Neural response to self- and other referential praise and criticism in generalized social phobia.

CONTEXT: Generalized social phobia (GSP) is characterized by fear/avoidance of social situations. Previous studies have examined the neural responses in GSP to one class of social stimuli, facial expressions. However, studies have not examined the neural response in GSP to another equally important class of social stimuli, the communication of praise or criticism. OBJECTIVE: To examine the neural response to receipt of praise or criticism in GSP; specifically, to determine whether patients with GSP show an increased response to the receipt of both praise and criticism and whether self-relevan

Specificity of disgust sensitivity in the prediction of fear and disgust responding to a brief spider exposure.

This study examined the specificity of disgust sensitivity in predicting fear and disgust responses to exposure to a spider. Participants high (n=22) and low (n=28) in spider fear completed self-report measures of disgust sensitivity, contamination fear, anxiety, and negative affect. They then participated in a behavioral avoidance task (BAT) in which they were briefly exposed to a realistic-looking, but fake, tarantula. Results revealed that disgust sensitivity was associated with fear and disgust responding to the BAT. The association between disgust sensitivity and disgust responding to the

Social anxiety predicts unconsciously provoked emotional responses to facial expression.

Previous psychometric studies using a visual search task suggested that interpersonal fear in individuals with social anxiety disorder (SAD) may be processed by unconscious preattentive mechanisms. However, little is known about relationships between social anxiety and preattentive emotional responses. We explored whether social anxiety is associated with preattentive emotional responses to facial expression. Groups with high and low social anxiety were selected from 125 healthy volunteers according to scores on the Social Phobia Inventory. Fearful and happy faces were presented subliminally u

Low-dose intranasal versus oral midazolam for routine body MRI of claustrophobic patients.

The purpose of this study was to assess prospectively the potential of low-dose intranasal midazolam compared to oral midazolam in claustrophobic patients undergoing routine body magnetic resonance imaging (MRI). Seventy-two adult claustrophobic patients referred for body MRI were randomly assigned to one of two treatment groups (TG1 and TG2). The 36 patients of TG1 received 7.5 mg midazolam orally 15 min before MRI, whereas the 36 patients of TG2 received one (or, if necessary, two) pumps of a midazolam nasal spray into each nostril immediately prior to MRI (in total, 1 or 2 mg). Patients' to

Fear of movement/injury in the general population: factor structure and psychometric properties of an adapted version of the Tampa Scale for Kinesiophobia.

In recent years, several studies have pointed out the importance of pain-related fear in the development and maintenance of chronic pain. An important instrument for measuring pain-related fear in the context of low back pain is the Tampa Scale for Kinesiophobia (TSK). Recently, a version of this questionnaire has been developed for administration among the general population (TSK-G). To determine the factor structure of the TSK-G, data from a random sample of the Dutch general population were studied separately for people who had had back complaints in the previous year, and people who had be

The Tampa Scale for Kinesiophobia: further examination of psychometric properties in patients with chronic low back pain and fibromyalgia.

The present study attempted to replicate the robustness of a two-factor model of the Tampa Scale for Kinesiophobia (TSK) in chronic low back pain (CLBP) patients and fibromyalgia patients, by means of confirmatory factor analysis. Construct and predictive validity of the TSK subscales were also examined. Results clearly indicated that a two-factor model fitted best in both pain samples. These two factors were labelled somatic focus, which reflects the belief in underlying and serious medical problems, and activity avoidance, which reflects the belief that activity may result in (re)injury or i

Some biochemical correlates of panic attacks with agoraphobia and their response to a new treatment.

Thirty-two patients with chronic debilitating agoraphobia and panic attacks participated in a comparative study of the triazolobenzodiazepine alprazolam and the anti-inflammatory agent ibuprofen. After a 2-week placebo washout period, patients were randomly assigned to 8 weeks of treatment with alprazolam (2 to 6 mg/day) or ibuprofen (0.8 to 2.4 g/day). Medication was identically packaged and patients were blind to the treatment condition, but investigators were aware of which medication was dispensed. Alprazolam recipients (mean daily dose: 5.4 mg) improved markedly with respect to physician

Kinesiophobia is not required to predict chronic low back pain in workers: a decision curve analysis.

