[Prevalence and risk factors for postoperative infection in pediatric orthopedic surgery: a study of 458 children].

PURPOSE OF THE STUDY: The incidence of postoperative infection in pediatric surgery has been studied little in the literature. It would be lower than in adults. In the present work, we attempted to define the incidence of postoperative infection in orthopedic pediatric surgery and identify risk factors. MATERIAL AND METHODS: This was a retrospective analysis of 458 children who underwent surgery between 1998 and 1999 for the following conditions: talipes equinus, congenital hip dislocation, supracondylar fracture of the elbow, and femur shaft fracture. We noted the prevalence of infection of

Functional limitations and poor physical performance as independent risk factors for self-reported fractures in older persons.

OBJECTIVE: This study examined whether three aspects of functioning (i.e., functional limitations, physical performance, and physical activity) were associated with fractures in older men and women. DESIGN: A 3-year prospective cohort study. PARTICIPANTS AND SETTING: A total of 715 men and 762 women, aged 65 years and older, of the population-based Longitudinal Aging Study Amsterdam. MEASUREMENTS: During an interview at home, three aspects of functioning were assessed: functional limitations (what people say they can do), physical performance, i.e., three performance tests and handgrip stre

Remodelling of sensorimotor maps in paraplegia: a functional magnetic resonance imaging study after a surgical nerve transfer.

The adult mammalian brain has the capacity of reorganising its neural connections in response to lesions/modifications of the peripheral and central nervous system. We show in vivo, using functional magnetic resonance imaging (fMRI), that in paraplegics the lower-limb sensorimotor cortex is invaded by the arm representation. This functional reshaping appears to be reversible. Indeed, surgical transfer of the ulnar nerve to the ipsilateral quadriceps and hip muscles allowed their contraction in a paraplegic patient. During fMRI, these voluntary movements activated the hip and thigh representati

Influence of soft tissue body composition on bone mass and metabolism.

Total body bone mineral (TBBM), lean body mass (LBM), and fat mass (FM) were measured by dual photon 153Gd absorptiometry in four female groups: (a) 27 patients with hip fracture; (b) 40 patients with spinal crush fracture; (c) 17 patients with osteoarthrosis, and (d) 26 healthy postmenopausal women. TBBM was significantly decreased in the two osteoporotic groups. The FM tended to be smaller in the two osteoporotic groups and larger in the osteoarthrotic group compared to the control group, whereas, LBM was virtually equal in all four groups. A significant negative correlation (p less than 0.0

Closure of defects from pressure sores requiring proximal femoral resection.

Proximal femoral resection (i.e., modified Girdlestone procedure) is often required in the paraplegic with an infection or dislocation of the hip joint. A philosophy and technique for dealing with this difficult problem are presented. The technique involves using an external osseous fixator to stabilize the femur and allow healing of the flap used to fill the defect.

Variations in Knee Kinematics After ACL Injury and After Reconstruction Are Correlated With Bone Shape Differences.

BACKGROUND: The factors that contribute to the abnormal knee kinematics after anterior cruciate ligament (ACL) injury and ACL reconstruction remain unclear. Bone shape has been implicated in the development of hip and knee osteoarthritis, although there is little knowledge about the effects of bone shape on knee kinematics after ACL injury and after ACL reconstruction. QUESTIONS/QUESTIONS: (1) What is the relationship between bony morphology with alterations in knee kinematics after ACL injury? (2) Are baseline bone shape features related to abnormal knee kinematics at 12 months after ACL reco

The effect of pelvic movements of a gait training system for stroke patients: a single blind, randomized, parallel study.

BACKGROUND: Aging societies lead to higher demand for gait rehabilitation as age-related neurological disorders such as stroke and spinal cord injury increase. Since conventional methods for gait rehabilitation are physically and economically burdensome, robotic gait training systems have been studied and commercialized, many of which provided movements confined in the sagittal plane. For better outcomes of gait rehabilitation with more natural gait patterns, however, it is desirable to provide pelvic movements in the transverse plane. In this study, a robotic gait training system capable of p

What are the risk factors for dislocation in primary total hip arthroplasty? A multicenter case-control study of 128 unstable and 438 stable hips.

INTRODUCTION: Dislocation after total hip arthroplasty (THA) is a leading reason for surgical revision. The risk factors for dislocation are controversial, particularly those related to the patient and to the surgical procedure itself. The differences in opinion on the impact of these factors stem from the fact they are often evaluated using retrospective studies or in limited patient populations. This led us to carry out a prospective case-control study on a large population to determine: 1) the risk factors for dislocation after THA, 2) the features of these dislocations, and 3) the contribu

How central obesity influences intra-abdominal pressure: a prospective, observational study in cardiothoracic surgical patients.

BACKGROUND: Intra-abdominal hypertension (IAH) is frequently present in critically ill patients and is an independent predictor for mortality. Better recognition of clinically important thresholds is necessary. Increased intra-abdominal pressure (IAP) is associated with renal dysfunction, and renal failure is one of the most consistently described organ dysfunctions associated with IAH. Obesity is also associated with kidney injury. The underlying mechanisms are not yet fully understood. Increased IAP may be a link in this association. The aim of this study was firstly to find the range in val

Thromboprophylaxis for trauma patients.

BACKGROUND: Trauma is a leading causes of death and disability in young people. Venous thromboembolism (VTE) is a principal cause of death. Trauma patients are at high risk of deep vein thrombosis (DVT). The incidence varies according to the method used to measure the DVT and the location of the thrombosis. Due to prolonged rest and coagulation abnormalities, trauma patients are at increased risk of thrombus formation. Thromboprohylaxis, either mechanical or pharmacological, may decrease mortality and morbidity in trauma patients who survive beyond the first day in hospital, by decreasing the
