Urethra-sparing surgery for a prostate cancer lesion in the anterior urethral zone with magnetic resonance-guided focused ultrasound: a case report.

Prostate cancer (PCa) is the most common malignancy among men worldwide. High-intensity focused ultrasound (HIFU) is a focal therapeutic strategy for low- to intermediate-risk PCa with a low risk of complications. However, lesions located in the anterior urethral zone are challenging to treat with this approach because it is difficult to avoid urethral injury during HIFU therapy, which might further cause urethral stricture and symptoms related to bladder outlet obstruction (BOO). Here, we present the case of a 79-year-old male with elevated prostate-specific antigen (PSA) levels for over 1 ye…

Beyond the tumor: recurrence-prone radiomics for prognostication in negative PSMA PET/CT scans of prostate cancer.

Purpose.Prostate-specific membrane antigen (PSMA) PET/CT is routinely used to restage prostate cancer (PCa) in patients with biochemical recurrence (BCR), yet negative scans may still harbor subclinical disease. This study investigated whether radiomics features extracted from recurrence-prone organs on negative [18F]DCFPyL PET/CT can predict clinical progression (CP) and clinical progression-free survival.Materials and Methods. We performed a post-hoc analysis of 132 patients with BCR (mean age, 74.5 years) who had negative [18F]DCFPyL PET/CT scans and received no further treatment after imagi

Treatment Discontinuation in Metastatic Castration-Resistant Prostate Cancer (mCRPC) Patients Treated With Lutetium-177 (177Lu)-PSMA-617: A Retrospective Real-World Study.

BACKGROUND: Despite the efficacy of Lutetium-177 (177Lu)-PSMA-617 in Metastatic Castration-Resistant Prostate Cancer (mCRPC), high discontinuation rates limit its therapeutic potential. This study aimed to characterize discontinuation reasons, timing, predictive factors, and survival impact in a real-world cohort. METHODS: We retrospectively analyzed 208 mCRPC patients treated with ¹⁷⁷Lu-PSMA-617 between 2017 and 2024. Discontinuation was defined as cessation before 6 cycles. Reasons were categorized as disease progression, serious adverse events, patient-related factors, physician decision,

Outcome and Safety of Radioligand Therapy With [ 161 Tb]Tb-PSMA-617 After Re-Progression in mCRPC Patients Previously Treated With [ 177 Lu]Lu-PSMA-617.

INTRODUCTION: To enhance the effectiveness of radioligand therapy (RLT), new prostate-specific membrane antigen (PSMA) targeting radiopharmaceuticals utilizing alternative radionuclides are under active investigation. One promising alternative to the established 177 Lu is the use of 161 Tb. A key advantage of 161 Tb is its emission of a higher proportion of low-energy conversion and Auger electrons, which may contribute to enhanced therapeutic effectiveness. This study provides a first exploratory evaluation of the efficacy and safety of [ 161 Tb]Tb-PSMA-617 in patients with progression after

A Phase I Study of Hydroxychloroquine and Suba-Itraconazole in Men with Biochemical Relapse of Prostate Cancer (HITMAN-PC): Dose Escalation Results.

PURPOSE: Biochemical recurrence (BCR) of prostate cancer presents a clinical challenge with limited systemic treatment options beyond androgen deprivation therapy (ADT), which carries significant morbidity. Preclinical data suggest that lysosomal homeostasis, including cholesterol trafficking and pH regulation, is a therapeutic vulnerability in hormone-dependent cancers. We therefore conducted HITMAN-PC, a phase I trial evaluating suba-itraconazole (SI) and hydroxychloroquine (HCQ) in men with BCR. PATIENTS AND METHODS: The synergy of SI and HCQ was validated in hormone-sensitive and castrati
