Criminalizing Serious Mental Illness: What Linked Urban Data Reveal and the Case for Community Care.

The United States has responded to multiple societal ills, from substance use to homelessness to mental health, with criminalization. A new analysis using City and County of San Francisco data shows jails disproportionately harm people with serious mental illness (SMI). In addition to longer periods of incarceration and higher rates of repeat jail bookings, people with SMI also experience higher rates of homelessness, co-occurring substance use disorder, and urgent and emergent health services utilization before and after incarceration compared to their peers without SMI. The study provides a

Post-release tuberculosis risk among formerly incarcerated populations in Lima Peru.

People who experience incarceration face a markedly elevated risk of tuberculosis (TB), yet the extent to which this vulnerability persists after release remains incompletely characterized. We analyze data from a prospective cohort study conducted in Lima, Peru, including 3,658 adults with newly diagnosed pulmonary TB and 6,335 household contacts aged 15-60. Recent incarceration (within the preceding five years) is reported by 187 (5%) index patients and 112 household contacts (1.7%). Compared with those without incarceration history, formerly incarcerated individuals are more likely to presen

Rationale, design, and methodology of a randomized controlled trial evaluating the impact of peer-enhanced care coordination on linkage to HCV care among formerly incarcerated individuals.

BACKGROUND: Hepatitis C Virus (HCV) disproportionately affects criminal legal-involved individuals (CLI), yet low treatment availability and high turnover in U.S. jails limits the number of individuals treated while incarcerated. HCV care coordination upon release is also an unmet need requiring improved models of care. This study will assess the feasibility and preliminary effectiveness of a peer-enhanced HCV care coordination intervention among individuals leaving jail. METHODS/DESIGN: We aim to enroll 80 current or formerly incarcerated CLI with current HCV infection. Participants will be

An investigation of drug use among first-time arrestees from 25 county jails across the United States in 2023.

BACKGROUND: Conducting research within a carceral health care context offers a unique view into the nature of drug use among arrestees with potential to identify and prevent drug use consequences. The purpose of this study was to characterize the nature and extent of drug use among first-time jail arrestees to inform detection and treatment. METHODS: This study utilized a naturalistic research design to collect de-identified urine drug screens (UDS), jail characteristics, and arrestee demographic variables among arrestees indicating drug use from 25 jails across the United States in 2023 thro

From foster care to incarceration: A prospective analysis of the National Youth in Transition Database.

BACKGROUND: Children in foster care often face significant adversity in adulthood, including a heightened risk of incarceration. Yet, it is uncertain whether adult incarceration rates differ between youth who age out of foster care, youth who are reunified with their families, and those adopted or placed with a legal guardian. OBJECTIVE: This prospective study investigates the prevalence of adult incarceration for youth in care at age 17 and examines whether the risk of incarceration varies by foster care exit type, both overall and among different racial/ethnic groups. PARTICIPANTS AND SETT

Mind the gap: a quality improvement study to determine health care value of rapid-access telemedicine addiction treatment in recently incarcerated individuals.

BACKGROUND: Previously incarcerated individuals experience a significant and vulnerable time post-release, highlighted by a greater than 129 times higher risk of overdose death. Many factors contribute to this high mortality including lack of health insurance and medications for opioid use disorder (MOUD) prescriptions upon release. Telemedicine provisions for MOUD greatly expanded access to MOUD for persons who previously could not engage in healthcare settings during critical times. Few rapid access, multidisciplinary transitional services exist to capture people in the immediate post-releas

Social support and trauma experiences of imprisoned men in Northern Ireland.

BACKGROUND: Social Support has multiple benefits for health and mental wellbeing. Its existence, and the extent to which it can be beneficial, is dependent upon the context in which it is provided, and the recipients' view of it. Social support has long been established as a 'buffer' to the negative impact of stressful life experiences. Trauma can negatively impact upon social support, reducing the extent of social networks and ability of some trauma experienced individuals to sustain extensive social support networks. However, some trauma experiences can also strengthen social relationships.

Implementation of Rapid COVID-19 Testing in Criminal Justice Residential Reentry Sites.

