Vulnerability of women in southern Africa to infection with HIV: biological determinants and priority health sector interventions.

OBJECTIVE: To review biomedical determinants of women's vulnerability to infection with HIV and interventions to counter this, within the southern African context. RESULTS: Apart from number of exposures, if any, several factors influence the efficiency of HIV transmission during sex. Acute HIV infection, with extraordinarily high semen viral load, in conjunction with concurrent partnerships maximizes this efficiency. Delaying sexual debut and avoiding HIV exposure among biologically and socially vulnerable youth is critical. Reducing unintended pregnancies keeps girls in school and prevents

Treatment of comorbidity in conduct disorder with attention-deficit hyperactivity disorder (ADHD).

Conduct disorder (CD) is one of the most common psychiatric disorders in childhood and adolescence. It is characterized by a variety of chronic antisocial behaviors, a repetitive and persistent pattern of behavior that violates the basic rights of others, major age-appropriate societal norms, or both. Aggressive behavior, lying, stealing, fire-setting, and running away from home and school are the most frequent manifestations of CD and are often accompanied by hyperactivity, impulsive behavior, explosiveness, cognitive and learning problems, and poor social skills. The rate of comorbidity is h

Mental retardation in teenagers: prevalence data from the Niagara region, Ontario.

OBJECTIVES: There are few Canadian prevalence studies of mental retardation (MR); those that do exist were conducted prior to the era of community integration. We undertook a population-based study to explore mental health disturbances in young persons with MR. The first requirement was to identify a population with MR and to establish its prevalence. Here, we report data on the prevalence of MR in a population aged 14 to 20 years. METHOD: We conducted the study in the Niagara Region of Ontario, which has a population base of around 400,000. Researchers worked closely with schools and with ag

[Imaging of atherosclerosis: magnetic resonance].

Atherosclerosis is the disease with the most important social impact in terms of cost and human life. Its study in the prevention and cure of its consequences needs integration using many imaging modalities, and magnetic resonance presents advantages due to its particular characteristics. The Authors make an overview of its indications and potential applications in the assessment of atherosclerotic disease. In vitro studies have demonstrated that it is possible to evaluate the normal and abnormal components of vessel walls by means of magnetic resonance imaging (MRI) as well as fatty depositio

Work dynamics in family care of hemophilic children.

School-aged boys with hemophilia, their parents, and siblings were interviewed and observed over a 5-year period, the field work taking place in their homes, schools, and during hospitalization, clinic visits, and summer camp periods. Sequential, open-ended interviews, and participant-observation techniques enabled the inquiry to study the process of the children's development as a major factor in changes that occurred in the management of their illness over time. Qualitative issues identified for study focused on the divisions of labor inherent in family interactions concerning the care of th

Community Perspectives and Behavior Change Strategies for Integrated Long-Acting Reversible Contraception and Long-Acting Injectable Preexposure Prophylaxis Roll Out for Adolescent Girls and Young Women in Uganda (COMPASS): Protocol for a Mixed Methods Implementation Science Study.

BACKGROUND: HIV incidence among adolescent girls and young women (AGYW) aged 15-24 years in Eastern and Southern Africa remains unacceptably high. Unprotected sex increases the risk of HIV and other sexually transmitted infections and unintended pregnancies, reflecting an unmet need for effective contraception among AGYW. Long-acting injectable preexposure prophylaxis (LAI-PrEP) and long-acting reversible contraception (LARC) offer effective, durable protection against HIV and unintended pregnancy. However, uptake and persistence remain suboptimal due to low awareness, fear of side effects, st

Caregiver burden in Parkinson's disease: a mixed-methods study.

BACKGROUND: Providing informal care for a person with Parkinson's disease (PD) can be a demanding process affecting several dimensions of a caregiver's life and potentially causing caregiver burden. Despite the emerging literature on caregiver burden in people with PD, little is known about the inter-relationship between quantitative and qualitative findings. Filling this knowledge gap will provide a more holistic approach to develop and design innovations aiming at reducing or even preventing caregiver burden. This study aimed to characterize the determinants of caregiver burden among informa

Patient Perspectives on Open-Door Policies in Psychiatry: Mixed Methods Study.

BACKGROUND: Open-door policies in psychiatric wards are increasingly recommended as a means to reduce coercion and enhance patient autonomy. However, evidence that integrates patient perspectives on ward openness and related safety measures remains limited. Traditional qualitative approaches often lack the breadth to fully capture the complexity of these views. We hypothesized that patients would prefer open-door treatment and hold a critical view of locked-ward environments, emphasizing autonomy and dignity in care. OBJECTIVE: This study aims to systematically explore psychiatric patients' p

Precision Prevention of Premature Cardiovascular Disease Among African Men: The Triple-Risk Convergence Framework Integrating Psychosocial, Metabolic, and Inherited Cardiovascular Susceptibility.

Premature cardiovascular disease (CVD) among African men is an escalating public health and health systems challenge across sub-Saharan Africa. Rapid urbanisation, psychosocial stress, cardiometabolic transition, and inherited cardiovascular susceptibility converge within contexts characterised by delayed healthcare engagement and fragmented preventive infrastructure. Current prevention strategies remain largely centred on metabolic risk factors and may inadequately capture the multidimensional nature of cardiovascular vulnerability. The objective of this conceptual paper is to develop and pre

Understanding physical exercise among individuals with substance use disorders using an integrated theoretical perspective of the health action process approach and theory of planned behavior.