BACKGROUND: Currently used performance measures for discrimination were not informative to determine the clinical benefit of predictor variables. The purpose was to evaluate if a former relevant predictor, kinesiophobia, remained clinically relevant to predict chronic occupational low back pain (LBP) in the light of a novel discriminative performance measure, Decision Curve Analysis (DCA), using the Net Benefit (NB). METHODS: Prospective cohort data (n = 170) of two merged randomized trials with workers with LBP on sickleave, treated with Usual Care (UC) were used for the analyses. An existin

Less is more: Neural activity during very brief and clearly visible exposure to phobic stimuli.

Research on automatic processes in fear has emphasized the provocation of fear responses rather than their attenuation. We have previously shown that the repeated presentation of feared images without conscious awareness via backward masking reduces avoidance of a live tarantula in spider-phobic participants. Herein we investigated the neural basis for these adaptive effects of masked exposure. 21 spider-phobic and 21 control participants, identified by a psychiatric interview, fear questionnaire, and approaching a live tarantula, viewed stimuli in each of three conditions: (1) very brief expo

Infralimbic Neurotrophin-3 Infusion Rescues Fear Extinction Impairment in a Mouse Model of Pathological Fear.

The inability to properly extinguish fear memories constitutes the foundation of several anxiety disorders, including panic disorder. Recent findings show that boosting prefrontal cortex synaptic plasticity potentiates fear extinction, suggesting that therapies that augment synaptic plasticity could prove useful in rescue of fear extinction impairments in this group of disorders. Previously, we reported that mice with selective deregulation of neurotrophic tyrosine kinase receptor, type 3 expression (TgNTRK3) exhibit increased fear memories accompanied by impaired extinction, congruent with an

Impact of family and school environment on the development of social anxiety disorder: a questionnaire study.

BACKGROUND: Social anxiety disorder (SAD) is a very common condition, although its prevalence is believed to be underestimated. The affected subjects often have trouble to search for support. The onset occurs mainly in early adolescence. The aim of this paper was to evaluate the impact of school and family background on the development of SAD. SUBJECTS AND METHODS: Our survey, available on a popular social network site, was divided into 4 parts: 1) demographic data (gender, age, site of residence), 2) genetic and organic background (comorbid mental disorders, addictions), 3) situation at scho

Internal and external attention in speech anxiety.

BACKGROUND AND OBJECTIVES: Cognitive models of social phobia propose that socially anxious individuals engage in heightened self-focused attention. Evidence for this assumption was provided by dot probe and feedback tasks measuring attention and reactions to internal cues. However, it is unclear whether similar patterns of attentional processing can be revealed while participants actually engage in a social situation. The current study used a novel paradigm, simultaneously measuring attention to internal and external stimuli in anticipation of and during a speech task. METHODS: Participants w

Altered brain functional connectivity in relation to perception of scrutiny in social anxiety disorder.

Although the fear of being scrutinized by others in a social context is a key symptom in social anxiety disorder (SAD), the neural processes underlying the perception of scrutiny have not previously been studied by functional magnetic resonance imaging (fMRI). We used fMRI to map brain activation during a perception-of-scrutiny task in 20 SAD patients and 20 controls. A multi-dimensional analytic approach was used. Scrutiny perception was mediated by activation of the medial frontal cortex, insula-operculum region and cerebellum, and the additional recruitment of visual areas and the thalamus

Social anxiety modulates amygdala activation during social conditioning.

Aversive social learning experiences might play a significant role in the aetiology of social anxiety disorder. Therefore, we investigated emotional learning and unlearning processes in healthy humans using a social conditioning paradigm. Forty-nine healthy subjects participated in a 2-day fMRI differential conditioning protocol. Acquisition and extinction were conducted on Day 1 and extinction recall on Day 2. BOLD responses, ratings and skin conductance responses were collected. Our data indicate successful conditioning and extinction on the neural and subjective level. As a main result, we
