Incontinence in patients with dementia.

The high prevalence of incontinence in dementia sufferers will only increase as the population ages (Alzheimer's Society, 2007), but the evidence-based knowledge for management of these long-term disorders combined is lacking (Hägglund, 2010). Management techniques for incontinence need to be developed to ensure that dementia patients receive the best care, as current methods such as behavioural techniques may not be appropriate for people with limited cognitive function. This article will address issues that arise with current incontinence management for dementia sufferers and possible course

Preventing deconditioning and incontinence in older people in acute care settings.

Hospital-associated deconditioning is a recognised and well-known condition. It refers to a loss of physiological fitness and functional ability in a patient resulting from prolonged immobility, inactivity or bed rest during a period of hospitalisation. Many patients who experience deconditioning also have new or existing continence issues, although the link between deconditioning and incontinence is not well understood. This article explores deconditioning and incontinence among older people in acute care settings. It outlines the multifactorial links between the two conditions and reviews cu

Review/Perspective: Operations for Cauda Equina Syndromes - "The Sooner the Better".

BACKGROUND: Although most studies recommended that early surgery for cauda equina syndromes (CES) be performed within <48 h., the largest and most comprehensive Nationwide Inpatient Sample Database (NISC) series, involving over 25,000 CES patients recommended that time be shortened to 0-<24 h. In short, CES surgery performed "the sooner the better," was best. METHODS: The 2 major variants of CES include; incomplete/partial ICES, and those with urinary retention/bowel incontinence (RCES). Those with ICES often exhibit varying combinations of motor weakness, sensory loss (i.e. including perinea

Five-year outcomes of the transection of synthetic suburethral sling tape for treating obstructive voiding symptoms after transobturator sling surgery.

OBJECTIVE: To evaluate the outcomes of sling transection in treating obstructive voiding symptoms after transobturator sling surgery and the recurrence rates of stress urinary incontinence after 5-year follow-up. MATERIAL AND METHODS: Between January 2004 and March 2006, we retrospectively reviewed 461 patients who underwent the transobturator sling procedure. Of 461 patients, 28 patients (6.1%) underwent transection of suburethral sling tape under local anesthesia and followed up for at least 5 years after sling transection. We analyzed the overall success rates and recurrence rates of stres…

Effects of transobturator tape procedure on female sexual function at 2-year follow-up: A prospective cohort study with matched control group.

OBJECTIVES: We aimed to compare the effect of elapsed time on sexual function in women who underwent a transobturator tape (TOT) procedure for stress urinary incontinence (SUI) with continent controls. METHODS: Urinary and sexual function of 70 females were assessed preoperatively at month 6 and postoperatively at month 24 in the study group. Forty-five patients without urinary incontinence and demographically matched with the study group were assessed for sexual function at first administration and 24 months later in the control group. The Female Sexual Function Index (FSFI) was used. RESUL…

Does visceral peritoneal closure affect post-cesarean urinary symptoms? A randomized clinical trial.

INTRODUCTION AND HYPOTHESIS: We hypothesized that upward bladder traction by visceral peritoneal closure during cesarean sections may have an impact on postpartum urinary complaints. METHODS: Based on a 90% power of the study and a 95% confidence interval, a sample size of 114 patients in each arm was needed to detect a 15% difference between both groups regarding postpartum urinary incontinence. To account for follow-up losses, we prospectively randomized 620 term primigravidas undergoing non-emergency cesareans into two groups (310 each): group 1, visceral peritoneal closure; group 2, non-c…

Urethral mobility and its relationship to stress incontinence in women.

Multiple physiologic factors, only one of which is urethral support, make up the female continence mechanism. Abnormal urethral support leads to deficient transmission of abdominal pressure to the urethra, which results in genuine stress incontinence in some women. Many techniques are available for measuring urethral mobility. The determination of urethral mobility is not useful in the diagnosis of urinary incontinence. However, it provides some information about which surgical procedure is most appropriate for the correction of genuine stress incontinence.

Diagnosis and treatment of voiding dysfunction caused by urethral obstruction after anti-incontinence surgery.

Voiding dysfunction after surgical treatment of stress incontinence may occur as a result of urethral obstruction secondary to hyperelevation of the bladder neck. The diagnosis of urethral obstruction after anti-incontinence surgery is often based upon patient symptomatology and physical examination without confirmation by urodynamic, endoscopic or other diagnostic studies. Urethrolysis seems effective in treating urethral obstruction and voiding dysfunction after surgical treatment of stress urinary incontinence. The reported high success rates of less-invasive procedures have not been confir…

[Intravesical migration of a sub-urethral tension-free vaginal tape (TVT) sling: about a case and review of the literature].

Sub-urethral support techniques using synthetic sling are the current gold standard in the treatment of stress urinary incontinence. We report the case of a 60-year-old female patient with a personal history of stress urinary incontinence treated with suburethral TVT sling in 2008. Intraoperative cystoscopy was normal. Six years later the patient had recurrent urinary tract infections caused by Escherichia Coli, resistant to all appropriate antibiotics. The patient underwent ultrasound which objectified intravesical stone measuring 2.5 cm on the major axis; post-void residual urine volume was …

Suspected congenital urethral diverticulum in a dog.

A 2-year-old neutered male dog with a history of urinary incontinence, recurrent urinary tract infections, and unilateral cryptorchidism was presented with an acute onset of perineal swelling. Urinary contrast studies revealed a urothelial lined structure in the perineum. Surgical resection of 80% to 90% of this structure, suspected to be a congenital urethral diverticulum, was successful.

Does the Kaufman prosthesis still have a place? Review of thirteen years' experience.

The cases of 57 patients who underwent implantation of a Kaufman prosthesis for urinary incontinence at this center are reviewed. All except one of the patients were incontinent following prostatic surgery. Although many of the patients required revision or adjustments after the initial implantation, 55 percent of the cases eventually had a satisfactory outcome. Patients rendered incontinent after retropubic prostatectomy had a higher success rate with the Kaufman prosthesis than patients after other types of prostatectomy. This simple and relatively inexpensive device can still play a useful …

An unusual complication of Burch colposuspension.

Burch colposuspension remains one of the successful operations for genuine stress incontinence. We report a patient who developed an intravesical foreign body granuloma post-Burch colposuspension. Any patient developing unexplained lower urinary tract symptoms following bladder or pelvic surgery for incontinence must be evaluated endoscopically in order to exclude this complication.
