A realist evaluation exploring simulated patient role-play in pharmacist undergraduate communication training.

BACKGROUND: Effective communication between pharmacists and patients is essential and improves health outcomes. Simulated patients (SPs) are trained to reproduce real-life situations and can help pharmacy students to develop and adapt their communication skills in a safe, learner-centred environment. The aim of this research was to explore how SP and pharmacy student role-play supports communication training. METHODS: A mixed methods realist evaluation approach was adopted to test an initial theory relating to SP role-play for pharmacy students. The intervention tested involved complex commun

Patient pathways and short-term outcomes following nurse-led first-contact unscheduled visits in primary care: A multicentre cohort study.

BACKGROUND: Unscheduled primary care demand is placing increasing pressure on primary health care systems, prompting policies that position nurses on the front line. Nurse-led care protocol-guides are now widely used. However, patient pathways from the reason for unscheduled daytime visits to the end of the care process remain poorly understood. OBJECTIVE: This study aimed to quantify the volume and clinical profile of unscheduled walk-in daytime visits in primary care, characterize the full nurse-led care pathway within a collaborative model, and evaluate the determinants of scheduled follow

April 15th war and hemodialysis patients in Sudan: a cross-sectional study.

BACKGROUND: On April 15th, 2023, intense clashes involving heavy weapons and airstrikes occurred between the Sudanese Armed Forces (SAF) and the paramilitary Rapid Support Forces (RSF) in Khartoum, the capital of Sudan, leading to the displacement of almost 8.1 million people. The ongoing armed conflict in Sudan has led to a worsening humanitarian catastrophe, posing serious challenges to the country's health-care system and even its collapse. Healthcare facilities have been severely affected, with barely one-third of hospitals operating in conflict zones. Hemodialysis patients are among the m

Association Between a Co-Designed Dashboard and Use of Costly Health Services in Patients With Chronic Kidney Disease and Advanced Cancer: Propensity Score-Adjusted Difference-in-Differences Study.

BACKGROUND: The US health care system faces escalating costs, increasing emphasis on patient autonomy, and a regulatory shift toward patient-centered care and patient-reported outcomes (PROs). Leveraging PROs to support shared decision-making has the potential to improve outcomes and reduce health care use for patients with advanced chronic conditions. OBJECTIVE: This study aims to evaluate the impact of a PRO-based clinical dashboard on the use of costly health services among patients with advanced cancer and chronic kidney disease (CKD). METHODS: We conducted a quasi-experimental, propensi

Youth co-production for sustainable engagement and empowerment in health (YiPEE): protocol for a cluster-randomised controlled trial of an intervention for mental health promotion among adolescents in schools in Chennai, India.

INTRODUCTION: Adolescence is a time of rapid physical, social and psychological development and many risk factors for mental disorders have their roots in this age period. Primary prevention through school platforms has been extensively used and evaluated, but many interventions have poor uptake, high dropout and limited long-term sustainability. Mental health is a complex phenomenon and may be best supported through multicomponent interventions that more holistically consider inner, social and environmental levels. Character education-focused interventions are effective for improving adolesce

Health Outcomes Associated with Initiation of Basal Insulin After 1, 2, or ≥ 3 Oral Antidiabetes Drug(s) Among Managed Care Patients with Type 2 Diabetes.

BACKGROUND: Type 2 diabetes mellitus (T2DM) is a progressive disease. Despite starting with single oral antidiabetes drug (OAD) therapy and then adding OAD(s), most patients eventually require insulin therapy to achieve and maintain glycemic control. The timely initiation of insulin therapy could help patients with T2DM whose glycemic control is not adequately maintained using OADs alone. OBJECTIVE: To describe and compare baseline characteristics and assess real-world health outcomes associated with initiating basal insulin after 1 OAD, 2 OADs, or ≥ 3 OADs among T2DM patients. METHODS: Data

Parental and neonatal risk factors for cryptorchidism.

BACKGROUND: Breast cancer (BC) is the most common cancer diagnosed in women globally and the leading cause of cancer-related deaths among younger women diagnosed between the ages of 20-49 years in the United States (US). Although the median age of BC diagnosis is 62 years overall, recent data show that early-onset BC is on the rise in young people. Black and Hispanic women are disproportionately affected by BC, often diagnosed at a younger age, and BC is the leading cause of cancer-related deaths for both groups of women. The aim of this report is to present the unique case of a young adult Hi

Cost-consequence analysis of a criterion-based screening and triage pathway compared with full ADHD assessment for adults referred to NHS secondary care.

BACKGROUND: National evidence indicates that attention-deficit hyperactivity disorder (ADHD) referrals, recorded diagnoses and waiting lists have increased substantially. The Comprehensive ADHD Screening Questionnaire (CASQ), a criterion-based tool delivered by trained Band 7 clinicians, achieved 100% sensitivity, with a 95% confidence interval of 61.0-100.0%, while screening out 39.6% of referrals in a prospective NHS validation study of 48 participants. This analysis compares the costs and diagnostic consequences of the CASQ-first pathway with full ADHD assessment from the NHS perspective.

Artificial intelligence, explainable deep learning, and generative models for osteoporosis and bone disease prediction: a systematic review and future research roadmap.

Osteoporosis is a major global health burden, with considerable morbidity, mortality, and healthcare costs. Artificial intelligence (AI) has opened new avenues for osteoporosis screening, bone mineral density quantification, fracture-risk prediction, and clinical decision support, but progress remains fragmented across machine learning, deep learning, explainable AI (XAI), generative models, and multimodal learning. This systematic review brings these strands together and identifies priority research needs for AI-based osteoporosis prediction. Following the PRISMA 2020 statement, studies were

Societal actors' perceptions of intersectoral collaboration and the participation of older adults in the development of age-friendly environments.

BACKGROUND: Older adults' participation and intersectoral collaboration are central to the development of age-friendly environments. While previous research has emphasised the importance of these principles, less is known about how they are understood and enacted by societal actors in practice. This creates a need to explore societal actors' perceptions of both intersectoral collaboration and older adults' participation in the development of age-friendly environments. METHOD: The study had a participatory qualitative design. Data were collected through a World Café involving 19 societal actor
