Nine-year cinacalcet monotherapy in a child with neonatal severe hyperparathyroidism caused by compound heterozygous CASR mutations.

OBJECTIVES: Neonatal severe primary hyperparathyroidism (NSHPT) is a rare, potentially life-threatening disorder caused by loss-of-function mutations in the CASR gene. Treatment traditionally includes hydration, diuretics, bisphosphonates, and parathyroidectomy. Long-term data on pharmacological therapy with cinacalcet are limited. CASE PRESENTATION: We report a female infant with NSHPT due to compound heterozygous CASR variants (c.893C>T, p.Ala298Val; c.2414A>C, p.Lys805Thr). Initial therapy with bisphosphonates provided transient calcium reduction. Cinacalcet monotherapy was started in the …

A novel optimized surgical strategy for secondary hyperparathyroidism: clean parathyroidectomy combined with autotransplantation and its clinical analysis.

BACKGROUND: Recurrent disease due to ectopic parathyroid tissue remains a significant challenge following conventional total parathyroidectomy with autotransplantation (TPTX+AT) for refractory secondary hyperparathyroidism (SHPT). This retrospective cohort study aimed to evaluate the safety and medium-term efficacy of a standardized, extended surgical approach-clean parathyroidectomy with autotransplantation (CPTX+AT)-which systematically resects the central neck compartment to address this issue. We also clarified the exclusion criteria for kidney transplantation candidates and the applicatio…

Parathyroid gland volume and treatment resistance in patients with secondary hyperparathyroidism: a 4-year retrospective cohort study.

BACKGROUND: The role of parathyroid gland (PTG) ultrasonography in the management of secondary hyperparathyroidism after the introduction of calcimimetics remains unclear. Recent investigations have prompted renewed interest in the use of PTG ultrasonography for assessing treatment resistance to calcimimetics and determining the optimal timing for surgical intervention. This study aimed to explore the hypothesis that the PTG volume correlates with the calcimimetic dose. METHODS: We retrospectively observed outpatients undergoing haemodialysis at baseline and a 4-year follow-up. PTG volume was…

Use of four-dimensional computed tomography to aid parathyroid localisation in primary hyperparathyroidism in British surgical practice.

INTRODUCTION: Four-dimensional computed tomography (4DCT) has emerged as an effective imaging modality to aid parathyroid localisation before surgery. Following a service change in 2018, we evaluated the accuracy of 4DCT to lateralise culprit parathyroid glands causing primary hyperparathyroidism (PHPT) in patients undergoing parathyroid surgery in our centre. METHODS: A total of 117 patients underwent 4DCT before parathyroidectomy for PHPT in NHS Grampian between March 2018 and January 2023, of whom 112 underwent first-time operations. Results of imaging, histopathology and surgery type alon…

Recurrent pathological fractures in chronic kidney disease revealing overlapping neglected primary hyperparathyroidism and GDF5-associated skeletal dysplasia.

INTRODUCTION: Primary hyperparathyroidism (PHPT) is a common endocrine disorder characterized by the excessive secretion of parathyroid hormone (PTH), resulting in significant hypercalcemia and skeletal complications. In the context of chronic kidney disease (CKD), neglected PHPT can progress to a biochemical profile resembling tertiary hyperparathyroidism (THPT), further complicating diagnosis, especially when concomitant genetic skeletal disorders (GSD) exist. CASE REPORT: We present a rare and complex case of a 63-year-old woman with stage 3 CKD who presented with recurrent pathological fr…

Left Thoracoscopic Robot-Assisted Excision of a Left Lower Paratracheal Parathyroid Adenoma for Cure of Primary Hyperparathyroidism.

We present a case of a 38-year-old male patient with symptomatic hypercalcemia secondary to primary hyperparathyroidism. After evaluation, the source of the excess parathyroid hormone was found to be an adenoma localized to the middle mediastinum. Specifically, it was located in the left paratracheal space along the lesser curve of the aortic arch. We discuss this case with a corresponding video to demonstrate the necessary equipment and setup as well as the 5 operative steps recommended to access this paratracheal subaortic location from a minimally invasive transthoracic approach. The pitfal…

Intraoperative Parathyroid Hormone Monitoring-Guided Subtotal Parathyroidectomy is an Effective and Valid Surgical Procedure for Secondary Hyperparathyroidism.

OBJECTIVES: Secondary hyperparathyroidism (sHPT) is a prevalent complication of end stage renal disease in which serious morbid conditions and mortality can be encountered. Although the best solution of this severe problem is renal transplantation, because of the huge demand and limited resources, this cannot be possible most of the time. Initial treatment alternative is medical treatment in patients with sHPT and parathyroidectomy (PTX) should be applied if does not help. Subtotal PTX, total PTX and total PTX together with autotransplantation are the current surgical options preferred for sHP…
