Effects of non-target leg activation, TMS coil orientation, and limb dominance on lower limb motor cortex excitability.

Transcranial magnetic stimulation (TMS) is used to examine corticospinal tract integrity after stroke, however, generating motor-evoked potentials (MEPs) in the lower limb (LL) can be difficult. Previous studies have used activation of the target leg to facilitate MEPs in the LL but this may not be possible after stroke due to hemiplegia. The dominance of the target limb may also be important, however the neurophysiological effects of LL dominance are not known. We investigated whether voluntary activation of the non-target leg combined with optimal TMS coil orientation increases corticomotor …

Corticospinal excitability underlying digit force planning for grasping in humans.

Control of digit forces for grasping relies on sensorimotor memory gained from prior experience with the same or similar objects and on online sensory feedback. However, little is known about neural mechanisms underlying digit force planning. We addressed this question by quantifying the temporal evolution of corticospinal excitability (CSE) using single-pulse transcranial magnetic stimulation (TMS) during two reach-to-grasp tasks. These tasks differed in terms of the magnitude of force exerted on the same points on the object to isolate digit force planning from reach and grasp planning. We a…

Motor cortical activity during motor tasks is normal in patients with complex regional pain syndrome.

UNLABELLED: Motor dysfunction in complex regional pain syndrome (CRPS) is often considered a functional movement disorder. Earlier studies i … ched for age and sex, and patients whose hand was immobilized to treat a scaphoid fracture. We used transcranial magnetic stimulation to measure corticospinal excitability at rest and during motor imagery (explicit motor task) and motor observation (implicit motor task). Motor corticospinal excitation measured at rest and during implicit and explicit motor tasks was similar for CR …

Alterations of motor cortical excitability and anatomy in Unverricht-Lundborg disease.

Unverricht-Lundborg disease is the most common form of progressive myoclonus epilepsies. In addition to generalized seizures, it is characte … ty healthy controls. The symptoms were evaluated with the Unified Myoclonus Rating Scale. Navigated transcranial magnetic stimulation was applied to characterize the excitability of the primary motor cortex by determining the motor thresholds and cortical silent periods. In addition, the induced cortical electric fields were estimated using individual scalp-to-cortex distances mea …

Patterns of cortical hyperexcitability in adolescent/adult-onset generalized epilepsies.

PURPOSE: To investigate whether using transcranial magnetic stimulation (TMS) to derive if measures of cortical excitability changes can distinguish between various adolescent/adult-onset generalized epilepsy syndromes at different phases of the disorder. METHODS: One hundred thirty-seven patients with adolescent/adult-onset generalized epilepsy divided into juvenile myoclonic epilepsy, juvenile absence epilepsy, and generalized epilepsy with tonic-clonic seizures only were studied. The cohorts were further divided into drug naive-new onset, refractory, and seizure-free groups. Motor threshol…

TMS-EEG reveals impaired intracortical interactions and coherence in Unverricht-Lundborg type progressive myoclonus epilepsy (EPM1).

Unverricht-Lundborg disease (EPM1) is an inherited neurodegenerative disorder, and the most common form of progressive myoclonus epilepsies. Its main symptoms, epileptic seizures and drug-resistant myoclonus, may be associated with neurophysiological evidence of abnormal cortical excitability or reduced inhibition. The aim of the present study was to utilize transcranial magnetic stimulation (TMS) to induce cortical responses measured with electroencephalography (EEG) in order to observe prevailing cortical excitability/inhibition changes, as well as power and coherence of the cortical oscilla…

Left dorsal premotor cortex and supramarginal gyrus complement each other during rapid action reprogramming.

The ability to discard a prepared action plan in favor of an alternative action is critical when facing sudden environmental changes. We tested whether the functional contribution of left supramarginal gyrus (SMG) during action reprogramming depends on the functional integrity of left dorsal premotor cortex (PMd). Adopting a dual-site repetitive transcranial magnetic stimulation (rTMS) strategy, we first transiently disrupted PMd with "off-line" 1 Hz rTMS and then applied focal "on-line" rTMS to SMG while human subjects performed a spatially precued reaction time (RT) task. Effective on-line r…

Abnormal acute changes in upper limb muscle cortical representation areas in the patients with writer's cramp during co-activation of distal and proximal muscles.

