Prophylactic Midurethral Slings at the Time of Pelvic Organ Prolapse Repair.

IMPORTANCE: Prophylactic midurethral sling (MUS) placement can prevent de novo stress urinary incontinence (SUI) after pelvic organ prolapse (POP) repair, but the concern about additional MUS-related adverse events remains. OBJECTIVE: The objective of this study was to compare postoperative outcomes between women with and without preoperative SUI after undergoing concomitant POP repair and MUS placement. STUDY DESIGN: This retrospective cohort study included women undergoing POP repair with concurrent MUS placement between 2017 and 2023. Patients in the prophylactic group did not have preope

The Surgeon, the Robot, and the Stapler: Comparison of Stapling Strategies During Robotic Lobectomy.

OBJECTIVE: There are 2 methods for surgical stapling during robotic lobectomy (RL): handheld assistant-driven (HH) and robotic (RS) staplers. This study aims to compare the clinical and financial outcomes between these HH and RS approaches used in RL for lung cancer. METHODS: Between January 1, 2021, and December 31, 2022, patients who underwent elective RL for lung cancer were identified and extracted from the PINC AI™ Healthcare Database. To compare RS and HH outcomes, 1:1 nearest-neighbor propensity score matching (PSM) was conducted using patient and provider characteristics. Chi-square,

Polymers for Pelvic Floor Repair: Current State and Future Prospects.

IMPORTANCE: Surgical repair of pelvic organ prolapse and stress urinary incontinence frequently relies on polymeric biomaterials in sutures, meshes, and membranes to provide structural support during tissue healing and beyond. Polypropylene has long served as the default material owing to its strength, durability, and processability, but the chronic foreign body response and mechanical mismatch between stiff implants and viscoelastic pelvic tissue have been linked to complications, including mesh contraction, exposure, and dyspareunia. These limitations, together with regulatory actions on tra

Comparative Outcomes of Left Anterior Thoracotomy Versus Resternotomy for Pulmonic Valve Replacement After Tetralogy of Fallot Repair.

OBJECTIVE: Redo pulmonary valve replacement (PVR) in patients with prior tetralogy of Fallot (TOF) repair is surgically challenging. This study aimed to compare the clinical outcomes of the left anterior minithoracotomy (LAT) approach with the traditional resternotomy approach for PVR. METHODS: A retrospective analysis was conducted on 38 TOF patients who underwent redo PVR, categorized into LAT (n = 20) and resternotomy (n = 18) groups. Patients requiring concomitant valvular surgeries or repair of residual intracardiac shunts were excluded. We compared demographics, operative details, and p

Chronic Anticoagulation Increases Complications in Obliterative Prolapse Surgery.

IMPORTANCE: Chronic anticoagulation has been identified as a risk factor for postoperative complications in other specialties, but there is a paucity of data on its effect on older women undergoing obliterative prolapse repair. OBJECTIVE: The objective of this study was to assess whether chronic anticoagulation is associated with increased serious adverse events (SAEs) after obliterative prolapse surgery. STUDY DESIGN: This was a retrospective cohort analysis of women 65 years or older undergoing obliterative prolapse repair at 2 academic institutions from 2016 to 2023. Patients were identif

Long-Term Foveal Microvascular Alterations in Children With Hypoxic-Ischemic Encephalopathy: An OCT Angiography Study.

PURPOSE: To investigate the foveal microvascular structure in children with a history of hypoxic ischemic encephalopathy (HIE) compared with healthy controls. METHODS: This prospective cross-sectional comparative study included 46 eyes of 23 children with HIE and 40 eyes of 20 age-matched healthy controls. The children were grouped as follows: Group 1: HIE Stage I (n=28 eyes); Group 2: HIE Stage II-III (n=18 eyes); Group 3: healthy age-matched controls (n=40 eyes). Patients were also subgrouped based on fundus appearance within the first week of life as follows: Group A: eyes without any reti

Serum Ang-1, sFLT-1, and sST2 Profiles Distinguish Anti-VEGF Responders in Diabetic Macular Edema.

PURPOSE: Diabetic macular edema (DME) remains a significant cause of vision loss despite the widespread use of anti-VEGF therapy. Nearly half of patients experience suboptimal responses in both anatomical and functional outcomes. This study correlates molecular signatures with clinical outcomes to identify associative markers of early treatment response and optimize DME management. METHODS: Serum samples from 44 DME patients, treated with ≥3 anti-VEGF injections and 6 months follow-up, classified as responders or non-responders. Multiplex bead-based assays were used to measure cytokines relat

CLINICAL AND DEMOGRAPHIC RISK FACTORS ASSOCIATED WITH RECURRENT AND FELLOW EYE RHEGMATOGENOUS RETINAL DETACHMENTS.

PURPOSE: To identify clinical and surgical risk factors associated with recurrent rhegmatogenous retinal detachment (RRD) in the index eye and RRD in the fellow eye. METHODS: Retrospective observational study of adult patients who underwent RRD repair at two tertiary care centers over six years. RESULTS: Among 794 patients with primary RRD, recurrence occurred in 20.5%, and RRD in the fellow eye was observed in 5.2%. Postoperative visual acuity significantly improved at one year (95% CI -0.44 to 0.30; P < 0.001). Higher recurrence rates were found in patients with prolonged symptom duration
