New potential markers for cartilage degradation of the knee joint.

OBJECTIVE: To determine the correlation of biochemical markers with the degree of cartilage degradation. METHODS: In a cross-sectional study, synovial fluid samples were obtained from 65 patients with cartilage lesions of the knee joint. The measured biochemical markers included MMP-1, MMP-3, and MMP-13, the tissue inhibitor of MMPs (TIMP-1), COMP, YKL-40, and tenascin. The marker levels were compared with the Outerbridge and the Noyes classification. RESULTS: For the majority of markers, the correlation coefficient was below r = 0.3. The highest correlation coefficients were obtained from t

Ball-and-socket ankle joint with hypoplastic sustentaculum tali.

A case of unilateral ball-and-socket ankle joint associated with short limb and hypoplastic sustentaculum tali is reported. We think that hypoplastic sustentaculum tali was the cardinal feature in the anatomical and kinematic alterations of the foot in this patient. We review the embryonic development of hindfoot to ascertain the possible origin of this unusual finding.

Temporomandibular joint morphology in Cavalier King Charles Spaniels.

Temporomandibular joint dysplasia has been reported in several breeds of dog. Three Cavalier King Charles Spaniel (CKCS) skulls were examine … emporomandibular joint, although varying degrees of dysplasia were identified in all dogs where the joints were adequately visualized (n = 20). The head of four CKCS cadavers was also radiographed, and similar changes were found. This finding suggests that temporomandibular joint dysplasia is a widespread asymptomatic condition in the CKCS and should be …

Pseudogout attack of the lumbar facet joint: a case report.

STUDY DESIGN: A case of acute low back pain caused by pseudogout attack of the lumbar facet joint is reported. OBJECTIVE: To report a new cause of acute low back pain previously unreported in the literature. SUMMARY OF BACKGROUND DATA: There have been some reports of lumbar spinal stenosis caused by calcium pyrophosphate dihydrate crystal deposition. However, there are no known reports of pseudogout attack of the lumbar facet joint. METHODS: An axial magnetic resonance imaging scan demonstrated joint effusion at the level of the bilateral L4-L5 facet joint. Aspiration of the left L4-L5 face

Large geodes in rheumatoid arthritis without joint destruction.

Although subchondral geodes are a well-known radiological feature of rheumatoid arthritis (RA), large geodes are uncommon. Progressive bone damage with pathological fractures has been reported. We report the case of a 49-year-old man with seropositive RA in whom large, rapidly progressive geodes in the wrists, hands, and feet contrasted with the absence of joint destruction, good functional tolerance, and moderate abnormalities of markers for inflammation. The location and rapid progression of the cyst-like lesions in this patient were highly unusual.

Transient synovitis: is there a need to aspirate hip joint effusions?

There is debate as to the optimal management of children with hip joint effusions especially regarding the decision to aspirate. The objective of this study was to determine whether there is a need to aspirate hip joint effusions detected on ultrasound in patients with clinical transient synovitis (TS) and to identify the natural history of these effusions. Twenty-five children with proven hip joint effusions were followed up sequentially by clinical and radiological examination until symptom free. The mean age was 6 years with equal right/left distribution. No child underwent hip aspiration a

Borrelia burgdorferi and its tropisms for adhesion molecules in the joint.

Borrelia burgdorferi, the spirochete that causes Lyme disease, has evolved elegant strategies for interacting with its mammalian hosts. Among them are several distinct mechanisms of adhesion to cells and extracellular matrix components. The mammalian receptors for B. burgdorferi that have been most thoroughly studied, and for which candidate bacterial ligands have been identified, are decorin, fibronectin, glycosaminoglycans, and beta3-chain integrins. This diversity of adhesion mechanisms allows B. burgdorferi to infect multiple tissues, including the synovial tissues of the joints.

Spastic paretic stiff-legged gait: joint kinetics.

OBJECTIVE: The authors previously suggested that spastic paretic stiff-legged gait, defined as reduced knee flexion in swing associated with upper-motor neuron injury, can be attributed to multiple impairments besides spastic quadriceps activity. This study hypothesizes that subjects with spastic paretic stiff-legged gait have altered kinetics not only about the knee but also about the hip and ankle. DESIGN: Joint kinetic data of 20 subjects with spastic paretic stiff-legged gait caused by stroke were compared with data obtained from 20 able-bodied subjects. RESULTS: Significant reductions i

[The management of temporomandibular joint disorders. The role of occlusal splints].

Occlusal appliances are often used for the diagnosis and treatment of pain and dysfunction related to the mastication system, despite the lack of a precise diagnosis. We reviewed the indications for occlusal appliances, classing them by accepted procedures for precise diagnosis. The type of appliance to use for each pathological situation is described. Conditions other than those described here do not, in the author's opinion, constitute warranted indications for occlusal appliances.

