Genetic and non-genetic determinants of clinical phenotypes in cardiomyopathy.

Cardiomyopathy, a leading cause of death worldwide, is etiologically and phenotypically heterogeneous and is caused by a combination of genetic and non-genetic factors. Major genomic determinants of dilated cardiomyopathy (DCM) are titin truncating mutations and lamin A/C mutations. Patients with these two genotypes show critically different phenotypes, including penetrance, coexistence with a conduction system abnormality, cardiac prognosis, and treatment response. The transcriptomic and epigenomic characteristics of DCM include activation of the DNA damage response, metabolic reprogramming, …

The association of lipoprotein(a) and apolipoprotein(a) phenotypes with peripheral artery disease.

AIM: Lipoprotein(a) [Lp(a)] is an independent risk factor of coronary heart disease (CHD) and myocardial infarction. Data about the role of Lp(a) in the development of peripheral artery disease (PAD) is controversial and uncertain. The aim of the study was to evaluate the association between Lp(a), apolipoprotein(a) [apo(a)] phenotypes and PAD. MATERIALS AND METHODS: The study included 998 patients (707 male and 291 female, average age 60±12). The patients were divided into 4 groups depending on the presence or absence PAD and CHD: group I (n=188, PAD+CHD+), group II (n=78, PAD+CHD-), group I…

Phenotypic and genetic characteristics of vancomycin-resistant Enterococcus faecium.

This study was based on 43 vancomycin-resistant Enterococcus faecium (VREfm) strains collected from clinical specimens. Susceptibility testing and resistance gene amplification revealed that these strains had multidrug resistance and all belonged to the VanA phenotype. Furthermore, there were seven ST types, and all belonged to the clonal complex (CC17); ST17 and ST78 were the main ST types. In particular, ST1392 and ST1394 are novel ST types first identified in this research. Genome analysis of SY1, LY19 and LY22 showed that tet(O)and tet(K) were the genes responsible for tetracycline resista…

A Comparative Analysis of Phenotypic Predictors of Mutations in Familial Hypercholesterolemia.

CONTEXT: The gold standard for diagnosing familial hypercholesterolemia (FH) is identification of a causative pathogenic mutation. However, genetic testing is expensive and not widely available. OBJECTIVE: To compare the validity of the Dutch Lipid Clinic Network (DLCN), Simon Broome (SB), Make Early Diagnosis to Prevent Early Deaths (MEDPED), and American Heart Association (AHA) criteria in predicting an FH-causing mutation. DESIGN, SETTING, AND PATIENTS: An adult cohort of unrelated patients referred to a lipid clinic for genetic testing. MAIN OUTCOME MEASURES: Odds ratio (OR), area under

Genetic basis and phenotypic features of congenital myasthenic syndromes.

The congenital myasthenic syndromes (CMS) are heterogeneous disorders in which the safety margin of neuromuscular transmission is compromised by one or more specific mechanisms. The disease proteins reside in the nerve terminal, the synaptic basal lamina, or in the postsynaptic region, or at multiple sites at the neuromuscular junction as well as in other tissues. Targeted mutation analysis by Sanger or exome sequencing has been facilitated by characteristic phenotypic features of some CMS. No fewer than 20 disease genes have been recognized to date. In one-half of the currently identified pro

An Acute Injury Model for the Phenotypic Characteristics of Geographic Atrophy.

PURPOSE: Geographic atrophy (GA) is the late stage of non-neovascular age-related macular degeneration. A lack of animal models for GA has hampered treatment efforts. Presented herein is a rat model for GA using subretinal injection of sodium iodate (NaIO3). METHODS: Rats were given subretinal injections of NaIO3 (5 μg/μL) using a pico-injector. Fundus photographs and spectral domain optical coherent tomography scans were collected at 1, 3, 7, 14, and 28 days after injection, at which time rats were euthanized and eyes were enucleated. Eyes were either cryopreserved or dissected into retinal

Sarcoidosis and IPF in the same patient-a coincidence, an association or a phenotype?

BACKGROUND: Idiopathic Pulmonary Fibrosis (IPF) and Sarcoidosis are distinct clinical entities. Fibrotic disease in pulmonary sarcoidosis is typically upper lobe predominant. In IPF fibrosis is basilar and peripheral predominant [usual interstitial pneumonia (UIP) pattern]. Sarcoidosis and UIP have rarely been observed in the same patient. We sought to characterize patients manifesting both sarcoidosis and IPF and compare clinical features and survival to that of patients with "Lone-IPF" (IPF only) and pulmonary sarcoidosis with fibrosis in a non-UIP pattern. METHODS: Patients were identified

Chronic Rhinosinusitis-Could Phenotyping or Endotyping Aid Therapy?

