Does Thoracodorsal Pedicle Dissection Affect Upper-Extremity Lymphatic Drainage? Arm Circumference After Free Latissimus Dorsi Muscle Flap Harvest in Patients Without Axillary Lymphadenectomy.

Background: The free latissimus dorsi (LD) muscle flap remains a workhorse for large soft-tissue defects. Its pedicle, however, must be followed proximally into the axilla-into the immediate vicinity of the lymphatics that drain the arm-and whether this compromises upper-extremity drainage has never been settled. The few studies that examined lymphoedema after LD surgery relied on questionnaires and were confined to breast reconstruction, where axillary clearance and irradiation offer a sufficient explanation for anything they found; the harvest itself has never been isolated. Methods: We iden

AI-augmented pathology: the experience of transfer learning and intra-domain data diversity in breast cancer metastasis detection.

BACKGROUND: Metastatic detection in sentinel lymph nodes remains a crucial prognostic factor in breast cancer management, with accurate and timely diagnosis directly impacting treatment decisions. While traditional histopathological assessment relies on microscopic examination of stained tissues, the digitization of slides as whole-slide images (WSI) has enabled the development of computer-aided diagnostic systems. These automated approaches offer potential improvements in detection consistency and efficiency compared to conventional methods. RESULTS: This study leverages transfer learning on…

Is less more? Revisiting elective nodal irradiation in head and neck cancer.

PURPOSE: Radiotherapy is an important pillar of treatment for patients with locally advanced head and neck squamous cell carcinoma (HNSCC) i … n after induction chemotherapy or dose reduction depending on hypoxia measured by FMISO-PET scan or sentinel lymph nodes measured by SPECT-CT after 99mTc-nanocolloid injection around the primary tumor. All these phase II trials indicate that the recurrence rate outside the radiation field is low (up to 4%). Most recurrences occur in high-risk regions of the clinical …

[Potential role of alveolar macrophages in HIV persistence and lung disease].

While antiretroviral therapy (ART) has revolutionized the management of human immunodeficiency virus (HIV) and has enabled people living with HIV (PLWH) to achieve near-normal life expectancies, an HIV cure remains elusive due to the presence of HIV reservoirs. Furthermore, compared with individuals in the general population, PLWH support a higher burden of multimorbidity, including pulmonary diseases of both an infectious and non-infection nature, which may be a consequence of the formation of HIV reservoirs. Their gut, lymph nodes, brain, testes and lungs constitute important anatomic sites

Detection of CK-19 mRNA-positive cells in the peripheral blood of breast cancer patients with histologically and immunohistochemically negative axillary lymph nodes.

BACKGROUND: To investigate the incidence of direct hematogenous spread of cancer cells in patients with early-stage breast cancer by studying the presence of occult tumor cytokeratin-19 (CK-19) mRNA(+) cells in the peripheral blood in relation to the status of sentinel (SLNs) and (ALNs) axillary lymph nodes. PATIENTS AND METHODS: SLNs and ALNs from 111 patients with operable stage I-II breast adenocarcinoma were evaluated for the presence of tumor cells by hematoxylin-eosin (H&E) staining and, if negative, by immunohistochemistry (IHC) using an anti-CK-19 antibody. Peripheral blood was also a

Comparative Evaluation of Cytological and Histopathological Diagnostic Performance in Detecting Nodal Metastasis of Canine Mast Cell Tumours.

Cytological evaluation of regional lymph nodes (LNs) is routinely used for staging canine mast cell tumours (MCTs), but its diagnostic accuracy requires further validation. This multicentre retrospective study compared Krick's cytological criteria with Weishaar's histopathological classification for detecting LN metastasis in canine MCTs, and evaluated whether cytology with modified Wright-Giemsa stain (WGS) could outperform routine haematoxylin-eosin (H&E) histology in identifying early metastatic LNs. Paired cytological and histopathological evaluations were performed on 65 LNs from 52 dogs

A rare case of long-term paraesthesia diagnosed as a paraneoplastic syndrome by anti-SOX1 antibody determination.

Paraneoplastic syndromes (PS) are a rare presentation of cancer, most commonly associated with small cell lung cancer (SCLC), breast cancer and haematologic malignancies. The diagnosis of PS is challenging because it could affect multiple organ systems and it may present before the tumour is visible by imaging. We report a malignant tumour diagnosed in a male patient who referred long-term paraesthesia and proximal muscle strength loss. After ruling out common causes of polyneuropathy, the anti-SOX1 antibody gave light to the diagnosis. A pulmonary opacity in the upper right lobe was observed

A comparison of the diagnostic accuracy of magnetic resonance imaging to axillary ultrasound in the detection of axillary nodal metastases in newly diagnosed breast cancer.

Patients with a diagnosis of invasive breast cancer are increasingly undergoing breast magnetic resonance imaging (MRI) for preoperative staging including evaluation of axillary lymph node metastases (ALNM). This retrospective study aims to evaluate the utility of adding axillary ultrasound (AUS) in the preoperative setting when an MRI is planned or has already been performed. This IRB approved, HIPAA compliant study reviewed a total of 271 patients with a new diagnosis of invasive breast cancer at a single institution, between June 1, 2010 and June 30, 2013. The study included patients who re

Management of the neck in metastatic cutaneous squamous cell carcinoma of the head and neck.

PURPOSE OF REVIEW: This article reviews the predictors, prognosis, and treatment of nodal metastases in cutaneous squamous cell carcinoma of the head and neck. RECENT FINDINGS: There is a better understanding of the risk factors that predict metastatic disease in cutaneous squamous cell carcinoma. Recent data has simplified the relationship between the site of the primary tumour and nodal disease, allowing for a more selective approach to the neck, in terms of both surgery and radiotherapy. Newer staging systems that take into account the clinicopathological factors allow better prognosticati
