The bidirectional effects of hemorrhoids, depression, and anxiety: a prospective cohort study in the UK Biobank.

BACKGROUND: This study examines the bidirectional association between hemorrhoidal disease and mental health conditions, specifically depression and anxiety. While a plausible connection exists, research on their interplay is limited. METHODS: Using data from the UK Biobank (until March 2024), this prospective study analyzed two cohorts: 48,620 participants in the hemorrhoids-to-mental disorder cohort and 87,310 in the mental disorder-to-hemorrhoids cohort. Analyses were adjusted for multiple sociodemographic, lifestyle, and clinical covariates, including age, sex, ethnicity, BMI, smoking sta

Association of TyG index with sepsis incidence and mortality: a prospective study with diabetes stratification.

BACKGROUND: The triglyceride-glucose (TyG) index, a validated surrogate marker of insulin resistance, has been widely investigated for its associations with cardiovascular outcomes in both diabetic and non-diabetic populations. However, its potential role in sepsis development remains largely unexplored. Moreover, whether the relationship between the TyG index and sepsis risk differs according to diabetes status has not been systematically examined. METHODS AND RESULTS: In this prospective cohort study, we analyzed 428,207 participants from the UK Biobank who had baseline TyG index measuremen

Improving the management and outcomes of preschool wheeze: protocol of a prospective multicentre cohort study.

INTRODUCTION: Preschool wheeze and asthma are associated with substantial morbidity and impaired future lung function. Yet, wheeze is unreliably reported with high disagreement (>50%) between parental and physician observations. Objectively defining wheeze and its reversibility could enable an earlier asthma diagnosis and improve preschool wheeze management. Our primary aim is to determine in preschool children (aged 0.5-6 years) suspected of asthma whether adding WheezeScan to routine clinical assessment (vs assessment without WheezeScan) improves the diagnosis of asthma. Our primary hypothesi

Evidence of immune-metabolic imbalance prior to sepsis: a prospective study in the UK Biobank.

BACKGROUND: Sepsis is a severe systemic inflammatory response syndrome, and effective metabolic signatures for predicting its risk are currently lacking. This study seeks to evaluate the associations between inflammatory vulnerability (IVX), metabolic malnutrition (MMX), and metabolic vulnerability (MVX) and the risk of sepsis. METHODS: We conducted a prospective cohort study using data from the United Kingdom Biobank. Cox proportional hazards models were utilized to examine the association between these composite indices and sepsis risk, while restricted cubic spline (RCS) analysis was appli

Gut microbial differences and function in infants with gastroschisis: a pilot prospective cohort study.

Newborns with gastroschisis hospitalised in the neonatal intensive care unit (NICU) are at risk for a disrupted gut microbiome. Infants with gastroschisis are particularly vulnerable to a dysbiotic microbiome; they require prolonged parenteral nutrition (PN) due to intestinal dysmotility, which often leads to growth faltering (GF). This pilot study's goals were to (1) compare the gut microbiome in infants with gastroschisis to infants admitted to the NICU without congenital anomalies, (2) identify differences in the gut microbiome between infants with gastroschisis requiring prolonged PN and t