AIM: We analysed cortical muscle representation areas during single muscle activation and during the co-activation of several upper arm muscles in the patients with writer's cramp to determine the possible occurrence of abnormal dynamic somatotopic changes in M1, in addition to the static map abnormalities already described in this form of dystonia. METHODS: Using transcranial magnetic stimulation, we assessed cortical representations of medial deltoid, extensor carpi radialis and the first dorsal interosseus muscles in eight patients with writer's cramp and in eight healthy control subjects.…

Corticospinal excitability in patients with anoxic, traumatic, and non-traumatic diffuse brain injury.

BACKGROUND: Transcranial magnetic stimulation (TMS) have been frequently used to explore changes in motor cortex excitability in stroke and traumatic brain injury, while the extent of motor cortex reorganization in patients with diffuse non-traumatic brain injury remains largely unknown. OBJECTIVE/HYPOTHESIS: It was hypothesized that the motor cortex excitability would be decreased and would correlate to the severity of brain injury and level of functioning in patients with anoxic, traumatic, and non-traumatic diffuse brain injury. METHODS: TMS was applied to primary motor cortices of 19 pat…

Impaired interhemispheric inhibition in writer's cramp.

OBJECTIVES: Reduced cortical inhibition is a feature of focal hand dystonia and this likely contributes to excessive muscle contractions. Inhibition from the opposite hemisphere, known as interhemispheric inhibition (IHI), was studied bidirectionally in 7 right-handed patients with writer's cramp (WC) and age-matched healthy controls in a cross-sectional physiologic study. METHODS: IHI was measured with paired transcranial magnetic stimulation with the conditioning stimulus applied to the motor cortex and the test stimulus applied to the contralateral motor cortex. Surface EMG was measured in…

Reversal of cortical reorganization in human primary motor cortex following thumb reconstruction.

Deafferentation such as the amputation of a body part causes cortical reorganization in the primary motor cortex (M1). We investigated whether this reorganization is reversible after reconstruction of the lost body part. We tested two patients who had long-standing thumb amputations followed by thumb reconstruction with toe-to-thumb transfer 9 to 10 mo later and one patient who underwent thumb replantation immediately following traumatic amputation. Using transcranial magnetic stimulation, we measured the motor evoked potential (MEP) threshold, latency, short-interval intracortical inhibition …

Effects of posture-related changes in motor cortical output on central oscillatory activity of pathological origin in humans.

Changes in shoulder position influence motor cortical outflow to Abductor Digiti Minimi (ADM) muscle in healthy humans. We examined whether these changes may affect finger tremor of central origin. Subjects had their shoulder positioned in two different configurations: 30 degrees horizontal adduction (ANT) and 30 degrees horizontal abduction (POST) with respect to neutral position at 0 degrees in the horizontal plane. In healthy subjects, patients with Parkinsonian tremor (PT) and essential tremor (ET), transcranial magnetic stimulation (TMS) of the motor cortex was performed under resting and…

Short intracortical and surround inhibition are selectively reduced during movement initiation in focal hand dystonia.

In patients with focal hand dystonia (FHD), pathological overflow activation occurs in muscles not involved in the movement. Surround inhibition is a neural mechanism that can sharpen desired movement by inhibiting unwanted movement in adjacent muscles. To further establish the phenomenon of surround inhibition and to determine whether short intracortical inhibition (SICI) reflecting inhibition from the local interneurons in primary motor cortex (M1), might play a role in its genesis, single- and paired-pulse transcranial magnetic stimulation (TMS), and Hoffmann reflex testing were applied to …

Concurrent action observation modulates practice-induced motor memory formation.

Motor practice is associated with the formation of elementary motor memories. Here we tested in human subjects the hypothesis that observation of a motor training associated with physical practice will modulate the encoding process of a motor memory relative to physical practice alone. Voluntary thumb motions were practiced (i) alone in a direction opposite to the baseline direction of transcranial magnetic stimulation (TMS)-evoked movements (physical practice, PP) and in combination with observation of synchronous movements that were either (ii) directionally congruent (same direction, PP + A…

The relationship between cortical inhibition, antipsychotic treatment, and the symptoms of schizophrenia.

BACKGROUND: Cortical inhibition (CI) deficits assessed by transcranial magnetic stimulation paradigms, short-interval cortical inhibition (SICI), and cortical silent period (CSP) have been demonstrated in schizophrenia (SCZ) patients. Antipsychotic treatments can modify CI and improve clinical symptoms, suggesting a neurophysiological link between the two. Previous studies have demonstrated that clozapine is associated with prolongation in CSP, a gamma-hydroxybutyric acid (GABA)(B)-mediated phenomenon. Furthermore, SICI deficits were associated with psychotic symptom severity, suggesting alter…