Therapeutic infiltration of the facet joint in minor intervertebral disease.

A CT-guided infiltration technique of the facet joints was employed on a group of 20 patients suffering from sciatica without any signs ofmorphostructural alteration of the facet joints, secondary to Minor Intervertebral Disease (MID). The typical flouroscopically guided intra-articular facet arthrography has been substituted by a CT-guided technique that privileges therapy rather than diagnosis. The study involved 20 patients between 29 and 57 years of age (average 44.5), of both sexes (16 females and four males). The average time which elapsed from onset of symptomatology was 9.5 months. At …

Septic arthritis of the posterior lumbar joints is extremely rare in comparison with spondylodiscitis which is much more common. We report a case of an 86-year-old women with septic arthritis of the left L4-L5 lumbar facet joint associated with endocarditis. Arthritis diagnosis was made on CT scan and MRI, infection by Staphyloccocus aureus was proved by blood cultures. Heart growth was seen by echocardiography. Twenty-three cases were reported in the literature. Clinical and biological data failed to discriminate between facet joint septic arthritis and spondylodicitis. Diagnosis is establish…

The trapeziometacarpal joint. Tenotomy of the accessory tendons in early osteoarthritis.

The TMC joint is an articulation with special articular surfaces adapted to produce simple (nonrotatory) and complex (rotatory) metacarpal movements. Its articular anatomy and biomechanics are closely related to the pathogenesis of osteoarthritis. The joint works under high transarticular compressive-shearing forces. In osteoarthritic thumbs, the articular forces are increased because of the constant presence of accessory APL tendons, almost exclusively of the digastric type. Other factors should be considered in the pathogenesis of TMC joint osteoarthritis, such as repetitive use of the thumb

Defining the relationship between obesity and total joint arthroplasty.

OBJECTIVE: The purpose of this study was to examine the relationship between obesity and patient-administered outcome measures after total joint arthroplasty. RESEARCH METHODS AND PROCEDURES: A voluntary questionnaire-based registry contained 592 primary total hip arthroplasty patients and 1011 primary total knee arthroplasty patients with preoperative and 1-year data. Using logistic regression, the relationships between body mass index and the several outcome measures, including Short Form-36 and Western Ontario and McMaster Universities Osteoarthritis Index, were examined. RESULTS: There w

[Acute temporomandibular joint arthritis after Lyme borreliosis].

CASE REPORT: This report is about a rare connection between Lyme disease and an inflammation of the left temporomandibular joint. In this case, an infection was documented in 1998, 5 years after contact with Borrelia burgdorferi. The patient, a 49-year-old female, first came to our department in 1999. She showed the symptoms of a left temporomandibular joint infection. THERAPY: We suggested treatment with ceftriaxone 1 x 2 g/day i.v.

Biomechanics of the ankle joint. A perspective on total ankle replacement.

The biomechanics of the ankle present a unique set of challenges for arthroplasty surgery. Its biomechanics are not simple. Although the ankle joint may seem like a hinge, it is not in a line perpendicular to the tibia. The axis of rotation does not stay constant during range of motion, despite a relative congruency of this joint. Allowing for rotational forces must be accomplished, while maintaining the stability of the joint and its components. Success of the arthroplasty depends on how successful designs can dissipate these rotational forces, while maintaining the stability of the joint. It

Evaluation of explicit criteria for total hip joint replacement.

We tested an appropriateness of indications tool for total hip joint replacement in patients with osteoarthritis. Criteria were developed using a modified Delphi panel judgment process. Ratings were analyzed regarding level of agreement among panelists. Another panel rated the same indications; results were compared with the main panel. Test-retest of the main panel was performed. Regression models were used to assess the contribution of each algorithm variable. Appropriateness indication judgment was applied to 84 patients and compared to health-related quality-of-life improvement before and

Posterolateral dislocation of the elbow joint. Relationship to medial instability.

BACKGROUND: Dislocation of the elbow joint is the second most common dislocation in the upper extremity, dislocation of the shoulder being the most common. It has been reported that uncomplicated dislocation of the elbow joint may be associated with a decreased range of motion, degenerative changes in the elbow joint, ectopic calcification, or neurological deficits. As the medial collateral ligament complex can be completely disrupted during dislocation, we evaluated the association between the long-term results of treatment of simple posterolateral dislocation of the elbow and the presence of

Evaluation of fiscal and treatment outcomes in major joint replacement.

The market demand for major arthroplasty procedures is increasing. This descriptive comparative study was conducted to examine clinical and fiscal outcomes in total joint arthroplasty patients discharged either to home or to a subacute unit. The post-acute care setting was self-selected by patients after information was provided on both options. The Self-Administered Joint Rating Questionnaire served as the primary data collection tool. Age, health status, and living alone were significant factors in post-acute care site selection. Although there were no significant differences in clinical out
