Comparison between hook plate fixation with and without coracoclavicular ligament suture for acute acromioclavicular joint dislocations.

BACKGROUND: Hook plates are widely used for repair of acromioclavicular joint (ACJ) dislocations. However, it is unclear whether repair of torn coracoclavicular (CC) ligament is necessary. The purpose of this study was to evaluate the outcomes of the hook plate fixation with direct CC ligament repair for acute ACJ dislocation in comparison with the hook plate fixation without direct CC ligament repair. METHODS: The study included 120 patients with acute ACJ dislocations who underwent surgery. The patients were divided into 73 patient groups with Arbeitsgemeinschaft für Osteosynthesefragen (AO

Effect of local anesthetic volume (20 mL vs 30 mL ropivacaine) on electromyography of the diaphragm and pulmonary function after ultrasound-guided supraclavicular brachial plexus block: a randomized controlled trial.

BACKGROUND AND OBJECTIVES: Diaphragmatic paralysis following supraclavicular brachial plexus block (SCBPB) is ascribed to phrenic nerve palsy. This study investigated the effect of 2 volumes of 0.375% ropivacaine on efficacy of block as a surgical anesthetic and as an analgesic and examined diaphragm compound muscle action potentials (CMAPs) and pulmonary function before and after SCBPB. METHODS: Eighty patients scheduled for removal of hardware for internal fixation after healing of an upper limb fracture distal to the shoulder were randomized to receive ultrasound-guided SCBPC for surgical

Evaluating gestational weight gain recommendations in pregestational diabetes.

OBJECTIVE: The Institute of Medicine (IOM) does not provide recommendations for gestational weight gain (GWG) specific to women with pregestational diabetes. We aimed to assess the impact of GWG outside the IOM recommendations on perinatal outcomes. STUDY DESIGN: We performed a retrospective cohort study of all singletons with pregestational diabetes from 2008 through 2013. Women were classified as GWG within, less than, or greater than IOM recommendations for body mass index per week of pregnancy. Maternal outcomes examined were cesarean delivery, preeclampsia, and percentage of visits with

Isokinetic evaluation of pronation after volar plating of a distal radius fracture.

INTRODUCTION: Pronator quadratus (PQ) is an important contributor to forearm pronation, and there is concern that volar plating of a distal radius fracture (DRF) may damage the PQ function. The purpose of this study was to determine whether isokinetic pronation strength would decrease considerably after volar locking plating of a DRF, and whether clinical outcomes would be affected by any pronation strength decrease. MATERIALS AND METHODS: Thirty-four patients of mean age of 55 years (range, 21-74 years) underwent bilateral isokinetic testing at 6 months and 1 year after open reduction and in

A systematic evaluation of musculoskeletal injuries in mixed martial arts athletes: a meta-analysis.

INTRODUCTION: Mixed martial arts (MMA) is a popular combat sport including wrestling, boxing, and martial arts, which involves danger and violence. It is reported to have one of the greatest injury rates among different sports, limits participation, and often consists of considerable medical costs. The available literature on MMA injuries is sparse and requires a comprehensive review of injuries in this popular sport. This systematic review and meta-analysis was conducted with the aim of critically evaluating the epidemiological literature on injury patterns, mechanisms, and associated risk fa

In-House 3D Planning for Corrective Osteotomy of the Distal Radius: Prospective Cohort Study With 1-Year Radiological and Clinical Follow-Up.

BACKGROUND: Distal radius malunions occur in 5% to 17% of fractures and may lead to pain and functional impairment. While 3D surgical planning with patient-specific guides has demonstrated high accuracy and reduced fluoroscopy use compared with conventional 2D techniques, its routine clinical implementation remains limited, particularly for in-house hospital-based workflows. OBJECTIVE: This study aimed to evaluate the precision of corrective osteotomy for distal radius malunions using in-house-designed 3D patient-specific surgical guides 1 year after surgery. Secondary objectives were to asse

Ulnar Epiphysiodesis for Distal Radius Physeal Arrest: Comparing Early Detection to Late Presentation Management.

BACKGROUND: Premature physeal arrest of the distal radius (PPAR) is a rare but serious complication following pediatric fractures that may result in ulnar impaction syndrome. The purpose of this study was to compare outcomes following distal ulnar epiphysiodesis between those identified without deformity (early cohort) and those identified with deformity (late cohort). The hypothesis was that treating children before deformity development would improve patient-reported outcomes (PROs). METHODS: All patients younger than 18 years of age who underwent ulnar epiphysiodesis due to PPAR from 2011

Neonatal and Maternal Outcomes following Shoulder Dystocia Resolution Utilizing ≥ versus < 3 Maneuvers.

Most shoulder dystocia (SD) cases do not have associated adverse outcomes. The objective was to assess whether SD relieved with ≥3 maneuvers, compared with fewer, is associated with a higher likelihood of adverse outcomes. The secondary objective was to examine if postpartum hemorrhage is associated with SD managed with ≥3 maneuvers versus fewer. This was a secondary analysis of the assessment of perinatal excellence (APEX) study, an observational cohort of over 115,000 deliveries in 25 U.S. hospitals from 2008 to 2011. We included individuals with singleton, vertex, and nonanomalous fetuses at

Carpal bone replacement using personalized 3D printed tantalum prosthesis.

Objective: Scaphoid and lunate fractures have a relatively high incidence rate. Traditional carpectomy and carpal arthrodesis in the treatment of carpal osteonecrosis will lead to many complications. Three-dimensional (3D) printed tantalum has good biocompatibility and can be designed to match the patient's personalized anatomical carpal structure. This study aims to investigate carpal function and prosthesis-related conditions after carpal bone replacement using 3D printed tantalum prostheses. Methods: From July 2020 to January 2022 at our center, seven patients with osteonecrosis of the carp
