Neuropsychiatric-pharmacologic synergy: interaction between psychiatric polypharmacy and cancer treatment-related neurotoxicity.

Altered mental status (AMS) in oncology populations frequently reflects complex interactions among medical, pharmacologic, and neuropsychiatric factors. Accurate identification of the primary contributors is crucial to minimize morbidity. We report the case of a man in his 60s with a relapsed plasma cell malignancy, status post autologous stem-cell transplant, who experienced recurrent AMS during ongoing combination chemotherapy and psychiatric polypharmacy. Neuroimaging excluded structural or vascular lesions. Laboratory evaluation revealed intermittent mild renal dysfunction without metaboli

Involuntary Choreiform Movements in Hyperglycemia: A Rare Neurological Manifestation of Diabetes.

Hemichorea represents a rare but clinically significant hyperkinetic movement disorder in diabetic patients, characterized by abrupt-onset, nonpatterned contractions involving both craniofacial and appendicular musculature. An 82-year-old Asian male who developed progressively worsening choreoathetoid movements with striking left-sided craniofacial predominance, accompanied by ipsilateral limb involvement. Comprehensive neurological assessment showed preserved sensorimotor function despite the dramatic movement disorder. Biochemical profiling revealed markedly elevated serum glucose (28.6 mmol

HOARDING DISORDER: HOLD YOUR HORSES! A NEURODIVERGENT AND SOCIALLY MEDIATED CONDITION REQUIRING RETHINK AND REFORM.

BACKGROUND: Hoarding disorder (HD) is increasingly recognized as a distinct, multifactorial condition. Recent findings in neuroimaging, psychopathology, compulsive acquisition, suicidality, stigma, and social context call for a redefinition of HD beyond its traditional association with obsessive-compulsive disorder. SUBJECTS AND METHODS: This narrative review synthesizes recent clinical, neurobiological, and sociocultural literature, including latent class analysis and resting-state fMRI studies, exploring cognitive dysfunction, comorbidities, suicidality, stigma, and environmental context.

Segmentation of the human tongue musculature using MRI: Field guide and validation in motor neuron disease.

This work addresses the challenge of reliably measuring the muscles of the human tongue, which are difficult to quantify due to complex interwoven muscle types. We introduce a new semi-automated method, enabled by a manually curated dataset of MRI scans to accurately measure five key tongue muscles, combining AI-assisted, atlas-based, and manual segmentation approaches. The method was tested and validated in a dataset of 178 scans and included segmentation validation (n = 103) and clinical application (n = 132) in individuals with motor neuron disease. We show that people with speech and swall

The dorsomedial prefrontal cortex promotes self-control by inhibiting the egocentric perspective.

The dorsomedial prefrontal cortex (dmPFC) plays a crucial role in social cognitive functions, including perspective-taking. Although perspective-taking has been linked to self-control, the mechanism by which the dmPFC might facilitate self-control remains unclear. Using the multimodal neuroimaging dataset from the Human Connectome Project (Study 1, N =978 adults), we established a reliable association between the dmPFC and self-control, as measured by discounting rate-the tendency to prefer smaller, immediate rewards over larger, delayed ones. Experiments (Study 2, N = 36 adults) involving hig

Hemispheric annealing and lateralization under psychedelics (HEALS): A novel hypothesis of psychedelic action in the brain.

Current models of psychedelic action in the brain propose changes along the dorsal-ventral and anterior-posterior axes but neglect to address the lateral axis. This article proposes a novel model of psychedelic action called HEALS (Hemispheric Annealing and Lateralization Under Psychedelics) which involves the reversal of the typical hierarchical relationship between the two hemispheres of the brain. In typical modes of consciousness, the hemispheres act in parallel process with the left predominating. Under psychedelics, as well as in other altered states of consciousness (ASCs), this hierarc

A child with recurrent headache, fever and diffuse meningeal enhancement on MRI.

Juvenile neurolupus presents primarily with neuropsychiatric manifestations which may also be the initial presentation. Such primary neuropsychiatric SLE (NPSLE) events are a consequence either of microvasculopathy and thrombosis, or of autoantibodies and inflammatory mediators. Diagnosis of NPSLE requires the exclusion of other causes, and clinical assessment directs the selection of appropriate investigations. These investigations include measurement of autoantibodies, analysis of cerebrospinal fluid, electrophysiological studies, neuropsychological assessment and neuroimaging to evaluate br

Hyperthyroidism and depression: a clinical case of atypical thyrotoxicosis manifestation.

The relationship between psychiatric symptoms and thyroid function has been well known and studied since antiquity. The common view is that clinical hypothyroidism is associated with depressive symptoms, whereas the psychiatric manifestations of hyperthyroidism are agitation, emotional lability, hyperexcitability, occasionally accompanied by angry outbursts, and euphoria. The case here reported overturns this conventional medical knowledge. A 73-year-old Italian woman experienced a severe major depressive episode with psychotic and melancholic features during laboratory thyrotoxicosis. No clas

Eye Signs in Stroke.

A large part of the central nervous system is involved in the normal functioning of the vision, and hence vision can be affected in a stroke patient. Transient visual symptoms can likewise be a harbinger of stroke and prompt rapid evaluation for the prevention of recurrent stroke. A carotid artery disease can manifest as transient monocular visual loss (TMVL), central retinal artery occlusion (CRAO), anterior ischemic optic neuropathy or ocular ischemic syndrome (OIS). Stroke posterior to the optic chiasm can cause sectoranopias, quadrantanopias, or hemianopias, which can be either congruous o

123I-FP-CIT Brain SPECT Findings in Succinic Semialdehyde Dehydrogenase (SSADH) Deficiency.