OBJECTIVES: We reviewed the phenotyping and endotyping of chronic rhinosinusitis (CRS) and treatment options. METHODS: We searched PubMed, Google, Google Scholar, and the Proquest Central Database of the Kırıkkale University Library. RESULTS: Phenotypes are observable properties of an organism produced by the environment acting upon the genotype, that is, patients with a particular disorder are subgrouped according to common characteristics. Currently, CRS is usually phenotyped as being with (CRSwNP) or without (CRSsNP) nasal polyps. However, this is not immutable as some individuals progres…

[The heritability of phenotypes related to primary angle closure glaucoma].

Glaucoma is the leading cause of irreversible blindness throughout the world. Primary angle closure glaucoma (PACG) is one of the common types of this disease. Research indicates that its prevalence is always associated with many factors, including ocular anatomical characteristics and the genetic susceptibility. According to many studies, phenotypes related to PACG, such as anterior chamber depth, relative lens position and thickness, chamber angle state, and axial length, are heritable. Heritability is an important indicator to quantify this genetic tendency. Therefore, the study of heritabi…

Functional Dysregulation of CDC42 Causes Diverse Developmental Phenotypes.

Exome sequencing has markedly enhanced the discovery of genes implicated in Mendelian disorders, particularly for individuals in whom a known clinical entity could not be assigned. This has led to the recognition that phenotypic heterogeneity resulting from allelic mutations occurs more commonly than previously appreciated. Here, we report that missense variants in CDC42, a gene encoding a small GTPase functioning as an intracellular signaling node, underlie a clinically heterogeneous group of phenotypes characterized by variable growth dysregulation, facial dysmorphism, and neurodevelopmental…

Species-conserved SYNGAP1 phenotypes associated with neurodevelopmental disorders.

SYNGAP1 loss-of-function variants are causally associated with intellectual disability, severe epilepsy, autism spectrum disorder and schizo … ons of Syngap1 and consider how these functions may contribute to the emergence of disease-relevant phenotypes. Finally, we identify major unexplored areas of Syngap1 neurobiology and discuss how a deeper understanding of this gene may uncover general principles of NDD pathobiology.

Trajectories of motor abnormalities in milder phenotypes of ataxia telangiectasia.

OBJECTIVE: To describe and classify the neurologic trajectories in patients with mild neurologic forms of ataxia telangiectasia (A-T) from the Dutch A-T cohort, combined with patients reported in the literature. METHODS: Clinical, genetic, and laboratory data of 14 patients with mild neurologic phenotypes of A-T from the Dutch cohort were analyzed and combined with corresponding data from the literature. A mild neurologic phenotype was defined by a later onset, nonataxia presenting or dominant feature, or slower progression compared to the classic A-T phenotype. Neurologic trajectories were c…

Phenotypic and molecular insights into PQBP1-related intellectual disability.

We report two discordant clinical and imaging features in four male patients from two unrelated families of Egyptian descent with hemizygous … tem hypoplasia. These imaging features are distinct from the previously described with a well-known phenotype that is already known for PQBP1. This report expands the phenotypic spectrum of PQBP1-related disorders and is the second reported missense PQBP1 variant. Further, it highlights the possible role of PQBP1 in hindbrain development.

The remarkable phenotypic variability of the p.Arg269HiS variant in the TRPV4 gene.

INTRODUCTION: Mutations in the TRPV4 gene are associated with neuromuscular disorders and skeletal dysplasias, which present a phenotypic overlap. METHODS: Next-generation sequencing and Sanger sequencing were used to analyze the TRPV4 gene. RESULTS: We present 2 Polish families with TRPV4-related disorder harboring the same p.Arg269His mutation. The disease phenotypic expression was extremely variable (from mild scapular winging to severe hypotonia, global weakness, inability to walk unaided, congenital contractures, scoliosis, and respiratory insufficiency), but did not suggest anticipatio

Enhancer redundancy provides phenotypic robustness in mammalian development.

Distant-acting tissue-specific enhancers, which regulate gene expression, vastly outnumber protein-coding genes in mammalian genomes, but th … ogy. By contrast, the removal of pairs of limb enhancers near the same gene resulted in discernible phenotypes, indicating that enhancers function redundantly in establishing normal morphology. In a genetic background sensitized by reduced baseline expression of the target gene, even single enhancer deletions caused limb abnormalities, suggesting that funct …
