Chronic myocardial infarction promotes atrial action potential alternans, afterdepolarizations, and fibrillation.

AIMS: Atrial fibrillation (AF) is increased in patients with heart failure resulting from myocardial infarction (MI). We aimed to determine the effects of chronic ventricular MI in rabbits on the susceptibility to AF, and underlying atrial electrophysiological and Ca(2+)-handling mechanisms. METHODS AND RESULTS: In Langendorff-perfused rabbit hearts, under β-adrenergic stimulation with isoproterenol (ISO; 1 µM), 8 weeks MI decreased AF threshold, indicating increased AF susceptibility. This was associated with increased atrial action potential duration (APD)-alternans at 90% repolarization, b

Rationale and design of the PRAETORIAN trial: a Prospective, RAndomizEd comparison of subcuTaneOus and tRansvenous ImplANtable cardioverter-defibrillator therapy.

BACKGROUND: Implantable cardioverter-defibrillators (ICDs) are widely used to prevent fatal outcomes associated with life-threatening arrhythmic episodes in a variety of cardiac diseases. These ICDs rely on transvenous leads for cardiac sensing and defibrillation. A new entirely subcutaneous ICD overcomes problems associated with transvenous leads. However, the role of the subcutaneous ICD as an adjunctive or primary therapy in patients at risk for sudden cardiac death is unclear. STUDY DESIGN: The PRAETORIAN trial is an investigator-initiated, randomized, controlled, multicenter, prospective

Acute and long-term outcome of transvenous cryothermal catheter ablation of supraventricular arrhythmias involving the perinodal region.

OBJECTIVE: Cryoenergy is a new valuable treatment option to perform ablation close to the atrioventricular (AV) node in the cure of supraventricular tachycardias because of its favourable properties, such as the possibility of creating reversible lesions. The aim of this study was to report our experience on the effectiveness and safety of catheter cryoablation performed in "critical areas" to treat a large cohort of patients with supraventricular arrhythmias. METHODS: One hundred and thirty-one patients suffering from supraventricular tachycardias underwent catheter cryoablation using a 7F c

Evidence for longitudinal and transverse fiber conduction in human pulmonary veins: relevance for catheter ablation.

BACKGROUND: Segmental ostial ablation of the pulmonary veins (PVs) allows for successful control of paroxysmal atrial fibrillation in many patients. We hypothesized that mapping of the left atrial-PV junction with a 64-electrode basket catheter would allow characterization of conduction patterns that would identify sites where ablation is required to electrically isolate the PV. METHODS AND RESULTS: A 64-electrode basket catheter was used to map the PVs of 50 patients undergoing PV isolation procedures for the treatment of atrial fibrillation. Activation along each spline was classified as re

Catecholamine-induced T-wave lability in congenital long QT syndrome: a novel phenomenon associated with syncope and cardiac arrest.

OBJECTIVE: To determine the effects of phenylephrine and dobutamine on repolarization lability in patients with genotyped long QT syndrome (LQTS). PATIENTS AND METHODS: Between December 1998 and August 2000, 23 patients with genotyped LQTS (13 LQT1, 7 LQT2, and 3 LQT3) and 16 controls underwent electrocardiographic stress testing at the Mayo Clinic in Rochester, Minn. Aperiodic repolarization lability was quantified from digitized electrocardiograms recorded during catecholamine stress testing with phenylephrine and dobutamine. T-wave lability was quantified as a root-mean-square of the diffe

Evolution of action potential propagation and repolarization in cultured neonatal rat ventricular myocytes.

INTRODUCTION: Cultured neonatal rat ventricular myocytes (NRVM) reestablish gap junctions as they form synchronously and spontaneously beating monolayers, thus providing a useful model for studying activation and repolarization. METHODS AND RESULTS: We used the multielectrode array data acquisition system with 60 unipolar electrodes to investigate the functional organization of cultured NRVM, by determining propagation and repolarization patterns. Activation maps were constructed from the local activation times at each electrode. During days 3 to 8 in culture, QRS amplitude and dV/dt(max) inc

Asymptomatic Intracardiac Cement Embolism Following Kyphoplasty.

Cement extravasation can occur during vertebral body augmentation such as kyphoplasty and vertebroplasty with diverse presentation and resultant treatment. The cement can embolize through venous vasculature to the thorax where it poses a potential threat to the cardiovascular and pulmonary systems. A thorough risk-benefit analysis should be conducted to select the appropriate treatment course. We present an asymptomatic case of cement extravasation to the heart and lungs during kyphoplasty.

Cardiovascular complications of systemic sclerosis: What to look for.