BACKGROUND: Succinic semialdehyde dehydrogenase (SSADH) deficiency is a rare autosomal recessive disorder. Neuroimaging findings are commonly considered rather non-specific. To date, no neuroreceptorial brain imaging with 123I-FP-CIT(DaTScan) is known in subjects with SSADH deficiency. METHODS: A 30-year-old man gained our attention to rule out any potential nigrostriatal dopaminergic presynaptic pathway alterations in a clinical context of a γ-hydroxybutyric aciduria. He showed impossibility to the autonomous gait, head and trunk retropulsion, lower limbs strength deficit, verbal and upper l

The pontine-driven somatic gaze tract contributes to affective processing in humans.

The relevance of subcortical structures for affective processing is not fully understood. Inspired by the gerbil retino-raphe pathway that has been shown to regulate affective behavior and previous human work showing that the pontine region is important for processing emotion, we asked whether well-established tracts in humans traveling between the eye and the brain stem contribute to functions beyond their conventionally understood roles. Here we report neuroimaging findings showing that optic chiasm-brain stem diffusivity predict responses reflecting perceived arousal and valence. Analyses o

Systemic Mitochondrial Oxidative Phosphorylation Protein Levels Correlate with Neuroimaging Measures in Chronically HIV-Infected Individuals.

Few studies have examined systemic mitochondrial function in conjunction with brain imaging in human immunodeficiency virus (HIV) disease. Oxidative phosphorylation enzyme protein levels of peripheral blood mononuclear cells were measured in association with neuroimaging indices in 28 HIV+ individuals. T1-weighted magnetic resonance imaging yielded volumes of seven brain regions of interest; diffusion tensor imaging determined fractional anisotropy (FA) and mean diffusivity (MD) in the corpus callosum (CC). Higher nicotinamide adenine dinucleotide dehydrogenase levels correlated with lower vol

Reduplicative paramnesia for places: A comprehensive review of the literature and a new case report.

Reduplicative paramnesia for places (i.e., the delusional belief that a place has been duplicated or exists in two different locations) is a rare disorder observed in neurological patients. We review the existing literature on the topic, highlighting commonalities and differences among the 51 cases published since the first report in 1903. Our results highlight the combination of multiple factors in the pathogenesis of this monothematic spatial delusion. From a neurological perspective, a crucial role is played by damage to the right frontal and temporal lobe. Deficits of non-verbal memory and

[The hypothalamus in Huntington's disease].

INTRODUCTION: Disorders affecting sleep and the circadian rhythm, autonomic clinical signs and symptoms, and neuroendocrine alterations are frequent characteristics in Huntington's disease, some of which present in early stages of the disease. It is reasonable to think that some of these features could result from a hypothalamic dysfunction affecting the centre regulating sleep, metabolism and the autonomic nervous system. AIM: The study presents the evidence available to date that suggests the involvement of a hypothalamic disorder in Huntington's disease. DEVELOPMENT: Histopathological, ho

Clinical, genetic and imaging findings identify new causes for corpus callosum development syndromes.

The corpus callosum is the largest fibre tract in the brain, connecting the two cerebral hemispheres, and thereby facilitating the integration of motor and sensory information from the two sides of the body as well as influencing higher cognition associated with executive function, social interaction and language. Agenesis of the corpus callosum is a common brain malformation that can occur either in isolation or in association with congenital syndromes. Understanding the causes of this condition will help improve our knowledge of the critical brain developmental mechanisms required for wiring

Emergency EC-IC bypass for symptomatic atherosclerotic ischemic stroke.

Previous studies have shown that extracranial-intracranial (EC-IC) bypass surgery has no preventive effect on subsequent ipsilateral ischemic stroke in patients with symptomatic atherosclerotic internal carotid occlusion and hemodynamic cerebral ischemia. A few studies have assessed whether an urgent EC-IC bypass surgery is an effective treatment for main trunk stenosis or occlusion in acute stage. The authors retrospectively reviewed 58 consecutive patients who underwent urgent EC-IC bypass for symptomatic internal carotid artery or the middle cerebral artery stenosis or occlusion between Jan

In vivo evidence for greater amphetamine-induced dopamine release in pathological gambling: a positron emission tomography study with [(11)C]-(+)-PHNO.

Drug addiction has been associated with deficits in mesostriatal dopamine (DA) function, but whether this state extends to behavioral addictions such as pathological gambling (PG) is unclear. Here we used positron emission tomography and the D3 receptor-preferring radioligand [(11)C]-(+)-PHNO during a dual-scan protocol to investigate DA release in response to oral amphetamine in pathological gamblers (n=12) and healthy controls (n=11). In contrast with human neuroimaging findings in drug addiction, we report the first evidence that PG is associated with greater DA release in dorsal striatum (

Feasibility of geometric-intensity-based semi-automated delineation of the tentorium cerebelli from MRI scans.

This paper describes a feasibility study of a method for delineating the tentorium cerebelli in magnetic resonance imaging (MRI) brain scans. The tentorium cerebelli is a thin sheet of dura matter covering the cerebellum and separating it from the posterior part of the temporal lobe and the occipital lobe of the cerebral hemispheres. Cortical structures such as the parahippocampal gyrus can be indistinguishable from tentorium in magnetized prepared rapid gradient echo and T1-weighted MRI scans. Similar intensities in these neighboring regions make it difficult to perform accurate cortical anal
