Homoharringtonine in Relapsed/Refractory Paediatric T-Cell Acute Lymphoblastic Leukaemia-A Case Series and a Report on the Use of In Vitro Drug Profiling.

Paediatric relapse/refractory T-cell acute lymphoblastic leukaemia (T-ALL) is notoriously difficult to treat. This group of heavily pre-treated patients needs effective agents that can rapidly control the disease while not having significant toxicity. Homoharringtonine (HHT) has been widely used in children with acute myeloid leukaemia, but there is little information on T-ALL. In this case series, HHT, in combination with other agents, achieved good response in five paediatric patients with T-ALL. The in vitro drug profiling in one patient demonstrated clinical correlation with HHT activity.

A Novel Liquid Chromatography-Tandem Mass Spectrometry Method for Monitoring Plasma l -Asparaginase Activity in Pediatric Patients With Acute Lymphoblastic Leukemia Receiving Pegaspargase Treatment.

BACKGROUND: Acute lymphoblastic leukemia (ALL) is the predominant malignancy in pediatric patients. As a crucial constituent of ALL chemotherapy, l -asparaginase is recognized as an integral element of treatment with a threshold concentration of 0.1 IU/mL used in treatment protocols. This study presents a novel liquid chromatography-tandem mass spectrometry method for evaluating plasma l -asparaginase activity in pediatric patients with ALL undergoing pegaspargase therapy. METHODS: Initially, an enzyme incubation was conducted using 20 μL of plasma and 100 μL of l -asparagine (0.1 mol/L) at 3

Avascular necrosis in older adolescents and adults with acute lymphoblastic leukemia treated with FRALLE protocols: a multicenter analysis of incidence, risk factors and morbidity.

Paediatric-inspired acute lymphoblastic leukaemia (ALL) regimens have led to improved leukaemic outcomes in adults. However, avascular necrosis (AVN) has emerged as a debilitating orthopaedic complication. AVN is likely multifactorial, with corticosteroids and asparaginase therapy playing key roles in its pathogenesis. We assessed the incidence and potential risk factors for AVN, in older adolescent and adult patients with ALL treated with the FRALLE-93 protocol across three Australian tertiary centres. AVN occurred in 24% of 59 patients, primarily affecting hip and knee joints. Diagnosis occu

Influence of xanthine oxidase and inosine monophosphate dehydrogenase polymorphisms on 6-mercaptopurine treatment response in pediatric acute lymphoblastic leukemia.

Over the past decades, survival outcomes for childhood acute lymphoblastic leukaemia (ALL) have significantly improved. However, adverse drug reactions (ADRs), particularly those related to 6-mercaptopurine (6-MP), remain a major concern. Myelosuppression associated with 6-MP administration can lead to infections or treatment interruptions. Excessive 6-thioguanine (6-TGN) levels worsen neutropenia, while elevated 6-methylmercaptopurine (6-MMP) levels contribute to hepatotoxicity. This study investigates genetic variants influencing 6-MP response in children with ALL. Seventeen tagSNPs in key e

Bone mineral density and vertebral fractures in teenage and young adult patients with acute lymphoblastic leukaemia and lymphoblastic lymphoma: A report from the British OsteoNecrosis Study (BONES).

The British Osteonecrosis Study (BONES) is the first multicentre prospective study assessing bone health and vertebral fractures in patients aged 10-24 in the United Kingdom undergoing treatment for acute lymphoblastic leukaemia (ALL) or lymphoblastic lymphoma (LBL). Sixty-one patients were recruited from three tertiary centres in the United Kingdom. Dual-energy X-ray absorptiometry (DXA) scans with vertebral fracture analysis were performed within 4 weeks of diagnosis and annually for 3 years. Subjective pain assessments were performed at the same time points. Bone mineral density (assessing

[Relationship between Peripheral Blood MiR-21 and Very Early Relapse after Chemotherapy in Children with Acute Lymphoblastic Leukemia].

OBJECTIVE: To analyze the relationship between microRNA-21 (miR-21) expression and the risk of very early relapse post-induction chemotherapy in children with acute lymphoblastic leukemia (ALL). METHODS: A total of 110 newly diagnosed children with ALL admitted to Huanggang Central Hospital from March 2020 to September 2022 were included. All patients received induction chemotherapy according to the CCLG-2008 protocol. The patients who achieved complete response (CR) after induction chemotherapy were followed up for 18 months, with very early relapse as the endpoint event. Then the patients w

Campylobacter jejuni bacteremia in a leukemic child: a nearly missed diagnosis.