The goal was to study the implementation of rapid BINAX COVID-19 testing at criminal justice reentry sites, focusing on framework-guided implementation outcomes. We implemented rapid COVID-19 testing at nine reentry sites in four states (Massachusetts, New Hampshire, Rhode Island, and New York) and collected test results to measure the (1) adoptability and (2) implementability of COVID-19 testing at reentry sites. We collected data on the acceptability, appropriateness, and feasibility of the implementation of COVID-19 testing using an anonymous employee Qualtrics survey. Testing was available

Health and care utilization among youth with a history of parental incarceration and homelessness.

INTRODUCTION: Despite widespread recognition of the health and social risks posed by parental incarceration (PI) and homelessness, these challenges are rarely considered in unison. We sought to (a) assess the experiences of homelessness among youth with and without a history of PI and (b) compare the health and healthcare utilization among youth with a combined history of PI and homelessness. METHOD: Examining data from eighth-, ninth-, and 11th-grade public school participants in the 2019 Minnesota Student Survey (N = 110,904), we calculated univariate and multivariate analyses to characteri

Initial implementation of HIV pre-exposure prophylaxis for people who are incarcerated in Zambia: a cross-sectional observational study.

BACKGROUND: There are over 23 000 incarcerated people in Zambia, a population which has higher HIV prevalence than the general population yet has no access to HIV prevention. To evaluate the feasibility of HIV pre-exposure prophylaxis (PrEP) implementation in Zambian criminal justice facilities, we offered PrEP services to incarcerated people and aimed to describe early implementation outcomes. METHODS: In this cross-sectional observational study, we implemented a PrEP programme between Oct 1, 2020, and March 31, 2021, supporting 16 criminal justice facilities in four Zambian provinces. Carceral-community epidemiology, structural racism, and COVID-19 disparities.

Black and Hispanic communities are disproportionately affected by both incarceration and COVID-19. The epidemiological relationship between carceral facilities and community health during the COVID-19 pandemic, however, remains largely unexamined. Using data from Cook County Jail, we examine temporal patterns in the relationship between jail cycling (i.e., arrest and processing of individuals through jails before release) and community cases of COVID-19 in Chicago ZIP codes. We use multivariate regression analyses and a machine-learning tool, elastic regression, with 1,706 demographic control

Social Determinants of Health and Retention in HIV Care Among Recently Incarcerated Women Living with HIV in Canada.

Women living with HIV (WLWH) are over-represented in corrections in Canada, yet little is known about women's experiences post-release. We used CHIWOS cross-sectional data from WLWH to estimate associations between social determinants of health and HIV-related care outcomes among WLWH with recent (within past year) or ever (before past year) incarceration experience. Lifetime incarceration prevalence was 36.9% (6.5% recent; 30.4% ever), with significant differences by province of residence (British Columbia: 10% recent; 52% ever; Ontario: 5%; 24%; Quebec: 6%; 22%; p < 0.001). Incarceration and Number of Sexual Partners After Incarceration Among Vulnerable US Women, 2007-2017.

Objectives. To examine whether women's incarceration increases numbers of total and new sexual partners.Methods. US women with or at risk for HIV in a multicenter cohort study answered incarceration and sexual partner questions semiannually between 2007 and 2017. We used marginal structural models to compare total and new partners at visits not following incarceration with all visits following incarceration and visits immediately following incarceration. Covariates included demographics, HIV status, sex exchange, drug or alcohol use, and housing instability.Results. Of the 3180 participants, 1

Effects of incentivizing viral suppression in previously incarcerated adults living with HIV.

Background: The amount of HIV in a person's blood can be suppressed to an undetectable level through antiretroviral therapy medications (ART). Adhering to an ART regimen can improve a person's health and reduce HIV transmission. Despite these benefits, many people with HIV do not maintain the level of adherence required to achieve an undetectable viral load. This problem is particularly common among people who have been incarcerated. Objective: To determine effects of incentivizing viral suppression in previously incarcerated adults with HIV.Methods: Adults with HIV (N = 102) and detectable vir

Management of genitourinary foreign bodies in a predominantly incarcerated population.

UNLABELLED: INTRODUCTION Genitourinary foreign body (FB) insertion is a rare occurrence. Commonly reported reasons for insertion include autoeroticism and intoxication, however psychiatric illness is thought to contribute in most cases. In the incarcerated population, malingering plays a prominent role. We examined clinical patient characteristics and management patterns for cases of genitourinary FB insertion and sought to identify risk factors for recidivism. MATERIALS AND METHODS: A retrospective review was performed of all patients presenting to a tertiary trauma center with a genitourina
