Minimally invasive spinal surgery: a historical perspective.

The concept of minimally invasive spinal surgery embodies the goal of achieving clinical outcomes comparable to those of conventional open surgery, while minimizing the risk of iatrogenic injury that may be incurred during the exposure process. The development of microscopy, laser technology, endoscopy, and video and image guidance systems provided the foundation on which minimally invasive spinal surgery is based. Minimally invasive treatments have been undertaken in all areas of the spinal axis since the 20th century. Lumbar disc disease has been treated using chemonucleolysis, percutaneous

Off-pump coronary artery bypass surgery may produce a hypercoagulable patient.

BACKGROUND: The incidence of thromboembolic events following traditional open heart surgery has not been clinically significant. However, with beating heart surgery, for which cardiopulmonary bypass (CPB) is not required, the incidence of spontaneous intravascular thrombosis may be similar to that encountered after general surgeries. Compounding this risk is that many cases of off-pump coronary artery bypass (OPCAB) surgery are reserved for the elderly patient with multiple comorbidities. The few studies to date that have assessed the coagulation profile in OPCAB patients have been limited to

Adhesion-related hospital readmissions after abdominal and pelvic surgery: a retrospective cohort study.

BACKGROUND: Adhesions after abdominal and pelvic surgery are important complications, although their basic epidemiology is unclear. We investigated the frequency of such complications in the general population to provide a basis for the targeting and assessment of new adhesion-prevention measures. METHODS: We used validated data from the Scottish National Health Service medical record linkage database to identify patients undergoing open abdominal or pelvic surgery in 1986, who had no record of such surgery in the preceding 5 years. Patients were followed up for 10 years and subsequent readmi

The use of lasers in pediatric surgery.

This report is a condensation of the Fred McLoed Lecture given at the Annual Meeting of the Canadian Association of Paediatric Surgeons. It briefly cites the basic fundamentals of lasers, the characteristics of those used in surgery, and their delivery systems. The advantages of laser surgery include the sealing of blood vessels, lymphatics, and nerve ends when used for cutting. This results in a relatively dry field of surgery and less postoperative pain. There is reduced opportunity for contaminants in a clean wound, and no spreading of infection in a contaminated one. Surgery can be carried

Near Complete Biliopancreatic Segment Necrosis: A Rare and Unknown Entity Observed After Roux-en-Y Gastric Bypass-A Case Report.

BACKGROUND: Roux-en-Y gastric bypass is one of the common bariatric surgery procedures performed worldwide. Necrosis of the biliopancreatic or Roux limbs in the late period after RYGB is a rarely observed condition, the most common causes of which are postoperative adhesion, inflammatory entities, volvulus, intussusception, and internal herniations. The goal of surgical treatment is the removal of the necrotic segment and the reconstruction of biliopancreatic or gastric drainage. CASE PRESENTATION: Here, we present a rare case in which a patient developed complete necrosis of the third and fo

PREDICTORS OF INTRAOPERATIVE HYPERTENSION IN NECK SURGERY: A SINGLE CENTER RETROSPECTIVE STUDY.

Intraoperative hypertension (IOHTA) during neck surgery is undesirable because this type of surgery has a propensity to bleed profoundly even in normotensive anesthesia. The purpose of our study was to detect predictors of IOHTA in patients undergoing neck surgery. This single center retrospective study included 880 adult patients who underwent neck surgery under general anesthesia. The impact of gender, age, comorbidity, difficult tracheal intubation, anesthesia duration, and induced hypotension on IOHTA was studied. IOHTA was present in 57 (6.7%) patients. The highest incidence of IOHTA was

INTRODUCTION: Pterygium surgery is a closed globe surface surgery. Although it is perfectly standard and followed by excellent results, it is not exempt from the general rule that there is no surgery without risk. It therefore seems important to integrate simulation as a pedagogical tool for training ophthalmology residents in pterygium surgery. AIM: To evaluate the effectiveness of procedural simulation as a training tool for pterygium surgery. RESULTS: During the study period, eight residents participated in the three workshops. The global assessment of residents' knowledge showed a mean s

Perioperative management of surgical orthodontic treatment in a patient with glucose transporter 1 deficiency: report of a case and review of the literature.

INTRODUCTION: Glucose transporter 1 (GLUT1) deficiency is a rare cerebral metabolic disorder caused by the shortage of glucose supply to the brain. For this disease, ketogenic diet therapy is essential. In addition, perioperative management requires not only the continuation of ketogenic diet therapy but also the management of nausea/vomiting, diarrhea, seizures, and infection. However, there have been few reports regarding oral and maxillofacial surgery. CASE PRESENTATION: We describe a patient with GLUT1 deficiency who underwent orthognathic surgery. An 18-year-old man was referred to our h

Does Topical Proparacaine Improve Postoperative Comfort After Strabismus Surgery?

BACKGROUND: Strabismus surgery is often performed on children and adults as a quick-turnover, outpatient procedure under general anesthesia. Ideal methods to reduce post-operative pain and nausea are not yet perfected. We postulated that a simple topical anesthetic drop after surgery might help. METHODS: In a prospective study of oculocardiac reflex (OCR) and strabismus surgery, a cohort of ongoing patients either received proparacaine immediately post-op, or none. Co-variables were Intraoperative opioid and OCR, patient age, type of surgery. Several post-operative recovery outcome variables
