Many cancers metastasize regionally through lymphatics before metastasizing systemically through blood vessels. However, metastasis through blood has been studied much more extensively than metastasis through lymph. Recently in Science, Lee et al. (2019) offered new insight into lymph node metastasis by showing that melanoma cells must undergo metabolic changes during this process and that it is driven by localized accumulation of bile acids.

Given the success of checkpoint inhibitors and the desire to test them in combination with other immunotherapies and targeted therapies, hundreds of clinical trials have been launched. To most efficiently study these agents, researchers and the FDA are exploring the use of novel endpoints, the use of new preclinical models, and adaptive trial designs. However, the cost and demands associated with the conduct of increasingly sophisticated early-phase clinical trials are putting smaller companies and some academic medical centers at a disadvantage.

Malignant cutaneous melanoma is the cancer which has the most increased these last ten years. His prognose is linked with melanoma thickness, tumoral ulceration and lymph node metastasis. The research of these node metastasis is important for the prognosis and the treatment. The sentinel lymph node biopsy is used since 1991 and is now considered as a reliable choice for the patient staging. However, his therapeutic interest remains discussed. We discussed the used of the sentinel lymph node biopsy for malignant cutaneous melanoma with the last publications.

The sentinel lymph node (SLN) procedure is used in our institute in the setting of an observational multicenter study investigating the reliability of the sentinel node procedure in vulvar carcinoma (GROINSS-V: The Groningen International Study on Sentinel Nodes in Vulvar Cancer). BACKGROUND: An initial presentation of inguinal metastasis in primary fallopian tube cancer in an asymptomatic patient is uncommon. All previously described palpable lymphadenopathies in fallopian tube cancer have been associated with widely disseminated intra-abdominal disease.
CASE: This is the first case of inguinal node metastasis from fallopian tube cancer in a 72-year-old asymptomatic patient with a previous hysterectomy with no evidence of intra-abdominal disease. The metastasis was detected in the contralateral inguinal area.
