A review based on expert opinions for robot-assisted simple prostatectomy for large benign prostatic hyperplasia.

OBJECTIVE: Robot-assisted simple prostatectomy (RASP) is increasingly used as a surgical treatment option for large benign prostatic hyperplasia (BPH) (>80 mL). However, there is no sufficient expert consensus or guidelines to guide clinical practice. We aimed to obtain expert opinions for RASP for large BPH. METHODS: A systematic review of the literature was performed in April 2024 using the PubMed, Embase, and Web of Science databases. Search terms were combined to construct the following search strings: (robotic) AND (simple OR benign) AND (prostatectomy). Search results were filtered by l

Robot-assisted reconstruction of posterior urethral stenosis: surgical techniques, graft use, and clinical outcomes.

Robot-assisted posterior bladder neck (BN) reconstruction is a key technique in complex urological surgery. The posterior BN presents unique challenges due to its narrow anatomical access and lack of surrounding spongiosum. Grafts enhance structural integrity in posterior BN reconstruction, particularly in cases of significant tissue loss, stenosis, or scarring. These challenges require precise graft placement and tissue handling, which we have outlined the best techniques within the existing literature in this review. This article provides a detailed review of robot-assisted posterior BN reco

Safety and feasibility of solo surgery using a novel robotic platform: an IDEAL 2a development study.

BACKGROUND: Minimally invasive surgery is the standard of care for general surgical procedures. While use continues to grow, current platforms are hindered by prolonged operative times, high costs, steep learning curves, and lack of surgeon autonomy, which restricts expansion and practicality in high-volume and ambulatory surgery centers. Our goal was to evaluate the safety and feasibility of a novel AI-powered robotic platform to perform solo surgery. METHODS: An IDEAL framework 2a prospective, multi-center observational study was performed on consecutive patients planned for solo abdominal

A Rare Case of Iatrogenic Left Atrial Roof Injury by a Guidewire During Robotic Mitral Valvuloplasty.

Minimally invasive cardiac surgery enhanced by the daVinci robotic system offers benefits such as reduced morbidity and superior visualization. However, complications like unexpected bleeding may require conversion to median sternotomy, necessitating careful intraoperative manipulation. We report a rare case of left atrial roof injury during robotic mitral valvuloplasty in a 61-year-old woman with severe mitral regurgitation. Cardiopulmonary bypass was established via femoral and jugular vein cannulation. Upon initiation, bleeding was identified from a wire perforation at the left atrial roof.

Robotic distal pancreatectomy: a comparative systematic review and meta-analysis of classic, Kimura, and Warshaw techniques.

Distal pancreatectomy (DP) remains a technically challenging procedure associated with substantial morbidity. With the advent of robotic surgery, three primary techniques have emerged: robotic classic distal pancreatectomy (RCDP; including splenectomy), the vessel-preserving Kimura technique (KT), and the vessel-sacrificing Warshaw technique (WT). These approaches differ in splenic vessel management, leading to varying perioperative risks and long-term splenic outcomes. We performed a comprehensive meta-analysis comparing these techniques across multiple clinical endpoints. We systematically s

Strategies for preventing port-site hernia following robot-assisted radical prostatectomy: a systematic review.

BACKGROUND: Trocar site hernia is a recognized but often underreported complication of minimally invasive surgery, including robotic-assisted radical prostatectomy. While relatively rare, trocar site hernia can lead to severe complications such as bowel obstruction, strangulation, and the need for emergency surgical intervention. Trocar size has been identified as a primary risk factor, with hernias occurring predominantly at sites where 10 mm or larger trocars are used. However, the role of fascial closure is still debated. METHODS AND RESULTS: A systematic literature review (1992-2022) iden

Robotic repair of off-midline abdominal wall hernias: a single institution consecutive case series.

INTRODUCTION: While off-midline hernias represent only a small percentage of abdominal wall defects, symptoms can be debilitating for the affected patients, and repair portends substantial operative challenges for the surgeon. There are no large-scale case series that describe outcomes with extraperitoneal repair using the robotic approach. The objective of this study was to describe our experience of patients undergoing robotic repair of off-midline hernias at a tertiary care medical center. METHODS: This study was a retrospective review of patients who underwent elective robotic repair of o

Severe Intra- and Post-Operative Lactic Acidosis in a Patient Who Underwent Robotic Thoracoscopic Surgery.

Background/Objectives: Lactic acidosis is one of the most common causes of metabolic acidosis in hospitalized patients. It happens when lactic acid production exceeds lactic acid clearance. The elevation of lactate was commonly improved after the restoration of tissue perfusion. However, there are rare cases of severe lactate elevation (greater than 8 mmol/L) in the intraoperative period of thoracoscopic surgery. A poor prognosis with high morbidity and mortality characterizes these cases. Case Description: A 72-year-old man was admitted to the Soroka University Medical Center for thoracoscopi

Iatrogenic Intrathoracic Insufflation During Robot-Assisted Laparoscopic Paraoesophageal Hernia Repair with Toupet Fundoplication.

Minimally invasive surgery, including robotic-assisted laparoscopic procedures, has largely replaced open surgery due to its numerous benefits. However, this approach introduces unique complications such as intraoperative pneumothorax, particularly during upper gastrointestinal (GI) surgeries near the diaphragmatic hiatus. We report the case of a 50-year-old male patient undergoing robotic paraesophageal hernia repair with Toupet fundoplication who developed a tension pneumothorax intraoperatively. Clinical signs included rising peak airway pressures, progressive hypoxemia, and absent right-si
