. In other labral injuries, 2 to 4

Between 1988 and 1992, 14,765 impacted third molars were removed in the Department for Maxillofacial Surgery of the University Hospital and Dental Clinic in Zurich. A report on significant complications is given and compared to previous studies. Special attention is given to major complications which needed hospitalisation and/or surgical intervention, like secondary infection of the head and neck region, mediastinal emphysema, mandibular fractures and dislocation of third molars, which could not be removed under local anaesthesia. Mediastinitis, excessive bleeding or permanent damage to crani

An army mechanic sustained a penetrating mandibular injury, associated with right atlantooccipital dislocation, when the inner tube of a military truck tyre burst. It is thought that he had used a sledgehammer, having wielded it lengthwise with the free end of its hollow cylindrical handle held uppermost, to reposition the locking ring of the fully inflated tyre. In all probability, the sudden release of highly pressurised air disloged the locking ring and forced the circular end of the handle into his right lower jaw, thereby fracturing the mandible and causing an acute hyperextension injury

Vascular injury associated with select blunt trauma-induced orthopedic injuries is thought to occur frequently enough to require angiography in most cases. We sought to establish the actual frequency of such vascular injuries by reviewing all orthopedic and vascular patients seen during 4 years at one institution. A total of 569 at-risk parajoint fractures or dislocations were seen with a 1.5% incidence of vascular injury; all were recognized on clinical examination and vascular consultation then obtained. Vascular injury in this setting is uncommon, associated with particular clinical present

We report a case of delayed presentation of vascular injury from a superficial dog bite of the upper extremity in a five-year-old male. This example of a significant arterial injury, in the setting of seemingly minor trauma, is the first known report of blunt arterial trauma owing to a dog bite in any age group. Blunt arterial trauma in children is rare unless associated with fractures, dislocations, or massive crush injuries. Additionally, persistent pulses following arterial occlusion are rare and can potentially lead to misdiagnosis of a serious arterial injury. A brief review of pediatric

The radiology of total hip replacement (THR) and its complication is reviewed in conjunction with a long-term follow-up study on 402 patients with 501 prostheses. The indications, contraindications, biomechanics, and operative management of these patients is discussed. Clinical complications such as deep vein thrombosis, pulmonary embolism, and hemorrhage are mentioned. Postoperative infections including granulomatous pseudotumors, dislocations and fractures, true loosening of the prosthesis, and heterotopic bone formation (HBF) are discussed and illustrated. The importance of differentiating

BACKGROUND: There are conflicting reports regarding the incidence of Monteggia fractures by Bado type and their associated risks of nerve and vascular injury and other complications.
METHODS: A registry was queried at a level I trauma center for Monteggia fracture codes. A retrospective chart and radiographic review were then conducted to correctly identify Monteggia fractures. The primary outcome was incidence by Bado type with secondary outcomes being associated injuries.
RESULTS: 69 patients sustained Bado I (42), Bado II (15), Bado III (8), and Bado IV (4) injuries. 83 % had a high-energ

PURPOSE: Current motor vehicle safety systems primarily focus on preventing life-threatening and serious injury during crashes, but occupants may still sustain less serious injury. Upper extremity injury is common in occupants involved in motor vehicle crashes. The purpose of this study was to compare occupants with scapula fractures to occupants with other shoulder injuries.
METHODS: We used data from the Crash Injury Research and Engineering Network (CIREN) database (1997-2008) to compare two groups of occupants: (1) occupants with only scapula fractures and (2) occupants with shoulder inju

OBJECTIVES: This study was designed to assess the results of surgical treatment for type 2-3 coronoid process fractures and to identify factors that might influence the outcome.
METHODS: Thirteen male patients (mean age 35 years; range 17 to 53 years) were treated with open reduction and internal fixation for displaced coronoid fractures. According to the Regan-Morrey classification, the fractures were type 2 in nine patients (69.2%), and type 3 in four patients (30.8%). Nine patients had associated injuries (elbow dislocation, radial head or olecranon fractures, and/or ligamentous injuries).

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) for chronic locked anterior shoulder dislocations (CD) remains challenging due to glenoid bone loss, soft-tissue contractures, and risk of post-operative stiffness and instability. Outcomes of rTSA for CD remain poorly defined, particularly compared with rTSA for proximal humerus fractures (PHF), another high-risk indication associated with inferior results relative to traditional rTSA indications. This study evaluated clinical and radiographic outcomes of rTSA for CD vs. PHF. We hypothesized that patients with CD would demonstrate inferio

PURPOSE: Pediatric complete cervical spinal cord injury (cSCI) is rare and associated with severe morbidity and mortality, yet limited data exist to guide management. This study aimed to describe national injury characteristics, management stratification, and inpatient outcomes among children with complete cSCI.
METHODS: Pediatric patients (< 18 years) with complete cSCI were identified from the American College of Surgeons Trauma Quality Program database (2019-2021). Cases with severe head trauma or pre-hospital deaths were excluded. Patients were grouped into surgical and non-surgical cohor

OBJECTIVES: Randomized controlled trials (RCTs) investigating cemented (CHA) vs press-fit (PFHA) fixation of femoral implants in hemiarthroplasty have major clinical implications. The purpose of this study is to assess the statistical fragility of reported outcomes in RCTs comparing CHA and PFHA fixation.
METHODS: RCTs published until June 2024 comparing PFHA and CHA fixation were identified systematically from PubMed. Fragility indices were calculated as the minimum number of event reversals needed to alter significance for a variable. Fragility quotients were computed by dividing fragility
