Arteriovenous malformation of the forehead, anterior scalp, and nasal dorsum.

Management of complex and relentless large arteriovenous malformations with long term control and acceptable aesthetic results can be accomplished. This outcome requires selective intra-arterial embolization, judicious surgical resection, composite reconstruction with free tissue transfer, other ancillary procedures, or both, and careful serial follow-up examinations to rule out recurrent or persistent disease.

Mitral valve plasty using artificial chordae in a 1.5-year-old boy with congenital mitral stenosis and absent anterolateral chordae.

A 1.5-year-old boy with refractory heart failure due to congenital mitral stenosis exhibited fusion of the anterolateral leaflets associated with moderate mitral regurgitation due to absent anterolateral chordae. Fused leaflets were incised and the mitral valve opening improved. Mitral regurgitation increased due to the absence of chordae, so 2 pairs of artificial chordae of 5-0 expanded polytetrafluoroethylene suture were anchored to the hypoplastic anterolateral papillary muscle, then attached to the free margin of incised leaflets. Postoperative examination showed improved mitral opening wi

[Short- and long-term outcome after urethroplasty in the patients with hypospadias treated by Hodgson III method].

Between 1977 and 1997, 29 patients with hypospadias were surgically treated by Hodgson type III urethroplasty at Tsukuba University Hospital. The duration of surgery ranged from 150 to 535 minutes with an average of 269.3 minutes and the urethral catheter was removed between 4 and 14 days (average 8.7 day) after surgery. The postoperative hospital stay ranged from 10 to 29 days with an average of 18.4 days. The overall success rate of initial surgery was observed in 18 of the 29 patients (62.1%). As early postoperative complications, urethral fistula and stricture were seen in 10 (34%) and 3 (

Lower eyelid and midface rejuvenation.

Lower eyelid blepharoplasty is one of the most common procedures in aesthetic plastic surgery. Although patients desiring lower eyelid blepharoplasty typically describe their problem as "bags in the lower eyelids," there are many anatomical imperfections that should be assessed. With aging, the youthful single convexity of the lower lid separates into a double convexity with a valley at the intersection of the lower lid and midface. Midface descent further drags this intersection inferiorly, leading to a vertically lengthened lower eyelid. This article discusses how to clinically evaluate lowe

Esthetic periodontal surgery for impacted dilacerated maxillary central incisors.

Clinicians do not frequently see impacted dilacerated maxillary incisors in their patients. When they do, there are several diagnostic and management challenges for correcting root dilacerations. An unfavorable esthetic outcome might occur as a result of soft-tissue complications during surgical eruption procedures. We present 2 patients with an impacted and dilacerated maxillary central incisor. Computed tomography scans with 3-dimensional reformation were used to accurately assess the positions of the dilacerated teeth, the degree of dilaceration, and the stage of root formation. The therapy

Interspecialty Variation in Perioperative Health Care Resource Usage for Carpal Tunnel Release.

BACKGROUND: We investigated whether any interspecialty variation exists, regarding perioperative health care resource usage, in carpal tunnel releases (CTRs). METHODS: The 2010 to 2021 PearlDiver Mariner Database, an all-payer claims database, was queried to identify patients undergoing primary CTRs. Physician specialty IDs were used to identify the specialty of the surgeon-orthopedic versus plastic versus general surgery versus neurosurgery. Multivariate logistic regression analysis was used to identify whether there was any interspecialty variation between the use of health care resources.

Efficacy of Manual Lymphatic Drainage in Reducing Postoperative Complications After Cosmetic Plastic Surgery: A Systematic Review.

BACKGROUND: Cosmetic plastic surgical procedures, such as liposuction and abdominoplasty, often disrupt the lymphatic system, leading to complications, including swelling, pain, fibrosis, and seromas. Manual lymphatic drainage (MLD) is commonly recommended postoperatively to enhance lymphatic flow and accelerate healing. However, evidence regarding its efficacy remains inconsistent, particularly in cosmetic procedures. This study aims to evaluate the effectiveness of MLD in reducing postoperative swelling, pain, fibrosis, seromas, and recovery time compared with standard care following cosmeti

[Therapeutic modalities in the treatment of palmar and axillary hyperhidrosis].

The authors summarize their therapeutic methods of palmar and axillary hyperhidrosis in this article. They discuss the characteristics, frequency of upper limb and patho-anatomical features of chronic sweating. Authors summarized their almost 40 years experience in treatment methods, technological improvements as well as they review recent knowledge and relevant literature. The open thoracotomic desympathization applied in the beginnings was gradually replaced by a minimally invasive VATS-sympathectomy (Video Assisted Thoracoscopic Surgery). The possibilities of conservative treatments, local

Simulation of Dural Repair in Minimally Invasive Spine Surgery With the Use of a Perfusion-Based Cadaveric Model.

BACKGROUND AND IMPORTANCE: In an era of curtailed work hours and concerns over achieving technical proficiency in the repertoire of procedures necessary for independent practice, many residencies have turned to model simulation as an educational adjunct. Cerebrospinal fluid (CSF) leak repair after inadvertent durotomy in spine surgery is a fundamental skillset for any spine surgeon. While primary closure with suture is not always necessary for small durotomies, larger defects, on the other hand, must be repaired. However, the dire consequences of inadequate repair dictate that it is generally

Tumescent anaesthesia.

Tumescent anaesthesia describes the practice of injecting a very dilute solution of local anaesthetic combined with epinephrine and sodium bicarbonate into tissue until it becomes firm and tense (tumescent). It was initially described in the field of liposuction but now surgical applications for the technique are widely varied ranging across vascular surgery, breast surgery, plastic surgery and ENT procedures. It is widely used in both hospital- and office-based environments and may form the sole method of anaesthesia for surgery. Advantages include a reduction in blood loss through both epine

Achieving gingival esthetics.

INTRODUCTION: Dentists traditionally have thought of periodontal treatment as a means of saving the teeth while leaving the patient with an esthetic problem. This no longer is true. The goal of this article is to show how esthetic crown-lengthening procedures, papillary regeneration and root coverage may enhance the overall esthetic results of periodontal treatment. METHODS AND RESULTS: Esthetic crown lengthening aims not only to provide biological width for the healthy restoration of teeth, but also to permit esthetic gingival and prosthetic contours. Papillary regeneration aims to fill the