Systemic sclerosis, an autoimmune disease characterized by fibrosis of the skin and various internal organs, is associated with cardiovascular abnormalities including pulmonary hypertension, atherosclerosis, right and left ventricular dysfunction, arrhythmias, conduction defects, pericardial disease, and valvular heart disease. Clinicians caring for patients with this disease should regularly screen for cardiac symptoms, and patients with abnormal findings should be managed in conjunction with a cardiologist to optimally modify cardiovascular risks.

Cost-effectiveness of ace inhibitors versus ARBs in heart failure management.

BACKGROUND: Heart failure is a chronic condition that imposes a significant burden on healthcare systems worldwide. Effective management is crucial for improving patient outcomes and reducing costs. Angiotensin-converting enzyme (ACE) inhibitors and angiotensin II receptor blockers (ARBs) are widely used to manage heart failure by reducing cardiac strain and preventing disease progression. Despite their common use, ACE inhibitors and ARBs differ in mechanisms, cost, and potential side effects. ACE inhibitors have long been the standard treatment, while ARBs are often prescribed to patients int

Standardization and clinical applications of retinal imaging biomarkers for cardiovascular disease: a Roadmap from an NHLBI workshop.

The accessibility of the retina with the use of non-invasive and relatively low-cost ophthalmic imaging techniques and analytics provides a unique opportunity to improve the detection, diagnosis and monitoring of systemic diseases. The National Heart, Lung, and Blood Institute conducted a workshop in October 2022 to examine this concept. On the basis of the discussions at that workshop, this Roadmap describes current knowledge gaps and new research opportunities to evaluate the relationships between the eye (in particular, retinal biomarkers) and the risk of cardiovascular diseases, including

Vagal tone regulates cardiac shunts during activity and at low temperatures in the South American rattlesnake, Crotalus durissus.

The undivided ventricle of non-crocodilian reptiles allows for intracardiac admixture of oxygen-poor and oxygen-rich blood returning via the atria from the systemic circuit and the lungs. The distribution of blood flow between the systemic and pulmonary circuits may vary, based on differences between systemic and pulmonary vascular conductances. The South American rattlesnake, Crotalus durissus, has a single pulmonary artery, innervated by the left vagus. Activity in this nerve controls pulmonary conductance so that left vagotomy abolishes this control. Experimental left vagotomy to abolish ca

Prognostic implications of intraventricular conduction disorders for the risk of sudden cardiac death in cardiac patients.

BACKGROUND: QRS duration is associated with sudden cardiac death (SCD). The prognostic implications of different intraventricular conduction disorders (IVCDs) are unclear. OBJECTIVE: We explored the risk of SCDs associated with prolonged QRS (110-119 ms), right bundle branch block, left bundle branch block (LBBB), and nonspecific IVCD (NIVCD) in cardiac patients. METHODS: We analyzed the data from 4 cohorts comprising data of 26,028 cardiac patients undergoing either coronary angiography or clinical exercise testing. The median follow-up time for SCD events was 8.4 years (5.5-11.6). SCD even

Extensive coronary venous system mapping for outflow tract ventricular arrhythmias: Utility of a dual over-the-wire microelectrode catheter technique.

BACKGROUND: During catheter ablation of epicardial or intramural outflow tract ventricular arrhythmia (OTVA), the presumed site of origin must often be inferred from coronary venous system (CVS) mapping, which is anatomically constrained. OBJECTIVE: This study sought to evaluate the feasibility and clinical utility of a dual over-the-wire (OTW) microelectrode catheter technique for extensive CVS mapping in OTVAs. METHODS: We prospectively enrolled 30 patients with OTVA exhibiting either a right bundle branch block or a left bundle branch block pattern with V2 or V3 transition. 2 2.7Fr OTW mi

Translational peak frequency mapping of human sinoatrial node-Bachmann bundle axis: Integrating human in vivo electroanatomic mapping with preclinical microstructural validation.

BACKGROUND: In vivo electroanatomic mapping has enabled functional characterization of the human sinoatrial node (SAN), yet precise localization of SAN exit zones (SAN-EZs) and preferential conduction pathways-particularly along the Bachmann bundle (BB)-remains challenging without direct anatomic validation. OBJECTIVE: This study aimed to establish a translational framework integrating human in vivo peak frequency (PF) mapping with preclinical anatomic and histologic validation of the SAN-BB conduction axis. METHODS: High-density endocardial electroanatomic mapping with PF analysis was perfo

Cusp overlap technique decreases paravalvular leakage in self-expandable transcatheter aortic valve replacement.

The cusp overlap technique allows greater visual separation between the basal annular plane and the conduction system and decreases the permanent pacemaker implantation rate. We assessed the impact of the cusp overlap technique on conduction disturbance and paravalvular leakage after transcatheter aortic valve replacement. A total of 97 patients underwent transfemoral transcatheter aortic valve replacement with self-expandable valves at our institution from November 2018 to January 2023. The mean age of the patients was 85 years, and 23% were male. The patients were divided into two groups: th