INTRODUCTION: Physical exercise is considered a useful non-pharmacological adjunctive treatment for promoting recovery from substance use disorders (SUD). However, adherence to physical exercise treatments is low, and little is known about what factors are associated with the initiation and maintenance of physical exercise behaviors. The aim of this study was to explore the psychosocial factors underlying these behaviors in individuals with SUD using an integrated theoretical model based on the health action process approach (HAPA) and the theory of planned behavior (TPB). METHODS: A total of

To what extent is equity entrenched in HIV/AIDS-related policy documents in Ethiopia? A policy content analysis.

INTRODUCTION: It is important to assess the extent of mentions of equity in policy and strategic documents for several reasons. First, it helps us understand the root causes of inequities, revealing both failures and successes. Second, it shows the readiness of stakeholders and leaders to take actions towards equity. Third, it identifies areas of improvement to plan strategies towards equity. The objective of this study was to assess the degree to which equity is incorporated in human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS)-related policy documents in Ethiopia.

Assessing health advocacy competencies through a novel simulation OSCE-based educational framework.

Health advocacy remains one of the least explicitly assessed CanMEDS roles in Emergency Medicine. Objective structured clinical examination (OSCE) simulation is used to assess performance in real time but has not previously incorporated social determinants of health. We developed and piloted a simulation-based OSCE, embedding advocacy within acute clinical care and introduced novel assessment tools, completed by both examiners and standardized patients. Here we report the feasibility and acceptability of this OSCE and assessment tools. Twenty-two residents completed the advocacy and standard c

Faecal microbiota transplantation and glucolipid metabolic disorders: the interventional role of gut microbiota.

Glucolipid metabolic disorders (GLMD) have emerged as a significant global public health issue, posing a significant threat to human health. With changes in modern social structures and an ageing population, the incidence of GLMD is on the rise and is increasingly affecting younger populations. Faecal microbiota transplantation (FMT) directly modifies the gut microbiota to reestablish its equilibrium and metabolites, consequently reinstating gut barrier integrity, mitigating chronic low-grade inflammation, and affecting the onset and progression of GLMD through the regulation of the gut-liver

Artificial Intelligence as Companionship: A Systemic and Relational Examination Using Empirical Data From Psyhelp.

Artificial intelligence (AI) is increasingly used as a source of emotional support, reflection, and perceived companionship. Despite growing public and clinical interest in AI companionship, little research has examined how individuals construct these experiences within broader relational systems. Drawing on systemic, attachment, and social constructionist perspectives, this study examines AI companionship using anonymized data from Psyhelp, an AI-assisted mental health platform. A qualitative-dominant mixed methods design integrated thematic analysis of user narratives with engagement pattern

Stop pushing junk science! User corrections of misinformation in science are common and sophisticated.

Misinformation is often equated with blatant falsehoods from dubious sources, overlooking that scientific sources, too, can accidentally publish inaccurate information to large online audiences. This article explores how social media users share, evaluate and address misinformation in science, defined as unreliable research rejected by relevant experts for violating scientific integrity. Using a mixed-methods analysis of Twitter discussions on retracted COVID-19 studies (33,730 tweets and 13,743 replies), the study found that, while most users accepted retracted science at face value, they wer

Rebuilding Lives: The Intersection of Reintegration and Psychological Distress after Incarceration for Genocide.

This article explores the reentry and reintegration of Rwandans who were incarcerated for genocide. Previous research indicates that these individuals are experiencing psychological distress, and we analyze the determinants of this distress in this study. Specifically, we assess three waves of longitudinal data following 185 Rwandans whom we interviewed before their release and approximately four months and one year afterward. We find that psychosocial distress generally improved after completing one's sentence. Such improvement can be explained through securing a livelihood, (re)establishing

Health Equity: Foundation of Serious Illness Care.

Health equity is foundational to the delivery of high-quality serious illness care. Despite advances in palliative care, substantial disparities persist in symptom burden, quality of life, caregiver distress, access to specialty services, and end-of-life outcomes across racial, ethnic, socioeconomic, geographic, and other structurally marginalized populations. These inequities are driven not by individual patient characteristics alone, but by structural, institutional, and social factors including discrimination, language discordance, disability, transportation barriers, housing instability, f

Cubical Model of Intelligence: Phenomenological and Cognitive Approach to Seven Aspects of Mind.

This paper presents the Cubic Model of Intelligence — a phenomenological and cognitive approach to understanding seven interrelated aspects of the mind: logic, emotion, intuition, memory, ethics, social context, and temporal foresight. The model is derived from observation of internal thinking processes and proposes to view these aspects not as separate functions but as facets of a unified cognitive structure, whose interactions shape human understanding and decision-making. In this study, the cube is not treated as a symbolic image but as a theoretical hypothesis about the structural organizat

Distance, Disparity, and Duty: An Empirical Bioethical Analysis of Adolescent Sexual and Reproductive Health Access in Rural Uganda.

Disparities in adolescent sexual and reproductive health (SRH) access in rural Uganda are commonly attributed to geographic distance, resource scarcity, and service availability. These explanations, while important, remain ethically incomplete unless they also account for duty: the social and economic responsibilities that constrain adolescents' capacity to prioritise health needs. Drawing on empirical findings from a study of 408 adolescents aged 13-19 years in the Bugweri and Iganga districts of eastern Uganda, this paper integrates Rawlsian distributive justice and the capability approach t