Globally, Campylobacter spp. are responsible for most cases of bacterial gastrointestinal infections in humans and although rare, extraintestinal Campylobacter infections have been described. A 2-yearold neutropenic girl with underlying precursor B-cell acute lymphoblastic leukemia presented with a 3-day history of diarrhea. Her stool culture yielded no enteric bacterial pathogens. However, when her blood culture was flagged as positive for bacterial growth, no colonies could be observed on routine bacteriological isolation media. Nonetheless, gram-negative bacilli with seagull and spiral morp

Case report: sub-clinical extramedullary B-ALL in the setting of relapse following targeted therapy.

Standard testing for disease evaluation in B-cell acute lymphoblastic leukemia (B-ALL) includes examination of the bone marrow and cerebrospinal fluid. Radiographic or functional imaging are indicated when clinical signs of non-CNS extramedullary disease are present but are not standard in the relapsed/refractory setting. We describe two cases of patients with relapsed/refractory B-ALL with prior exposure to blinatumomab and/or inotuzumab ozogamicin presenting for CAR-T cell treatment. Both patients were thought to only have minimal residual disease (MRD) at the pre-CAR disease assessment, wit

Hepatic veno-occlusive disease with asparaginase products: a review of cases reported to the FDA adverse event reporting system and published in the literature.

Multiple asparaginase products have been approved by the United States (US) Food and Drug Administration (FDA) for the treatment of acute lymphoblastic leukemia in pediatric and adult patients. Hepatic veno-occlusive disease (VOD) is a potentially life-threatening disorder resulting from damage to the liver sinusoidal endothelial cells. To evaluate this safety concern with asparaginase (i.e. Asparlas, Oncaspar, Rylaze, and Erwinaze) use, we performed a postmarketing review of hepatic VOD reports retrieved from the FDA Adverse Event Reporting System database and literature with these four produ

Evaluating genomic polygenic risk scores for childhood acute lymphoblastic leukemia in Latinos.

The utility of polygenic risk score (PRS) models has not been comprehensively evaluated for childhood acute lymphoblastic leukemia (ALL), the most common type of cancer in children. Previous PRS models for ALL were based on significant loci observed in genome-wide association studies (GWASs), even though genomic PRS models have been shown to improve prediction performance for a number of complex diseases. In the United States, Latino (LAT) children have the highest risk of ALL, but the transferability of PRS models to LAT children has not been studied. In this study, we constructed and evaluat

Hydrocortisone to reduce dexamethasone-induced neurobehavioral side-effects in children with acute lymphoblastic leukaemia-results of a double-blind, randomised controlled trial with cross-over design.

BACKGROUND: Dexamethasone is a cornerstone of paediatric acute lymphoblastic leukaemia (ALL) treatment, although it can induce serious side-effects. Our previous study suggests that children who suffer most from neurobehavioural side-effects might benefit from physiological hydrocortisone in addition to dexamethasone treatment. This study aimed to validate this finding. METHODS: Our phase three, double-blind, randomised controlled trial with cross-over design included ALL patients (3-18 years) during medium-risk maintenance therapy in a national tertiary hospital between 17th May 2018 and 5th

CD43-positive, EWSR1::FLI1 -rearranged Soft Tissue Sarcoma in a Pediatric Patient With History of B-Cell Acute Lymphoblastic Leukemia.

Ewing sarcoma is a small round blue cell tumor typically characterized by an EWSR1 rearrangement and expression of CD99 and NKX2.2, without expression of hematopoietic markers such as CD45. CD43 is an alternative hematopoietic immunohistochemical marker often utilized in the workup of these tumors and its expression typically argues against Ewing sarcoma. We report a 10-year-old with history of B-cell acute lymphoblastic leukemia presenting with an unusual malignant shoulder mass with variable CD43 positivity, but with an EWSR1::FLI1 fusion detected by RNA sequencing. Her challenging workup hi

Either IL-7 activation of JAK-STAT or BEZ inhibition of PI3K-AKT-mTOR pathways dominates the single-cell phosphosignature of ex vivo treated pediatric T-cell acute lymphoblastic leukemia cells.

T-cell acute lymphoblastic leukemia (T-ALL) is an aggressive cancer arising from lymphoblasts of T-cell origin. While TALL accounts for only 15% of childhood and 25% of adult ALL, 30% of patients relapse with a poor outcome. Targeted therapy of resistant and high-risk pediatric T-ALL is therefore urgently needed, together with precision medicine tools allowing the testing of efficacy in patient samples. Furthermore, leukemic cell heterogeneity requires drug response assessment at the single-cell level. Here we used single-cell mass cytometry to study signal transduction pathways such as JAK-ST
