Longitudinal study on the changes of maxillary arch dimensions in Japanese children with cleft lip and/or palate: infancy to 4 years of age.

The focus of this study was an analysis of maxillary arch growth changes prior to the time of cheiloplasty up to 4 years of age. Serial dental casts were obtained and measured in 95 children with cleft lip and palate, or both: 7 unilateral cleft lip and alveolus (CLA), 52 unilateral cleft lip and palate (UCLP), 24 bilateral cleft lip and palate (BCLP), and 12 isolated cleft palate (CP). The children were treated at the Kyushu University Dental Hospital. The results are as follows: (1) Prior to cheiloplasty, the maxillary buccal segments in the subjects with cleft lip and palate showed lateral …

Roentgenocephalometric study of complete unilateral cleft lip and palate patients treated with delayed closure of the hard palate--a preliminary report.

Cephalometric radiographs of a sample of complete unilateral cleft lip and palate patients (15 males, 15 females, aged 6.5 to 7.5 years) were analysed. In most cases, an adhesion cheiloplasty was done at 3 months of age, followed by definitive cheiloplasty at 6 to 12 months, then by posterior palatoplasty at 1.5 years. The hard palate was not repaired till the age of 5-7 years. This preliminary study indicated that: (1) The growth of maxilla and mandible shows no sex difference, except the anteroposterior maxillary length, which is longer in the cleft males. (2) The maxilla of our group is mor…

A comparative study of facial growth following cleft lip repair with or without soft-tissue undermining: an experimental study in rabbits.

The present study was performed in rabbits to test the hypothesis that incisions in the sulcus and soft-tissue undermining at the time of cleft lip repair contribute more to midfacial growth inhibition and aberrations than lip repair only. The design of this study, using three control groups, permitted assessment of the effects of soft-tissue undermining independently of the other surgical procedures. This study revealed that lip repair combined without soft-tissue undermining. Direct cephalometry on the skulls indicated that anteroposterior facial growth was most severely affected mean quanti…

Dermal sling as an alternative to palmaris longus tendon suspension in radial forearm flap lower lip reconstruction: A case report.

BACKGROUND: Lower lip reconstruction with the radial forearm free flap (RFFF) requires reliable static suspension to maintain oral competence. The palmaris longus tendon (PLT) is commonly used for this purpose, but its absence limits applicability. We describe a dermal sling harvested from the RFFF as an alternative suspension method. Case presentation After total lower lip resection for squamous cell carcinoma, reconstruction was performed using an RFFF. A full-thickness dermal strip was harvested from the proximal flap, deepithelialized, and used as a non-tunneled, onlay suspension sling anc…

Three-dimensional CT evaluation of anatomical constraints during posterior lip drilling in the retrosigmoid approach to the internal auditory canal.

PURPOSE: To develop a CT-based three-dimensional morphometric framework for evaluating the depth and orientation of posterior lip drilling during the retrosigmoid approach. METHODS: One hundred normal temporal bones from 50 adults were reconstructed from high-resolution CT. Five surgically relevant landmarks were identified at the endolymphatic sac-duct junction, posterior internal auditory canal (IAC) porus, common crus, lateral IAC fundus, and Bill's bar. Six linear distances and four angular relationships were measured to characterise the drilling corridor relative to the IAC, labyrinth, a…

Three-dimensional stereophotogrammetry vs. standard measurements for soft tissue and dental arches analysis in cleft lip and palate: Systematic review.

BACKGROUND: Cleft lip and palate (CLP) are the most prevalent congenital anomalies, often resulting in impaired maxillofacial complex in all three planes of space and maxillary growth deficiency, leading to aesthetic and functional challenges. Traditional methods like calipers and rulers have limitations in precision, reproducibility, and volumetric capture for soft tissues (ST) and dental arches (DA) in CLP patients. Three-Dimensional Stereophotogrammetry (3DS) offers a non-invasive digital alternative for accurate 3D imaging. The aim of this systematic review of comparative studies was to ev…

Early maxillary arch development in unilateral cleft lip and palate: a retrospective 3D morphometric analysis following nasoalveolar molding and cheiloplasty.

BACKGROUND: Early assessment of maxillary arch development is crucial in infants with unilateral cleft lip and palate (UCLP). This study aimed to evaluate three-dimensional changes in the maxillary arch following modified nasoalveolar molding (NAM) and cheiloplasty using digital model analysis. METHODS: This retrospective study included 25 infants with complete UCLP who underwent modified NAM therapy prior to cheiloplasty. Digital maxillary models were obtained at two time points: T1 on the day of cheiloplasty at 3–6 months of age and T2 on the day of palate repair at 9–18 months of age. Three…

Cleft Lip Repair: Trends of Nasoalveolar Molding, Concomitant Tip Rhinoplasty and/or Septoplasty, and Revisions Using a National Database Study.

BACKGROUND: The literature is limited regarding the prevalence of nasoalveolar molding (NAM), primary tip rhinoplasty and/or septoplasty, and revision surgery. Our study aims to evaluate national utilization rates of NAM, primary tip rhinoplasty or septoplasty, and lip revisions using an international database. METHODS: A retrospective review was performed on TriNetX, a de-identified claims database. Patients younger than 1 year with a primary unilateral or bilateral cleft lip repair from 2000 to 2023 were included. Procedures, including NAM, initial repair, primary tip rhinoplasty, and/or se…

Three Data-Driven Lip Soft-Tissue Phenotypes and Their Dento-Skeletal Associations in Orthodontic Patients: An Exploratory Unsupervised Learning Study.

BACKGROUND: Conventional facial diagnosis emphasizes skeletal and dental measures while lip features are assessed piecemeal. We aimed to identify data-driven lip soft-tissue phenotypes using unsupervised machine learning and examine their associations with skeletal and dental characteristics. METHODS: In a retrospective cross-sectional study, 10 lip measurements were extracted from standardized lateral cephalometric radiographs. Variables were z-score standardized and analysed using k-means clustering. Of 26 NbClust validation indices, 14 (53.8%) supported a three-cluster solution. Cluster st…

Three-Dimensional Palatal Morphology and Obstructive Sleep Apnea Severity in Children with Unilateral Cleft Lip and Palate: A CBCT Study.

Background/Objectives: Obstructive sleep apnea (OSA) is highly prevalent in children with unilateral cleft lip and palate (UCLP) due to maxillary retrusion and upper airway compromise. While palatal morphology may influence pediatric OSA, three-dimensional (3D) evaluations in this population remain limited. This study evaluated palatal dimensions and maxillary widths in UCLP children with and without OSA using cone-beam computed tomography (CBCT) and examined their associations with OSA severity (apnea-hypopnea index, AHI). Methods: Forty CBCT scans of Thai children with non-syndromic UCLP (me…

Long-term Follow-up of Unilateral Cleft Lip Repair Using the Trilobate Flap Combined with Nasolabial Muscle Tension System Reconstruction.

BACKGROUND: Unilateral cleft lip (UCL) repair often struggles to adequately lower the cleft-side philtral peak and achieve durable nasal correction. The trilobate flap technique redistributes tissue to the nasal tip, nasal base, and upper lip, while aligning the incision symmetrically with the contralateral philtral column. When paired with nasolabial muscle tension system reconstruction, it may better reproduce physiologic biomechanics and stability. METHODS: We retrospectively reviewed UCL patients repaired by one senior surgeon (2010-2019) with ≥5-year follow-up. Standardized frontal and s…
